A combined posterior-anterior approach is usually proposed for the fixation of highly unstable sp... more A combined posterior-anterior approach is usually proposed for the fixation of highly unstable spinal lesions. A monocortical anterior fixation seems to become more and more popular. In the period from 1993 to 1998, 43 patients with minimally anterior and middle column destruction of thoracolumbar spine were anteriorly instrumented. There were 23 tumors, 11 specific infections, 5 posttraumatic conditions with failed posterior instrumentation, 4 acute fractures. Anterior instrumentation (45Nm rod-screw rotation rigidity) were used in all cases. A four screws principle with two non connected rods were bicortically applied to correct the deformity and to fix the corpectomy gap. No postoperative bracing was necessary. There was one pseudarthrosis 2yrs post op. due to poor anterior fusion in a posttraumatic case. In one case instrumentation failure occurred due to widespreading of the prostatic tumor. The study revealed no complications due to bicortical screw fixation in thora-columbar region. It is suggested that combined anterior and posterior procedure is only exceptionally necessary.
130 patients with idiopathic thoracic scoliosis were operated on by means of the same anterior in... more 130 patients with idiopathic thoracic scoliosis were operated on by means of the same anterior instrumentation principle in the period from 1987 through 1998. There were 21 males and 109 females in the age of 16(±4). Patients were randomly chosen in the group with rib hump apical osteotomy added to the anterior instrumentation (group 1) and the group with the anterior instrumentation without rib hump osteotomies (group 2). A prospective study was performed. Preoperative and postoperative gibbometry was performed. Results were as follows: frontal correction: 67% for group 1., and 74% for group 2. Rib hump changed from 23 ± 15 to 6 ± 6.2 (74%) for group 1, and 20 ± 1.5 mm (62%) for group 2. Rib valley changed from 21 ± 11 to 7 ± 4 (63%) (group 1) and 37 ± 2.8 to 16 ± 3.4 (56%) (group 2). There were 4 hematothoraxes in group 2. and no haematothoraxes in group 1. It seems that rib hump osteotomy has no insuence upon frontal scoliosis correction, and little insuence upon rib valley corre...
It is well documented that whole body-vibration training has effects on muscle strength and flexi... more It is well documented that whole body-vibration training has effects on muscle strength and flexibility, blood circulation, decreases pain perception and strengthens bone and tendon. Although whole body-vibration has benefits in athletes’ flexibility, we are not sure what its actual effects are in artistic gymnastics (since they already have stunning flexibility). Hence, the aim of this study was to analyse the studies on whole-body vibration in artistic gymnastics and to present the effects on flexibility. The search and analysis were carried out in accordance with PRISMA guidelines. The databases search (PubMed, Scopus, Google Scholar, Cochrane Library, ProQuest, EBSCOhost and Science Direct) yielded 18,057 potential studies. The last three decades have witnessed a tremendous increase in female sports participation at all... more The last three decades have witnessed a tremendous increase in female sports participation at all levels. However, increased sports participation of female athletes has also increased the incidence of sport-related injuries, which can be either acute trauma or overuse injuries. Overuse injuries may be defined as an imbalance caused by overly intensive training and inadequate recovery, which subsequently leads to a breakdown in tissue reparative mechanisms. This article will review the most frequent overuse injuries in female athletes in the context of anatomical, physiological, and psychological differences between genders.

Izdvojene su važne znacajke koje se odnose na postavljanje specificnoga klinickog pitanja, prikupljanje objavljenih studija, kriticku procjenu podataka i objedinjavanje znanstvenih spoznaja. Glavni je cilj ovog rada potaknuti sto vise medicinskih djelatnika da se u svakodnevnom donosenju klinickih odluka služe najboljim dostupnim spoznajama iz literature te da se i sami okusaju u izradi kvalitetnoga sustavnog pregleda.

To present the case of warfarin-cloxacillin interaction that resulted in an increased internation... more To present the case of warfarin-cloxacillin interaction that resulted in an increased international normalized ratio (INR). A 70-year-old man had been treated with warfarin for atrial fibrillation. He was hospitalized because of superficial thrombophlebitis of the left median cubital vein, which developed after venipuncture. An antibiotic therapy with cloxacillin was initiated immediately after the admission. Two days later, INR value increased from baseline 1.9 to 4.6. Anticoagulation therapy was discontinued and INR value was measured daily. His INR remained high for the entire duration of antibiotic therapy. Three days after the cloxacillin therapy was discontinued, the INR decreased to the baseline value. In the presented case, the temporal relationship between the administration of cloxacillin and increased INR suggests that the cloxacillin was responsible for the enhanced warfarin activity.
