Objective To identify the etiology and current clinical characteristics of male urethral strictur... more Objective To identify the etiology and current clinical characteristics of male urethral stricture disease (USD) in Senegal. Patients and methods A retrospective and mono-centric study over a 10-year period (between January 1999 and December 2008) was carried out and included all male patients with USD admitted to the Urology Department of Aristide Le-Dantec Hospital (Dakar). Results We recorded 414 USD cases. The maximum annual incidence was observed in 2004, and from then, the incidence tended to decrease gradually. The mean age of patients was 43.7 ± 19.6 years (1–89 years), and 43% of patients were younger than 40 years old. The mean time from symptoms onset to the consultation was 20.1 ± 24.9 months (1–120 months). Most patients (42.2%) were admitted because they suffered dysuria. In 57.8% of patients, USD was diagnosed with complications. The most common site of stricture was at the bulbar urethra (72.7%). The length of the urethral stricture was smaller than 1 cm in 59.6% of patients, between 1 and 3 cm in 28.5% and between 3 and 5 cm in 7.7%. The stricture was longer than 5 cm in eight patients and was staged (multifocal) in nine patients. The etiology of urethral stricture was infectious in 63% of patients, traumatic in 13.7% and iatrogenic in 8.2%. The etiology was not identified in 14.9% of cases. The infectious etiology was urethritis in 259 cases and urogenital schistosomiasis in 12 cases. A study of the etiology according to stricture site showed that bulbar USD was infectious in 77.7% of cases and that all membranous USD cases were traumatic. Conclusion Male urethral stricture in Senegal is often a sequela of gonococcal urethritis and is frequently detected because of infectious complications such as gangrene of the genitalia or urinary tract infections. Its prevention in this geographical region thus requires fighting against sexually transmitted infections and treating appropriately and effectively all urethritis.

Patients et méthodes Étude rétrospective colligeant 61 cas de varicocèle dont 46 (75,4 %) ayant une oligozoospermie sévère (groupe I) et 15 (24,6 %) ayant une azoospermie non obstructive (groupe II). Résultats Dans le groupe I (oligozoospermie sévère), l’âge moyen des patients était de 35,5 ± 6,4 ans (23–47 ans). La durée moyenne de l’infertilité était de 4,9 ± 3,4 ans (1–13 ans). La varicocèle était bilatérale chez 41 patients (89,1 %) et unilatérale gauche chez cinq patients (10,9 %). Cette varicocèle a été classée de grade I dans deux cas (4,3 %), de grade II dans 39 cas (84,7 %) et de grade III dans cinq cas (10,9 %). En préopératoire, la concentration moyenne des spermatozoïdes était de 1,85 ± 1,4 millions/ml. La mobilité normale à la première heure des spermatozoïdes était en moyenne de 43,3 ± 21,5 %. Les spermatozoïdes de morphologie anormale représentaient 65,05 ± 21,6 % en moyenne. Après varicocélectomie, la concentration moyenne des spermatozoïdes était de 8,3 ± 10,3 millions/ml. La mobilité normale était en moyenne de 47,6 ± 29,2 %, et les spermatozoïdes de morphologie anormale de 50,08 ± 26,9 %. Après un recul moyen de 26,2 ± 11,6 mois, 12 couples (26,1 %) avaient rapporté des grossesses spontanées. Conclusion La varicocélectomie chez les hommes infertiles ayant une azoospermie non obstructive peut entraîner une apparition dans l’éjaculat de spermatozoïdes mobiles utilisables dans le cadre d’une assistance médicale à la procréation (AMP). Elle entraîne une amélioration significative de la qualité de la spermatogenèse et de la fertilité en cas d’oligozoospermie sévère. La réalisation de cette intervention chez de tels hommes est donc une option raisonnable. Conclusion Retroperitoneal varicocele repair resulted in spermatogenesis induction with presence of motile ejaculated spermatozoa for some men with non-obstructive azoospermia. It induced spermatogenesis and fertility enhancement in men with severe oligozoospermia. Varicocele repair should be considered in men with non-obstructive azoospermia or severe oligozoospermia.

