Scheduling Exams through CDA
(Students eligible for Exam Accommodations)
Students, during high volume times, it is best to complete this Exam Form to schedule your exams. You will receive a confirmation email once your exam is scheduled.
Reminder, CDA cannot proctor same day or next day exams.
Faculty Exam Information
Faculty must submit exam dates through the Faculty Portal as soon as possible after receiving accommodation notifications so that CDA can reserve proctoring locations. The actual exams do not need to be submitted until two days prior to the exam date.
The Center for Disability & Access is Moving!
Beginning August 5th, all student appointments (except testing) will be held virtually until our move to a permanent location in the Student Services Building. You may visit us in our temporary office in the Student Services Building, Basement Level, Room 65.

During our office relocation, we ask that students schedule ALL exams and quizzes taken with CDA as early in the semester as possible. Exams will be proctored by CDA in the usual manner. Check your email for Exam locations and information.
