Pourtant, les patients atteints de maladies auto-immunes ont été exclus des principaux essais thérapeutiques ayant permis de valider la tolérance et l’efficacité de la vaccination contre le SARS-CoV2. Nous avons évalué l’activité de la maladie et les facteurs associés à la réponse vaccinale, au décours d’une vaccination par le vaccin BNT162b2, chez des patients atteints de lupus systémique (LS). Patients et méthodes Dans cette étude prospective, conduite entre janvier et mai 2021, l’activité clinique et biologique de 126 patients atteints de LS et les éventuels effets secondaires étaient recueillis lors de 4 consultations réalisées lors des 2 injections du vaccin espacées de 3 ou 4 semaines, et à 15jours de chacune des injections. La réponse humorale, critère principal de jugement, était mesurée 15jours après l’injection de rappel. Elle était déterminée par la mesure de la concentration en anticorps spécifiques dirigés contre la protéine spike, et par l’étude de la capacité de neutralisation de différents variants viraux par le sérum des patients vaccinés. Nous avons également étudié la réponse cellulaire T par des tests Interferon gamma release assay (IGRA) spécifiques du SARS-CoV2. Les résultats de l’analyse multivariée étaient exprimés par des coefficients bêta, dont la valeur mesure la taille de l’effet du facteur considéré sur la réponse vaccinale, et le degré de significativité p associé indique si cette valeur est significativement différente de 0. Résultats Parmi les 126 patients inclus, 114 (90,5 %) étaient des femmes, d’âge médian 46,6 ans (IQR 33,9–58,7). Le score SLEDAI-2K était supérieur à 5 chez 24 (19,0 %) patients et 20 (16,7 %) patients présentaient au moins un score BILAG à B. Soixante-dix (55,6 %) patients recevaient des corticoïdes, 106 (84,1 %) de l’hydroxychloroquine et 54 (42,9 %) étaient traités par un autre immunosuppresseur, principalement le mycophénolate mofétil, le méthotrexate ou l’azathioprine. La tolérance du vaccin BNT162b2 était bonne, sans variation significative des scores BILAG et SLEDAI-2K au décours de la vaccination, ni observation d’effets secondaires graves. La régression linéaire multivariée a permis de montrer que le mycophénolate mofétil et le méthotrexate étaient indépendamment associés à une moindre réponse humorale (Bêta=−78, p =0,007; Bêta=−122, p <0,001 respectivement). À l’inverse, la concentration d’immunoglobulines G (IgG) et la proportion de lymphocytes B naïfs le jour de la première injection étaient significativement associés à une concentration supérieure en anticorps anti spike (Bêta=2; p =0,018; Bêta=2,5; p =0,003 respectivement). L’hydroxychloroquine (n =106), les corticoïdes (n =70), le belimumab (n =15) ne diminuaient pas significativement la réponse vaccinale. Les réponses humorale et cellulaire T étaient significativement corrélées (r=0,46; p =0,003). L’activité neutralisante étaient diminuée dans le cas de certains variants porteurs de la mutation E484K mais restait détectable chez la majorité des patients (entre 76,2 et 60,3 % de sujets neutralisant les variants contre 81,7 % pour la souche initiale). Conclusion Contrairement aux critères démographiques et à l’activité de la maladie lupique, le mycophénolate mofétil, le méthotrexate et le déficit lymphocytes B naïfs ou en IgG semblent indépendamment associés à une moindre réponse humorale à la vaccination par BNT162b2, devant faire discuter dans ces cas une adaptation du schéma vaccinal, par exemple en proposant une troisième injection.
