Adenosine agonists have now been shown by several laboratories to have profound neuroprotective e... more Adenosine agonists have now been shown by several laboratories to have profound neuroprotective effects when administered either pre- or postischemia. In an effort to determine whether these effects are centrally mediated, the effects of the non-brain-permeable adenosine receptor antagonist 8-sulfophenyl-theophylline (8-SPTH) on cyclohexyladenosine (CHA) -mediated protection was determined. Both survival and neurologic outcome were assessed in gerbils following 30 minutes of bilateral carotid occlusion. A dose of 2 mg/kg of CHA 5 minutes postreperfusion resulted in highly significant increases in survival relative to saline injected controls. Administration of doses of 8-SPTH sufficient to normalize the hypotension observed with CHA resulted in the same degree of postischemic protection. Similar results were obtained when neurologic status was evaluated. The results indicate that the neuroprotective effects of CHA are apparently centrally mediated.

A 10-min long ischemic insult followed by up to 60-min survival results in several changes of the... more A 10-min long ischemic insult followed by up to 60-min survival results in several changes of the synaptic ultrastructure in the hippocampal CA-1 region. The alternations consist of gradual change of synaptic curvature from neutral to positive, cleavage and decrease in thickness of the postsynaptic densities and, in the case of many terminals, wrinkling of their profiles. The most striking form of damage are membrane discontinuities which begin to appear in very small numbers after 20 min of blood reflow and become much more pronounced after 60 min. The development of those modifications seems to be time-related, whereas decrease in the number of synaptic vesicles, as shown by the morphometric analysis, occurs after 10 min and does not progress any further after 20 and 60 min. This decrease is most pronounced in the immediate vicinity of the presynaptic membrane. Although the observed signs of ultrastructural alternations of synapses in the postischemic period appear to conform to the general pattern of synaptic degeneration observed under other conditions, the severity of ischemia is underlined with the rate at which those changes develop, thus pointing toward grossly disturbed metabolism of postischemic neurons. Recently, a number of theories have been advanced, discussing significance of ischemic destruction of membrane phospholipids. These theories are discussed in the context of membrane discontinuities reported in this investigation.

To alleviate this predicament the merits of adopting and adapting the doctrine of network centric operations to healthcare has been recommended [1]. Critical to this approach is the ability to continuously extract and analyse high quality information from the environment or “operational space ” in order to support rapid decision making. This can only be attained through the effective use of IC2T (Information/Computer / Communications Technology). Thus, making the network, or world healthcare information grid (WHIG); a system of interconnecting technology grids, the backbone of healthcare network centric operations (HNCO). Although the required technology and individual components of WHIG exist, attention needs to be paid to the design of this network, with particular considera...
Simulation offers the most efficient adjunct in education and “refresher” training of medical per... more Simulation offers the most efficient adjunct in education and “refresher” training of medical personnel. However, simulation devices and facilities are expensive and the combination of cost, distance from the training centers, and professional constraints prevent medical personnel in rural and remote regions from simulation-based training. We have demonstrated that fiscal and logistic barriers can be overcome by the implementation of distance simulation methods that we have already developed. However, whenever High Fidelity Patient Simulators are used in a multi-unit training environment (e.g., mass casualties) the problems of simulator incompatibility may introduce major problems in the orchestration and control of the simulated events. The paper discusses problems of international large scale “just-in-time” training and the initial solutions to simulation-based preparation of medical personnel using multiple simulators simulator sites separated by ultra-long distances

Exposure of Mongolian gerbils to a 100% oxygen atmosphere after 15 minutes of global brain ischem... more Exposure of Mongolian gerbils to a 100% oxygen atmosphere after 15 minutes of global brain ischemia resulted in a marked increase in the production of pentane, an in vivo product of lipid peroxidation. Much less pentane production occurred in animals subjected to global brain ischemia then exposed to an air atmosphere and in animals exposed to a 100% oxygen atmosphere without ischemia. Gerbils placed in 100% oxygen for 3-6 hours after 15 minutes of ischemia also had a threefold increase in 14-day mortality compared with gerbils subjected to ischemia and then placed in an air atmosphere. These findings raise a serious question about the use of oxygen-enriched atmospheres during reperfusion following ischemia.
