The ability of MRI to identify lesions that were within 5 mm of the prostatic urethra on whole-mount pathology was determined. Multivariable regression was utilized to determine clinical characteristics associated with having prostate cancer within 5 mm of the urethra. RESULTS: A total of 90 patients were identified to be candidates for focal-gland ablation. In 66 men (73.3%), the tumor was within 5 mm of the urethra on whole-mount pathology. MRI correctly identified the cancer location either within or outside a 5 mm radius of the urethra in 68 men (75.5%). The sensitivity of MRI for identifying cancer near the urethra was 78.8% (73.5-84.1%), and specificity was 66.7% (58.175.3%). The PPV was 86.7% (79.4-91.6%), and NPV was 53.3% (39.8-66.4%). The overall agreement was moderate (kappa[0.47). In adjusted analysis, no clinical factors (PI-RADS, PSA density, or tumor diameter) were significantly associated with having prostate cancer within close proximity of the urethra. CONCLUSIONS: A significant number of men who are potential candidates for focal ablation may have cancer close to the urethra that could serve as a significant source of infield recurrence after treatment. MRI can be used to identify these tumors and the distance from the tumor to the urethra should be calculated to improve patient selection for focal gland ablation.

High‐resolution maps can serve as predictive tools for locating subsurface architecture based on patterns in surface topography. This paper presents recent intensive mapping conducted at three Late Archaic (3,000‐1,800 B. C. E.) archaeological sites in the Fortaleza Valley of Peru. Total station data were used to generate 3D surface maps in ArcGIS to delineate elevation changes in fine‐grained detail. Distinctive features on the surface and microtopography reveal small‐scale patterns that can inform our choices for targeted excavations. This procedure will provide a methodological tool for finding subsurface architecture at large‐scale sites elsewhere.

Here we review our combined experience treating 46 stage I postlumpectomy patients with this approach. Methods: Twenty-one patients at the Swedish Medical Center in Seattle were treated with total doses ranging from 25-36 Gy delivered in 5 to 10 equal fractions. Twenty-six patients at Winthrop University Hospital were treated with 30 Gy in 5 equal fractions. Margin and isodose schemes differed between sites, but were chosen to assure lumpectomy cavity coverage, including a margin to account for potential microscopic disease and a small margin to account for residual uncertainty, and low doses to organs at risk. Several series have published health related quality of life outcomes following SBRT, however none have specifically examined phosphodiesterase-5 inhibitor's (PDE5i) effect on erectile function after SBRT treatment. In this preliminary review, our objective is to measure PDE5i's influence on erectile function and sexual satisfaction in SBRT men in a single institution using validated questionnaires. The thermal expansion behavior of pure, Ge-doped and Li-doped hexagonal cordierites with respecti... more The thermal expansion behavior of pure, Ge-doped and Li-doped hexagonal cordierites with respective compositions: 2MgO 2Al2O3 5Si02 2Mg0 2Al2O3 4Si02 Ge02) and 2MgO (2+x)AL203 (5-2x)Si02 xLi20 with x = .174, was investigated using time-of-flight neutron powder diffraction at temperatures from 22 to 750°C in vacuum. The data were refined in space group P6/mcc using the Rietveld method. The lattice thermal expansion curves of all 3 samples were quite similar. The negative axis expansion is associated with (1)displacement of the T2 cations generally toward the axis channels and (2) changes in the distortion of the coupled T1/M tetrahedra/octahedra in the structure. Both contributions were present in all 3 samples but the first was more dominant in the Ge doped sample. The nature and origin of the distortions in T1 and M are discussed.

However, the realm of warfare is the one glaring exception to this understanding of a major disconnect between past and present hunters and gatherers. Archaeologists, anthropologists, and other social scientists studying the origins of warfare have found the archaeological record somehow lacking in their efforts to understand the beginnings of warfare in the ancient past. To fill in the perceived gaps, they have turned to the historic record of hunters and gatherers, and in doing so have fallen into the trap of ethnographic tyranny. This chapter argues that the biggest problem with using historical ethnographies to make inferences about patterns of past human behavior is that they burden us with pictures only painted in the light of the modern, dense, colonial world of nation states. These indications include positive margin or uncertain margin status of the primary excision or residual microcalcifications on postbiopsy mammogram. If the pathology from the re-excision does not confirm negative margin status, mastectomy is generally recommended. This article examines patients who have undergone a second re-excision (i.e., a lumpectomy followed by two re-excisions) who have been treated with breast conservation therapy rather than a mastectomy.
