Simultaneously, information on practices was collected from Belgian pediatric and neonatal departments. Based on the review and these practices, a consensus was developed and voted on by all co-authors and heads of pediatric departments. Today, practices vary. In line with literature, the advised prophylactic regimen is 1 or 2 mg IM vitamin K once at birth. Signs and symptoms of MCADD typically appear during infancy or early childhood and include vomiting, lethargy, and hypoglycemia. Pulmonary haemorrhage has previously been described in patients with MCADD, but has always been considered a pre-terminal complication caused by heart failure. We report on a newborn term infant that presented on the second day of life with signs of encephalopathy, followed by hypovolemia and respiratory distress caused by a severe pulmonary haemorrhage. Fluid resuscitation and mechanical ventilation were initiated and the coagulopathy was corrected by the administration of fresh frozen plasma. Echocardiography revealed a normal cardiac function. After 6 days of full intensive care, the patient survived without sequellae. The clinical presentation in absence of signs of infection raised a strong suspicion for a metabolic disorder and g...

We have developed a simple scoring system to assess four parameters of cerebral maturation--myelination, cortical folding, glial cell migration, and germinal matrix distribution--to determine the total maturation score (TMS). The aim of this study was to validate this scoring system in a large population of preterm infants across a range of gestational ages. A retrospective analysis was conducted of MR images acquired over a 3-year period with an identical imaging protocol. Babies were resuscitated and then observed for signs of multi-organ system dysfunction. Apart from standard supportive management, a new arsenal of potential neuroprotective strategies have emerged over the past years, in order to decrease the severity of brain injury following asphyxia. Today, several neuroprotective therapies are being evaluated in human infants.
This article will define the concept of fetal/neonatal inflammatory response, and examine the com... more This article will define the concept of fetal/neonatal inflammatory response, and examine the complex interaction between inflammation and neurotoxicity. There appear to be important interactions between infection/inflammation and hypoxia-ischaemia leading to cytokine release and subsequent brain injury. This article will also define adverse outcome and summarize the complexities inherent to neurodevelopmental assessment. Finally, this article will investigate the currently available evidence suggesting a link between inflammatory response and adverse neurodevelopmental outcome, and focus on those variables that need further study: timing and nature of the infectious/inflammatory process; established and new anti-insult strategies; morbidity in organs other than the brain; genetic influences; and environmental factors.

Long-term visual memory performance was impaired by two types of challenges: a diazepam challenge... more Long-term visual memory performance was impaired by two types of challenges: a diazepam challenge on acquisition and a sensory challenge on recognition. Using positron-emission tomography regional cerebral blood flow imaging, we studied the effect of these challenges on regional brain activation during the delayed recognition of abstract visual shapes as compared with a baseline fixation task. Both challenges induced a significant decrease in differential activation in the left fusiform gyrus, suggesting that this region is involved in the automatic or volitional comparison of incoming and stored stimuli. In contrast, thalamic differential activation increased in response to memory challenges. This increase might reflect enhanced retrieval attempts as a compensatory mechanism for restoring recognition performance.

In the present study, we used a long-term visual recognition task to evaluate the persistence of the scopolamine-induced anterograde amnesia beyond drug clearance intervals. Following memorization of a list of abstract shapes, subjects were evaluated on recognition performance immediately after encoding, and after a 3-day interval. Administration of scopolamine (0.4-0.8 mg) 70 min prior to encoding induced a significant (8-16%) deficit in delayed recognition performance. In contrast, a scopolamine challenge on delayed recognition following a drug-free encoding did not influence memory performance. In contrast, even at peak levels, scopolamine did not alter immediate recognition, detection or visual discriminative performances. Hence, the presence of scopolamine during the encoding of the shapes induced a significant long-term memory deficit that persisted after scopolamine clearance. Therefore, this paradigm is useful for imaging regional brain activation during impaired recognition without the confounding direct effects of scopolamine on cerebral blood flow or metabolism, two physiological variables underlying the indirect measurement of brain activation.

Current drug-based therapeutic options for large haemangiomas include corticosteroids, α-interferon and vincristin, all of which can result in harmful side effects. Recently, promising results have been reported using the non-cardio-selective β-blocker propranolol for the treatment of cutaneous capillary haemangiomas, in which a spectacular size reduction was observed during the first 7 days of treatment. We here report a similar significant and rapid inhibitory effect of propranolol on the growth of a viscerally located congenital haemangioma.