Introduction Les histiocytoses sont un groupe d’affections heterogene, parmi lesquelles on distin... more Introduction Les histiocytoses sont un groupe d’affections heterogene, parmi lesquelles on distingue les maladies comportant une composante clonale constante associee a une mutation somatique sur la voie des MAPkinases (histiocytoses du groupe L comportant la maladie d’Erdheim-Chester, MEC et l’histiocytose Langerhansienne, HL), et les histiocytoses pour lesquelles la composante clonale est moins frequemment mise en evidence (histiocytoses du groupe R de type Destombes-Rosai-Dorfman, RDD). L’utilisation de l’interferon-alpha, un immunomodulateur et myelosuppresseur qui ameliore la survie au cours de la MEC, est limitee par les frequents effets indesirables (syndrome grippal, troubles thymiques, auto-immunite). Si les therapies ciblant la voie des MAPkinases ont revolutionne la prise en charge des histiocytoses depuis 2012, leur utilisation est grevee d’effets indesirables parfois severes, rendant necessaires les alternatives therapeutiques, notamment pour les formes les moins severes. La cladribine, une chimiotherapie myelosuppressive efficace sur la lignee monocytaire, est efficace dans les HL mais les donnees sur son utilisation au cours des histiocytoses non Langerhansiennes (MEC et RDD) sont limitees. L’objectif de l’etude etait d’evaluer l’efficacite et la tolerance de la cladribine au cours des histiocytoses non Langerhansiennes. Patients et methodes Les patients ont ete inclus a partir du registre des histiocytoses et des donnees de pharmacie hospitali7re. Les criteres de selection etaient: uns histiocytose non Langerhansienne de type MEC ou RDD ou non classee, la realisation d’au moins une perfusion ou injection de cladribine, et des donnees suffisantes dans le dossier medical. L’efficacite a ete evaluee par la reponse clinique, radiologique, et metabolique (remission partielle, complete, stabilite, ou progression). Les donnees de tolerance etaient extraites a partir des dossiers medicaux. Resultats Un total de 21 patients a ete inclus (17 hommes et 4 femmes), d’âge median au diagnostic de l’histiocytose de 48 ans (18 a 73). Dix patients avaient une maladie de RDD, 5 une histiocytose mixte (MEC + HL), 4 une MEC, et 2 une histiocytose non Langerhansienne inclassee. Cinq patients presentaient une mutation BRAFV600E. Les patients presentaient une atteinte du systeme nerveux central (n = 15), osseuse (n = 14), ORL (n = 8), ophtalmologique (n = 8), pulmonaire (n = 6), vasculaire (n = 6), et ganglionnaire, cutanee, et/ou perirenale (n = 4 chacun). Seize patients recevaient le traitement par 2CDA en seconde ligne ou plus (apres un traitement variable comportant: corticotherapie, rituximab, methotrexate, cyclophosphamide, infliximab, interferon, vinblastine, anakinra, vemurafenib, chirurgie), alors que 5 le recevaient en premiere ligne. Le nombre median de cycles etait de 4 (1 a 8). Les traitements concomitants etaient des corticoides chez 6 patients, de l’interferon-alpha chez 1, et du vesanoide chez 1. Tous les patients recevaient une prophylaxie anti-infectieuse par cotrimoxazole et valacyclovir. Le pourcentage de reponse clinique (complete + partielle) etait de 62 %, et de 43 % pour les reponses radiologiques (complete + partielle). La reponse metabolique etait evaluable chez 13 patients: parmi eux on observait 8 reponses (62 %) (2 completes et 6 partielles), 1 patient stable (8 %), et 4 progressions (31 %). Parmi les patients ayant obtenu une reponse sous cladribine, 6 (29 %) ont rechute avec un delai median de 16 mois. Quinze patients ont eu une nouvelle ligne therapeutique apres la cladribine, dont une therapie ciblee chez 10 (48 %). Les effets indesirables de la cladribine etaient hematologiques chez 19 patients (90 %), avec une lymphopenie chez 19/21 patients (90 %), une neutropenie Discussion La revue de litterature a permis d’analyser 39 patients, et montre un taux de reponse global radiologique de 59 %. Comparativement aux 43 % trouves dans notre cohorte, cette meilleure reponse peut etre liee a une frequence plus elevee d’atteintes neurologiques centrales dans notre serie (71 % versus 41 %), et egalement a un biais de publication. Ces taux de reponse dans les histiocytoses non-Langerhansiennes restent inferieurs a ceux observes dans les series d’histiocytoses Langerhansiennes. Conclusion La cladribine est une option therapeutique interessante dans les histiocytoses non-Langerhansiennes, avec un taux de reponse entre 43–59 %. La tolerance est excellente malgre une lymphopenie constante.

