An 80-year-old man with a history of an orthotopic heart transplant was found to have a 25 × 40 m... more An 80-year-old man with a history of an orthotopic heart transplant was found to have a 25 × 40 mm centrally ulcerated mass at the hepatic flexure during evaluation of anemia. Owing to comorbidities, the patient was deemed to be a poor surgical candidate and was referred to the advanced endoscopy team to explore palliative and potentially curative options. We present a novel sequence of intervention involving full-thickness resection with subsequent morcellation clean-up to achieve complete endoscopic removal of a neoplastic lesion.

Sharp changes in state, such as transitions from survival to extinction, are hallmarks of evoluti... more Sharp changes in state, such as transitions from survival to extinction, are hallmarks of evolutionary dynamics in biological systems. These transitions can be explored using the techniques of statistical physics and the physics of nonlinear and complex systems. For example, a survival-to-extinction transition can be characterized as a non-equilibrium phase transition to an absorbing state. Here, we review the literature on phase transitions in evolutionary dynamics. We discuss directed percolation transitions in cellular automata and evolutionary models, and models that diverge from the directed percolation universality class. We explore in detail an example of an absorbing phase transition in an agent-based model of evolutionary dynamics, including previously unpublished data demonstrating similarity to, but also divergence from, directed percolation, as well as evidence for phase transition behavior at multiple levels of the model system's evolutionary structure. To this end, the semiotics of the pragmatist C.S. Peirce is explained and its application in OR research, and consulting practice is explored. It is the fourth in a series of papers each seeking to ground OR in a different logic: philosophic/mathematical logic, pragmatic logic, economic logic and now semiotic logic. For Peirce, semiotics is a logic, and as such it is argued in the paper that it can embrace philosophical/mathematical, pragmatic and economic logics. The comprehensive nature of semiotics is such that it brings to a close the research programme aimed at establishing foundations for OR. However, the closure of one programme opens up another—the application of semiotics in OR practice and academic research. Possible avenues for further research include learning from, and collaboration with, neighbouring disciplines such as information systems and computer science.

A keen interest in the measurement of economic power, particularly from regulatory authorities, d... more A keen interest in the measurement of economic power, particularly from regulatory authorities, derives from its potentially adverse impact on resource allocation. The concept of economic power encapsulates the dual forces of market (monopoly) power and political power. It is generally associated with the large-scale accumulation of wealth and significant assets. When viewed from political, economic and ethical perspectives it is seen as a ‘threat to the operation of both economy and society’ (Gerber, 2001: 87). The threat derives from ills associated with the economic dependency of one firm on another, and with the potential for the poor treatment of workers. An alternative view focuses on its unfair representation that amounts to a punishment of entrepreneurial skills. In particular, Peritz (2000) perceives the animosity generated by economic power as evidence of the tension between the rhetorics of competition and property. This perception is especially unfair when economic power reflects commercial success as a result of the fruits of research and development (R&D) or economies of scale from which an economy benefits.

As OR consultants, we compete for commercial contracts with clients based on our practical expertise. The paper seeks to identify the economic foundations of this contracting process, to separate those aspects most likely to endure from those that are more ephemeral. The focus of the analysis is on the behaviour of the players in the consulting services market. The market for consultancy thrives despite the fact that it is difficult for both consultants and clients to understand what is being sought and what is being offered. A key question for everyone concerned is how this problem is resolved in practice. The paper distils existing theories into a set of foundational statements that inform OR research aimed at improving the practice of OR consultants and client interactions. Eight economic foundational statements are derived.

The paper reviews the application of pragmatisms in five selected areas of professional practice: technology, law, politics, medical and social work, and education. Each area is examined for evidence that the ideas of pragmatism have been taken up academically in the research literature. The literature is then examined for evidence that the academic work has permeated the actions of professionals. Each of the chosen practice areas is clearly dominated by pragmatic, instrumental activities. Despite this and a good deal of discussion in the academic literature, there is only limited evidence that professionals, in so far as they exhibit pragmatist behaviour, make any explicit connection with the philosophy of pragmatism. Multiple myeloma (MM) is an incurable hematological malignancy characterized by the clonal prolif... more Multiple myeloma (MM) is an incurable hematological malignancy characterized by the clonal proliferation of malignant plasma cells in the bone marrow. Like other cancers, MM is a genetically complex and heterogeneous disease. One of its distinctive characteristics is that it is preceded by a pre-malignant condition known as monoclonal gammopathy of undetermined significance (MGUS), which then progresses to asymptomatic (smoldering) multiple myeloma (SMM) and, ultimately, to late-stage MM. Its progression through these stages is determined by a sequence of genomic aberrations, starting with germline events that predispose to the disease, followed by early initiating events and the later acquisition of mutations that contribute to disease progression.
