We report the results of a survey on the use of smartphone photography in OMFS departments in the United Kingdom, and highlight the guidelines that govern their use and the associated ethical and medicolegal implications.

Nye IKT-løsninger endrer muligheter og spillerommet. Aktører innen forbrukerfleksibilitet, prosumenttjenester og off-grid vokser frem som følge av denne utviklingen. Felles for disse feltene er at forbrukere involverer seg i større grad i sin kraftsituasjon. For at aktører skal kunne skape verdi på lengre sikt, er det nødvendig at de kan tilpasse seg utfordringer og muligheter fremtiden bringer med seg. Aim Parathyroid carcinoma is vanishingly rare in childhood and the presenting symptoms are often ... more Aim Parathyroid carcinoma is vanishingly rare in childhood and the presenting symptoms are often vague and varied. While orphan diseases are individually rare, and clinicians may only encounter each one once in their career, together they form a significant volume of paediatric practice. We aim to demonstrate a patient journey of this rare condition and illustrate how through basic principles the diagnosis was reached logically. Case A 14 year old boy with no significant past medical or family history presented after a short history of painful frank haematuria. Obstructive renal failure from urolithiasis was diagnosed and relieved. Directed diagnostic workup, starting from first principles revealed hypercalcaemia and hypercalciuria resulting from hyperparathyroidism. Further directed investigations revealed a solitary parathyroid nodule. After parathyroidectomy he required management of hypocalcaemia from expected hungry bone syndrome. Histological examination finally revealed parathyroid carcinoma. Workup for multiple endocrine neoplasia syndromes was negative. Conclusion Our case illustrates that rare conditions in children can be diagnosed and managed using a logical stepwise approach. Although rare diagnoses are seldom encountered, a structure approach using basic principles can be helpful for all clinical problem solving.
Aim Paediatric junior doctors have the potential to identify inefficiency and poor quality care i... more Aim Paediatric junior doctors have the potential to identify inefficiency and poor quality care in the systems they work in. Some engage with influencing and affecting system change, but many do not. What are the barriers to and enablers of quality improvement engagement of junior doctors? Methods We delivered a questionnaire to junior doctors in the North East paediatric deanery. The qualitative and quantitative results were triangulated to identify the barriers and enablers to engagement that are most important to junior doctors. Results 28 trainees (ST1-8) completed the questionnaire. All the junior doctors had encountered system problems and felt that they had a role in improving services; two thirds identified system problems at least weekly or daily; 61.5% had made suggestions or engaged in trying to improve the work in their environments. The key motivators that would encourage junior doctors to suggest, initiate or lead change were; an increased awareness of the true cost of resources; understanding the consequences of not improving the quality of the service; recognition of quality improvement efforts in appraisals, portfolios and career progression. 88% wanted training in how to achieve change. Over two-thirds of the junior doctors concluded that audit had not been a good use of their time nor contributed to any improvement. The following were considered barriers to change (based on majority score): Limited knowledge of quality improvement methods; Differing definitions of high quality care; Belief that such initiatives never work and are a waste; Don't feel part of the organisation; Senior clinicians not valuing the views of juniors; Conflicting views on who is responsible for change; Lack of time; Lack of resources; A history of failed initiatives; Potential negative effects elsewhere in the system; Lack of practical examples of benefit; Lack of quick delivery on benefits; Poor training in change management; Unfamiliarity with management; Distrust of management; Problems with group dynamics; Conclusions Junior doctors are a motivated but untapped resource to affect quality improvement. The barriers identified can be overcome by deaneries and employers recognising the need to building quality improvement capability through training and application of proven Qi methods.

Det ene scenariet beskriver et tilfelle med store begrensninger i kraftnettet, mens det andre beskriver et tilfelle der prisen på lagringsmuligheter har falt og de har blitt mer effektive. Videre blir aktørenes tilpasningsmuligheter til disse scenariene diskutert. Studien av forretningsmodellene viser først og fremst at alle aktørene har muligheter for verdiskapning i begge scenariene i det eksisterende kraftmarkedet. Samarbeid mellom aktører for forbrukerfleksibilitet og prosumenter vil bidra med å balansere forbruk og produksjon fra uregulerbare produksjonskilder på en effektiv måte. Samtidig vil samarbeidet bidra med å avlaste kraftnettet i perioder med begrensninger. Teknologiutviklingen og utfordringer med å dekke økende forbruk kan føre til at flere kobler seg fra kraftnettet. Det kan i noen tilfeller være av interesse for staten å støtte aktørene med midler for dette. For å kunne danne seg et bilde av hvilke implikasjoner som vil forekomme med de aktuelle utfordringene og mulighetene i fremtiden, burde mer enn kvalitative analyser bli tatt i betraktning. En studie i form av kvantitative analyser vil være et godt supplement til denne studien