We conducted a retrospective study aiming to describe clinical features, treatments and outcome of SLE-related APD. Methods We retrospectively included all patients with SLE and ALD admitted from November 1996 and September 2018 to La Pitié-Salpêtrière Hospital, after exclusion of viral or bacterial lung infection, cardiac failure or any other alternate diagnosis. Introduction Les maladies du spectre des neuromyelites optiques (neuromyelitis optica spectrum di... more Introduction Les maladies du spectre des neuromyelites optiques (neuromyelitis optica spectrum disorder, NMOSD) sont des maladies demyelinisantes auto-immunes du systeme nerveux central. Elles se manifestent par des episodes recidivants de myelites et de nevrite optique (NO). Le plus souvent, elles sont associees a la presence d’anticorps anti-aquaporine 4 (anti-AQP4). L’association d’une NMOSD anti-AQP4+ et d’une autre maladie auto-immune n’est pas exceptionnelle. En particulier, l’association avec le lupus systemique (LS) et le syndrome de Sjogren primaire (SS) a ete rapportee dans la litterature, sans que l’on sache si cette association avait un impact sur la prise en charge et le pronostic. L’objectif de l’etude etait d’analyser les caracteristiques cliniques, radiologiques, et le suivi a long terme des patients presentant une NMOSD avec anticorps anti-AQP4 associee a un LS et/ou un SS. Patients et methodes Il s’agissait d’une etude retrospective menee dans 4 centres. Les patients presentant une NMOSD avec anticorps anti-aquaporine 4 (criteres IPND 2015) et un LS (criteres EULAR/ACR 2019) ou un SS (criteres ACR/EULAR 2016) ont ete retrospectivement inclus. Les caracteristiques cliniques, biologiques et radiologiques en lien avec la NMOSD, le LS et/ou le SS ont ete recueillies a partir des dossiers medicaux. Lorsque cela etait possible, nous avons cherche la presence d’anticorps anti-AQP4 dans le serum des patients anterieurs aux premieres manifestations clniques de la NMOSD. Resultats Quinze patients presentant une NMOSD avec anticorps anti-AQP4 associee a un LS et/ou un SS (9 patients presentaient un LS, 5 patients un SS et 1 patients un LS associe a un SS) ont ete inclus. La duree de suivi mediane etait de 120 mois (4 a 336 mois) a partir du diagnostic de NMOSD. Quatorze patients (93 %) etaient des femmes et leur âge median lors de la premiere manifestation de NMOSD etait de 40 ans (11–56 ans). Le diagnostic de LS et/ou de SS precedait celui de NMOSD chez 10 patients (67 %) avec un delai median de 72 mois (4–168 mois). Le diagnostic de NMOSD etait concomitant de celui de LS et/ou SS chez 3 patients (20 %) et le precedait chez 2 patients (13 %). Les patients lupiques presentaient une atteinte cutaneo-articulaire dans 90 % des cas ; des cytopenies auto-immunes dans 30 % des cas, et une atteinte des sereuses ou une atteinte renale dans 30 % des cas chacun. Deux patients remplissaient les criteres de syndrome des anticorps anti-phospholipides. Tous les patients avaient des anticorps antinucleaires positifs, 13/15 des anticorps anti-ADN natif, 5/15 des anticorps anti-SS-A, 2/16 des anticorps anti-RNP, 1/15 des anticorps anti-SS-B, et 1/15 des anticorps anti-Sm. Le nombre moyen de poussees de NMOSD par patient etait de 2,7 (1–6). Les manifestations incluaient une atteinte medullaire dans 80 % des cas, une NO dans 60 % des cas, et une atteinte encephalique dans 33 % des cas (dont un syndrome de l’area postrema). Au diagnostic de NMOSD, les patients avec un LS et/ou SS prealable etaient traites par hydroxychloroquine dans 60 % des cas, prednisone dans 20 % des cas (tous a la dose de 5 milligrammes par jour), et azathioprine dans 10 % des cas. Les poussees de NMOSD n’etaient pas concomitantes de poussees de LS chez nos patients. Seul un patient a presente, lors de sa cinquieme poussee de NMOSD, une pericardite avec elevation du titre d’anticorps anti-ADN natifs. Les poussees de NMOSD etaient traitees par bolus de methylprednisolone dans 60 % des cas, par rituximab dans 49 % cas, par cyclophosphamide dans 12 % des cas, et par echanges plasmatiques dans 7 % des cas. Les molecules utilisees comme traitement d’entretien etaient le rituximab dans 60 % des cas, l’azathioprine dans 47 % des cas, et le mycophenolate mofetil dans 33 % des cas. Parmi nos 15 patients, 14 etaient en remission lors de la derniere consultation, apres une duree mediane de 33 mois (6–240 mois). Chez un patient, nous avons pu effectuer la recherche d’anticorps anti-AQP4 dans un serum preleve deux ans avant les manifestations cliniques de la NMOSD, recherche qui s’est averee positive. Conclusion Les NMOSD associees aux anti-AQP4 peuvent etre concomitantes d’un LS ou d’un SS, parfois peu severe, et peuvent survenir plusieurs annees apres le diagnostic de LS ou SS, et en dehors des poussees de ces dernieres. Par ailleurs, les anticorps anti-AQP4 pourraient apparaitre quelques annees avant le diagnostic de la NMOSD. L’impact clinique et therapeutique de cette constatation reste a preciser.