L’exploration chirurgicale avait permis de retrouver une masse funiculaire surplombant le testicule, masse dont l’analyse histologique était revenue en faveur d’une funiculite pseudotumorale. À travers l’analyse de ce cas et une revue de la littérature, nous allons discuter des aspects diagnostiques et thérapeutiques de ces tumeurs paratesticulaires rares. The authors report the first case of pseudotumor of the spermatic cord in a patient presenting with an ectopic testis. On surgical exploration, a funicular mass overriding the testis was found, and histologic study of the specimen showed a pseudotumoral funiculitis. Through the analysis of this case and a review of the literature, we shall discuss the diagnostic and therapeutic features of rare paratesticular tumors.

Résultats L’âge moyen de nos patients était de 31,04 ± 8,4 ans (extrêmes 18 et 55 ans). Les principales circonstances de découverte ont été l’infertilité masculine (38 cas), la vacuité des bourses (21 cas) et la dégénérescence maligne du TND (six cas). La cryptorchidie était unilatérale dans 69,5 % et bilatérale dans 30,4 % des cas. Le spermogramme effectué chez 60 de nos patients a montré une azoospermie dans 46,6 % des cas, une oligozoospermie dans 38,3 % et une numération de spermatozoïdes normale dans 15 % des cas. Tous les patients ayant une cryptorchidie bilatérale avaient une azoospermie. À l’exploration chirurgicale, le TND a été retrouvé dans 95,6 % des cas (66 patients). Le principal type de traitement réalisé a été l’abaissement testiculaire. L’histologie des pièces d’orchidectomie sur testicule non tumoral (n = 7) a trouvé dans tous les cas une atrophie testiculaire sévère avec absence complète de cellules germinales. Les types histologiques des cancers étaient le séminome (n = 3) et le carcinome embryonnaire (n = 3). Parmi les patients ayant une azoospermie (n = 28), aucun cas d’induction de la spermatogenèse après abaissement testiculaire n’a été observé. Parmi les patients ayant consulté pour infertilité masculine, trois d’entre eux ont eu des enfants après abaissement testiculaire, soit un taux de paternité de 7,8 %. Conclusion Dans la cryptorchidie de l’adulte, l’abaissement du testicule a peu d’impact sur la fertilité masculine. Mais, vu les progrès actuels de l’assistance médicale à la procréation (AMP), le dogme d’orchidectomie systématique devant un TND diagnostiqué à l’âge adulte doit être reconsidéré. The histological type of testicular cancer was seminoma (three cases) and embryonic carcinoma (three cases). In azoospermic men (28 cases) no induction of spermatogenesis was achieved after orchidopexy. In infertile patients, the improvement of sperm count was seen in three patients with unilateral cryptorchidism. Three patients with unilateral crytorchidism achieved pregnancy (pregnancy rate of 7.8%). Conclusion Orchidopexy for adult cryptorcidism had little impact in male fertility. Because of the risk of testicular cancer, orchidectomy was recommended as treatment of unilateral cryptorchidism. But with the recent development of TESE, orchidopexy appears as a reasonable treatment of adult cryptorchidism.

