Elle étudie en particulier le rôle joué par l’assurance complémentaire sur l’accès aux soins, la mutualisation des dépenses de santé et le bien-être de la population. Les travaux menés dans cette thèse révèlent l’importance de l’assurance complémentaire pour accéder à des soins reportés dans le temps pour raisons financières. Ils montrent en revanche que, généraliser l’assurance complémentaire, dans le modèle actuel de co-financement des soins, ne permet ni de répondre à des objectifs d’équité ni-même d’améliorer le bien-être de la population. Thus, they partially reveal their health risks. Yet, on the French health insurance market this hypothesis, known as 'adverse-selection', has only been tested on the supplementary health insurance purchase decision. However, the supplementary health insurance market is extremely heterogeneous, at least in the same way as beneficiaries' health risk levels. It covers health care and medical goods expenditures not covered by the Statutory Health Insurance scheme, also referred to as out-of-pocket (OOP) payments. Between 1980 and 2008, OOP payments increased from 217 € to 547 € per person per year, at comparative price levels2 (EcoSanté, Health Expenditure Accounts 2009). Two factors explain this increase. In France, the Ani reform mandates all private sector employers to offer sponsored Complementary ... more In France, the Ani reform mandates all private sector employers to offer sponsored Complementary Health Insurance (CHI) to all of their employees beginning on January 1 st , 2016. If this mandate may reduce the cost of CHI coverage for employees, it may also prevent them choosing their optimal level of coverage given their health care needs, their income and their risk preferences. Furthermore, as employees are on average in good health status, the mandate is going to deteriorate the health risk of the pool of insured covered by individual policies, which may increase premiums. Welfare of individuals not affected by the reform (as retired and long term unemployed) may thus decrease. Wages may also potentially decrease by the employer subsidy amount. This research simulates the likely effects of this employer CHI mandate on the social welfare of the population making the most likely scenarios on the increase in individual policies premiums and the decrease in wages. Lors du passage a la retraite, de nombreux assures changent d'organisme complementaire sante ... more Lors du passage a la retraite, de nombreux assures changent d'organisme complementaire sante : c'est le cas de 51 % des titulaires d'un contrat d'entreprise obligatoire, 39% des titulaires d'un contrat d'entreprise facultatif et 23% des titulaires d'un contrat individuel. Les changements plus frequents parmi les personnes ayant un contrat collectif - a fortiori un contrat obligatoire - sont confirmes apres avoir pris en compte d'autres caracteristiques pouvant expliquer cette mobilite, notamment l'etat de sante. Ce resultat reflete l'impact, lors du passage a la retraite, de la hausse des cotisations pour les titulaires d'un contrat collectif, hausse principalement due a la perte de la tarification de groupe et de la participation de l'employeur. The higher mobility rates observed among group contract holders – especially those with a compulsory contract – can be confirmed after controlling for other characteristics that may explain this greater mobility, in particular their health status. This reflects the impact of the increase in group contract premiums for recently retired people, an increase that is primarily due to the loss of both group pricing and the employer’s contribution. Their greater mobility may also mean that cover designed for a group of employees does not meet retirees’ health care needs. Ainsi, la generalisation a tous les Francais de l’acces a une complementaire sante de qualite se trouve au cœur de la strategie nationale de sante enoncee en 2013 par le gouvernement.
