It is characterized by (1) the use of a mirror during the preoperation visit and the taking of pictures from different angles, allowing for a precise analysis of the redistribution of the facial soft tissues on each hemi-skeleton; (2) the Divaris et al 2 locked cheek lift technique (Locked Cheek-Lift), which is done with 2 vectors that achieve a tri-dimensional redistribution of the soft tissues; and (3) postoperative photographs which are very useful in precisely evaluating the results and comparing them with the preoperative pictures. Reduction mammaplasty is one of the most frequent procedures in plastic surgery practice. We eval... more Reduction mammaplasty is one of the most frequent procedures in plastic surgery practice. We evaluated the restoration of nipple areola complex sensibility and nipple erection after more than a year from the surgical intervention for 60 patients who underwent reductive mammaplasty either by superior pedicle technique = 33, or posterior pedicle technique = 27. The patients were divided into two groups according to the weight of resected tissue (A: less than 500g, B: 500g or more). The subjective assessment shows that the restoration of nipple areola complex sensibility and nipple erection is directly related with the weight of resected tissue regardless the technique. The results show that the superior pedicle technique has better restoration of sensation of nipple areola complex in both groups. No notable difference between the two techniques in restoration of nipple erection.

Major infectious complications have significantly decreased from 8.7% in 2010 to 2.6% in 2013 (p=0.03) and are similar to total submuscular coverage at 1.4%. Additionally, after the introduction of sterile ADM, major infectious complications (10.3% (n=56) to 5.6% (n=8) (p=0.112)) and explantation (7.7% (n=50) to 2.8% (n=4) (p=0.004)) has decreased. Conclusion: A cellular dermal matrix is a useful adjunct in immediate implant-based breast reconstruction. The development of strict guidelines for use along with the availability of sterile ADM has dramatically lowered complications and explantations.
bFGF and VEGF165 in ischemic tissue for therapeutic purposes as a form of pharmacological local p... more bFGF and VEGF165 in ischemic tissue for therapeutic purposes as a form of pharmacological local preconditioning before tissue ischemia occurs. Functional evaluations were performed to detect the protein expression and resulting clinical effects. MATERIAL AND METHODS: Primary skin fibroblasts were transfected with eukaryotic expression vectors harboring VEGF and bFGF cDNAs mediated by Amaxa Nucleofector. To determine an improvement in ischemically challenged tissue, a genetically modified cellspool was injected in an ischemic rat flap model. Cells were implanted into 40 rats. Gene expression and protein production were measured in vitro and in vivo by real time PCR and immunoassay (BioPlex) respectively at different time points. Clinical outcome was demonstrated by planimetric measurements and immunohistology. RESULTS: After injection of genetically modified cells, transient protein expression of bFGF and VEGF165 in the target tissue of the ischemic flap model increased significantly compared to controls. A reduction in flap necrosis by one-third was detected, after two weeks if transfected cells were applied 1 week before ischemia. A highly significant improvement of endothelial cell counts was observed after administration of the transfected cells. CONCLUSION: Our results indicate that transient expression of bFGF and VEGF165 induces therapeutically relevant effects after local preconditioning with non-viral transfected fibroblasts in the ischemic rat flap model. Our standardized transfection technology is now used in preclinical research.