Objectif Étudier les aspects diagnostiques et thérapeutiques de la torsion du cordon spermatique ... more Objectif Étudier les aspects diagnostiques et thérapeutiques de la torsion du cordon spermatique (TCS) dans notre pratique quotidienne et faire une revue de la littérature. Patients et méthodes Étude rétrospective de janvier 2002 à décembre 2009 sur tous les cas de suspicion de TCS reçus en urgence. Résultats La TCS était confirmée chez 58 patients après scrototomie exploratrice. L’âge moyen des patients était de 21 ans (avec des extrêmes de 1 et de 44 ans) et 48 patients (83 %) étaient âgés de plus de 15 ans. Le délai moyen entre le début de la symptomatologie douloureuse et l’arrivée aux urgences était de 102 heures (avec des extrêmes de 2 et de 480 heures); 47 patients ont été reçus après la sixième heure et 19 patients ont été référés d’une structure périphérique. La torsion était supravaginale chez cinq sujets âgés de plus de 15 ans et était sanctionnée d’une orchidectomie chez 30 patients (52 %). Conclusion Notre série est caractérisée par un taux élevé de castration d’où l’intérêt de mettre l’accent sur la sensibilisation des populations qui doivent venir consulter en urgence devant une grosse bourse douloureuse aiguë. Objective To study the diagnostic and therapeutic features of testicular torsion in our daily practice, and to compare our results with that of the existing literature. Patients and methods A retrospective study was conducted from January 2002 to December 2009 on all patients who presented in emergency with suspicion of testicular torsion. Results Testicular torsion was confirmed in 58 patients after scrotal exploration. The average age was 20 years (range, 1–44 years), and 48 patients (83%) were more than 15 years old. The average duration from time of onset of pain to arrival at the emergency department was 102 hours; 47 patients (81%) were received after the sixth hour and 19 (33%) were referred from peripheral health facilities. Torsion was supravaginal in 5 patients, all more than 15 years old; orchidectomy was performed in 30 patients (52%). Conclusion In our study, we have a high proportion of orchidectomy. To reduce this, it will be important to sensibilize population to go to the hospital when they have cases of testicular pain and edema.
To study current epidemiological and clinical features of adult renal cancer in Senegal and the e... more To study current epidemiological and clinical features of adult renal cancer in Senegal and the evolution of these features over the two past decades. We conducted a retrospective ten years study (2000-2009) that analyzed all the renal cell cancer in adult admitted in the urology department and the pathology department of the university teaching hospital Aristide-Le-Dantec (Dakar). The results of this study were compared with those of the previous decade. We included 74 cases of renal cell cancer. The median age of patients was 49 years (18-72 years). There was a slight female predominance (51.3%). Twenty-eight (37.8%) patients presented at least with one renal cell cancer risk factor. The median duration of symptoms was 10 months (1-96 months). The localization of the renal cancer was right in 42 patients (56.7%) and left in 32 cases (43.8%). Almost all the tumors were symptomatic. Symptoms were dominated by loin pain (87.8%) and abdominal mass (77%). There were only two cases (2.7%) of incidental renal cell cancer. The median tumor size was 12 cm (2.4-26 cm). The more frequent tumor stages according to the TNM 2002 staging system were T2 (39.2%) and T3 (33.7%). Metastases were found in 23 (31%) patients. Forty-four patients underwent nephrectomy (43 radical and one partial). No adjuvant treatment or metastasectomy were done. The main histological subtype of tumors was renal cell carcinoma (47 cases). The mean duration of the follow up was 30.5 ± 33.6 months. Among the 44 patients who underwent nephrectomy, no tumor recurrence was found on 35 cases. Tumor recurrence occurred on nine patients. The specific mortality rate was 47.3%. Epidemiological and clinical features of the adult renal cell cancer in Senegal haven't significantly changed over the 20 past years. The adult renal cell cancer incidence was low in Senegal. Its clinical profile was characterized by a predominance of locally advanced and metastatic forms.

Conclusion Retroperitoneal varicocele repair resulted in spermatogenesis induction with presence of motile ejaculated spermatozoa for some men with non-obstructive azoospermia. It induced spermatogenesis and fertility enhancement in men with severe oligozoospermia. Varicocele repair should be considered in men with non-obstructive azoospermia or severe oligozoospermia.

