The prevention and management of bone loss after renal transplantation is not elucidated thus far. The purpose of this study was to investigate whether the initiation of alendronate therapy immediately after transplantation can prevent bone loss and also to assess its safety on graft function and serum cyclosporine level. Materials and Methods: We conducted our study on 50 patients and randomly divided them into two groups: alendronate treatment group ( n = 25) and placebo group ( n = 25). Bone mineral density (BMD) of lumbar and femoral neck was measured using dual-energy X-ray absorptiometry prior to transplantation and after 6 months. The graft function was assessed by measuring the serum creatinine level and the serum cyclosporine trough level, which was measured by radioimmunoassay every 2 months until the end of the study. Results: The BMD in patients treated with alendronate increased significantly both at the lumbar vertebrae and femoral neck, while it decreased in the placebo group. The changes in the serum cyclosporine levels were independent of alendronate or placebo within 6 months after transplant. The graft function was similar between the groups. Conculsion: Alendronate was effective as well as safe for prophylaxis of rapid bone loss after renal transplantation.

Oxidative stress has a critical role in the pathogenesis of RA and CVD. Sesamin, the main lignin constituent of sesame, has several antioxidant and anti-inflammatory effects. This study aimed to investigate the effects of sesamin supplementation on anthropometric indices, lipid profile, blood pressure, and oxidative stress markers in women with RA. Methods: In this randomized, double-blind, placebo-controlled clinical trial, 44 patients with RA were randomly divided into 2 groups (intervention and control). Patients consumed 200 mg/day sesamin supplement and placebo in the intervention and control groups, respectively, for 6 weeks (spring 2014). At baseline and at the end of the study, anthropometric indices and blood pressure were assessed. Serum concentrations of lipid profile, malondialdehyde (MDA), and total antioxidant capacity (TAC) were also determined. Results: At the end of study, sesamin supplementation significantly decreased serum levels of MDA (p = 0.018) and increased TAC and high-density lipoprotein cholesterol (HDL-C) levels in patients with RA (p = 0.020 and p = 0.007, respectively). In the sesamin group, the mean of weight, body mass index, waist-to-hip ratio, body fat, systolic blood pressure, and the concentration of other lipid profiles (triglycerides, total cholesterol, and low-density lipoprotein cholesterol [LDL-C]) were also significantly decreased at the end of study compared to baseline values (p < 0.05). However, the difference between the 2 groups was not statistically significant in this regard (p > 0.05). Conclusion: Sesamin exhibited a protective effect on cardiovascular risk factors in patients with RA. However, further investigation is suggested.

Pomegranate juice is rich in polyphenolic compounds that possess antioxidant and anti-inflammatory activities. This study aimed to determine the beneficial effects of pomegranate extract (POMx) in RA patients. A total of 55 RA patients were enrolled and randomly allocated to an intervention group (n=30) or a control group (n=25). The intervention group received 2 capsules of 250 mg POMx and the control group 2 capsules of 250 mg cellulose per day for 8 weeks. At the beginning of the study and after 8 weeks, Health Assessment Questionnaire (HAQ) and Disease Activity Score (DAS) 28 were completed and serum concentrations of C-reactive protein (CRP), matrix metalloproteinases 3 (MMP3), malondialdehyde (MDA), glutathione peroxidase (GPx) and erythrocyte sedimentation rate (ESR) were analyzed using standard methods and compared between the two groups. Compared with the placebo group, POMx supplement significantly reduced the score of DAS28 (P&lt;0.001) which could be related to the decrease in swollen (P&lt;0.001) and tender joints (P=0.001) count, pain intensity (P=0.003) and ESR levels (P= 0.03). We evaluated the results of alendronate on bone density in patients with chronic kidney disease (CKD). Objectives: The main aim of the study was evaluation of the changes in bone density, one year after treatment by dual energy x-ray absorptiometry technique. Patients and Methods: We evaluated two treatment regimens on bone density of 44 patients between 18-45 years old (22 in experimental and 22 in control group) in a controlled double blind randomized trial. The experimental group was prescribed alendronate (10 mg), calcitriol (0.25 μg) and calcium carbonate (1500 mg) daily. The control group was treated with the same regimen except for alendronate. Methods: In this cross-sectional study, 120 RA patients referred to the rheumatology clinic, the patients were diagnosed by rheumatologists according to ACR / EULAR 2010 criteria. Based on Based on DAS28 score the patients were divided into 4 groups: Remission, Mild, Moderate and Severe. Each group contained 30 patients. Serum levels of ESR, CRP, anti-cyclic citrullinated peptide (anti-CCP) and RF, as well as serum levels of IL-6, were measured in these patients. Then, the relationship between these factors in the four groups was measured and compared, and the relationship between IL-6 and these factors, and especially the severity of the disease, was evaluated based on DAS-28. So, the aim of the study was to evaluate the prevalence T. gondii and T. canis in patients with myositis and healthy individuals. Methods: A total of 108 samples were randomly selected as the control (54 healthy individuals) and test (54 myositis patients) groups. IgG and IgM antibodies against T. gondii and IgG antibodies against T. canis were measured by the ELISA. The detection of chronic and acute toxoplasmosis was performed by the ELISA IgG avidity. The presence of T. gondii in blood was evaluated by the nested-PCR. Results: Of 108, 33 (30.6%) cases were detected positive for IgG against T. gondii that 19 (35.2%) and 14 (25.9%) were observed in myositis patients and healthy individuals, respectively (P=0.296).