L’exploration chirurgicale avait permis de retrouver une masse funiculaire surplombant le testicule, masse dont l’analyse histologique était revenue en faveur d’une funiculite pseudotumorale. À travers l’analyse de ce cas et une revue de la littérature, nous allons discuter des aspects diagnostiques et thérapeutiques de ces tumeurs paratesticulaires rares. The authors report the first case of pseudotumor of the spermatic cord in a patient presenting with an ectopic testis. On surgical exploration, a funicular mass overriding the testis was found, and histologic study of the specimen showed a pseudotumoral funiculitis. Through the analysis of this case and a review of the literature, we shall discuss the diagnostic and therapeutic features of rare paratesticular tumors.

Patients et méthodes Il s’agit d’une étude rétrospective colligeant 69 patients âgés de plus de 18 ans pris en charge au service d’urologie-andrologie du CHU Aristide-Le-Dantec pour cryptorchidie entre janvier 1999 et décembre 2007. Résultats L’âge moyen de nos patients était de 31,04 ± 8,4 ans (extrêmes 18 et 55 ans). Les principales circonstances de découverte ont été l’infertilité masculine (38 cas), la vacuité des bourses (21 cas) et la dégénérescence maligne du TND (six cas). La cryptorchidie était unilatérale dans 69,5 % et bilatérale dans 30,4 % des cas. Le spermogramme effectué chez 60 de nos patients a montré une azoospermie dans 46,6 % des cas, une oligozoospermie dans 38,3 % et une numération de spermatozoïdes normale dans 15 % des cas. Tous les patients ayant une cryptorchidie bilatérale avaient une azoospermie. À l’exploration chirurgicale, le TND a été retrouvé dans 95,6 % des cas (66 patients). Le principal type de traitement réalisé a été l’abaissement testiculaire. L’histologie des pièces d’orchidectomie sur testicule non tumoral (n = 7) a trouvé dans tous les cas une atrophie testiculaire sévère avec absence complète de cellules germinales. Les types histologiques des cancers étaient le séminome (n = 3) et le carcinome embryonnaire (n = 3). Parmi les patients ayant une azoospermie (n = 28), aucun cas d’induction de la spermatogenèse après abaissement testiculaire n’a été observé. Parmi les patients ayant consulté pour infertilité masculine, trois d’entre eux ont eu des enfants après abaissement testiculaire, soit un taux de paternité de 7,8 %. Conclusion Dans la cryptorchidie de l’adulte, l’abaissement du testicule a peu d’impact sur la fertilité masculine. Mais, vu les progrès actuels de l’assistance médicale à la procréation (AMP), le dogme d’orchidectomie systématique devant un TND diagnostiqué à l’âge adulte doit être reconsidéré. Cryptorchidism was unilateral in 69.5% and bilateral in 30.4% of cases. Semen analyses were done for 60 patients and showed azoospermia in 46.6%, oligozoospermia in 38.3% and a normal sperm count in 15% of cases. In all bilateral cryptorchidism cases, semen analyses showed azoospermia. At surgery, the undescended testis was found in 66 cases (95.6%) and orchidopexy was the most done surgical procedure. Seven patients (without testicular cancer) underwent unilateral orchiectomy and histology of specimens showed sertoli-cell-only syndrome with no spermatogenesis in all cases. The histological type of testicular cancer was seminoma (three cases) and embryonic carcinoma (three cases). In azoospermic men (28 cases) no induction of spermatogenesis was achieved after orchidopexy. In infertile patients, the improvement of sperm count was seen in three patients with unilateral cryptorchidism. Three patients with unilateral crytorchidism achieved pregnancy (pregnancy rate of 7.8%). Conclusion Orchidopexy for adult cryptorcidism had little impact in male fertility. Because of the risk of testicular cancer, orchidectomy was recommended as treatment of unilateral cryptorchidism. But with the recent development of TESE, orchidopexy appears as a reasonable treatment of adult cryptorchidism.

Objectif Étudier les aspects diagnostiques et thérapeutiques de la torsion du cordon spermatique ... more Objectif Étudier les aspects diagnostiques et thérapeutiques de la torsion du cordon spermatique (TCS) dans notre pratique quotidienne et faire une revue de la littérature. Patients et méthodes Étude rétrospective de janvier 2002 à décembre 2009 sur tous les cas de suspicion de TCS reçus en urgence. Résultats La TCS était confirmée chez 58 patients après scrototomie exploratrice. L’âge moyen des patients était de 21 ans (avec des extrêmes de 1 et de 44 ans) et 48 patients (83 %) étaient âgés de plus de 15 ans. Le délai moyen entre le début de la symptomatologie douloureuse et l’arrivée aux urgences était de 102 heures (avec des extrêmes de 2 et de 480 heures); 47 patients ont été reçus après la sixième heure et 19 patients ont été référés d’une structure périphérique. La torsion était supravaginale chez cinq sujets âgés de plus de 15 ans et était sanctionnée d’une orchidectomie chez 30 patients (52 %). Conclusion Notre série est caractérisée par un taux élevé de castration d’où l’intérêt de mettre l’accent sur la sensibilisation des populations qui doivent venir consulter en urgence devant une grosse bourse douloureuse aiguë. Objective To study the diagnostic and therapeutic features of testicular torsion in our daily practice, and to compare our results with that of the existing literature. Patients and methods A retrospective study was conducted from January 2002 to December 2009 on all patients who presented in emergency with suspicion of testicular torsion. Results Testicular torsion was confirmed in 58 patients after scrotal exploration. The average age was 20 years (range, 1–44 years), and 48 patients (83%) were more than 15 years old. The average duration from time of onset of pain to arrival at the emergency department was 102 hours; 47 patients (81%) were received after the sixth hour and 19 (33%) were referred from peripheral health facilities. Torsion was supravaginal in 5 patients, all more than 15 years old; orchidectomy was performed in 30 patients (52%). Conclusion In our study, we have a high proportion of orchidectomy. To reduce this, it will be important to sensibilize population to go to the hospital when they have cases of testicular pain and edema.
To study current epidemiological and clinical features of adult renal cancer in Senegal and the e... more To study current epidemiological and clinical features of adult renal cancer in Senegal and the evolution of these features over the two past decades. We conducted a retrospective ten years study (2000-2009) that analyzed all the renal cell cancer in adult admitted in the urology department and the pathology department of the university teaching hospital Aristide-Le-Dantec (Dakar). The results of this study were compared with those of the previous decade. We included 74 cases of renal cell cancer. The median age of patients was 49 years (18-72 years). There was a slight female predominance (51.3%). Twenty-eight (37.8%) patients presented at least with one renal cell cancer risk factor. The median duration of symptoms was 10 months (1-96 months). The localization of the renal cancer was right in 42 patients (56.7%) and left in 32 cases (43.8%). Almost all the tumors were symptomatic. Symptoms were dominated by loin pain (87.8%) and abdominal mass (77%). There were only two cases (2.7%) of incidental renal cell cancer. The median tumor size was 12 cm (2.4-26 cm). The more frequent tumor stages according to the TNM 2002 staging system were T2 (39.2%) and T3 (33.7%). Metastases were found in 23 (31%) patients. Forty-four patients underwent nephrectomy (43 radical and one partial). No adjuvant treatment or metastasectomy were done. The main histological subtype of tumors was renal cell carcinoma (47 cases). The mean duration of the follow up was 30.5 ± 33.6 months. Among the 44 patients who underwent nephrectomy, no tumor recurrence was found on 35 cases. Tumor recurrence occurred on nine patients. The specific mortality rate was 47.3%. Epidemiological and clinical features of the adult renal cell cancer in Senegal haven't significantly changed over the 20 past years. The adult renal cell cancer incidence was low in Senegal. Its clinical profile was characterized by a predominance of locally advanced and metastatic forms.