Comparativement aux 43 % trouves dans notre cohorte, cette meilleure reponse peut etre liee a une frequence plus elevee d’atteintes neurologiques centrales dans notre serie (71 % versus 41 %), et egalement a un biais de publication. Ces taux de reponse dans les histiocytoses non-Langerhansiennes restent inferieurs a ceux observes dans les series d’histiocytoses Langerhansiennes. Conclusion La cladribine est une option therapeutique interessante dans les histiocytoses non-Langerhansiennes, avec un taux de reponse entre 43–59 %. La tolerance est excellente malgre une lymphopenie constante.

SLE-related acute lung disease (ALD) is rare, poorly investigated and can lead to acute respiratory failure. We conducted a retrospective study aiming to describe clinical features, treatments and outcome of SLE-related APD. Methods We retrospectively included all patients with SLE and ALD admitted from November 1996 and September 2018 to La Pitié-Salpêtrière Hospital, after exclusion of viral or bacterial lung infection, cardiac failure or any other alternate diagnosis. Introduction Les maladies du spectre des neuromyelites optiques (neuromyelitis optica spectrum di... more Introduction Les maladies du spectre des neuromyelites optiques (neuromyelitis optica spectrum disorder, NMOSD) sont des maladies demyelinisantes auto-immunes du systeme nerveux central. Elles se manifestent par des episodes recidivants de myelites et de nevrite optique (NO). Le plus souvent, elles sont associees a la presence d’anticorps anti-aquaporine 4 (anti-AQP4). L’association d’une NMOSD anti-AQP4+ et d’une autre maladie auto-immune n’est pas exceptionnelle. En particulier, l’association avec le lupus systemique (LS) et le syndrome de Sjogren primaire (SS) a ete rapportee dans la litterature, sans que l’on sache si cette association avait un impact sur la prise en charge et le pronostic. L’objectif de l’etude etait d’analyser les caracteristiques cliniques, radiologiques, et le suivi a long terme des patients presentant une NMOSD avec anticorps anti-AQP4 associee a un LS et/ou un SS. Patients et methodes Il s’agissait d’une etude retrospective menee dans 4 centres. Les patients presentant une NMOSD avec anticorps anti-aquaporine 4 (criteres IPND 2015) et un LS (criteres EULAR/ACR 2019) ou un SS (criteres ACR/EULAR 2016) ont ete retrospectivement inclus. Les caracteristiques cliniques, biologiques et radiologiques en lien avec la NMOSD, le LS et/ou le SS ont ete recueillies a partir des dossiers medicaux. Lorsque cela etait possible, nous avons cherche la presence d’anticorps anti-AQP4 dans le serum des patients anterieurs aux premieres manifestations clniques de la NMOSD. Resultats Quinze patients presentant une NMOSD avec anticorps anti-AQP4 associee a un LS et/ou un SS (9 patients presentaient un LS, 5 patients un SS et 1 patients un LS associe a un SS) ont ete inclus. La duree de suivi mediane etait de 120 mois (4 a 336 mois) a partir du diagnostic de NMOSD. Quatorze patients (93 %) etaient des femmes et leur âge median lors de la premiere manifestation de NMOSD etait de 40 ans (11–56 ans). Le diagnostic de LS et/ou de SS precedait celui de NMOSD chez 10 patients (67 %) avec un delai median de 72 mois (4–168 mois). Le diagnostic de NMOSD etait concomitant de celui de LS et/ou SS chez 3 patients (20 %) et le precedait chez 2 patients (13 %). Les patients lupiques presentaient une atteinte cutaneo-articulaire dans 90 % des cas ; des cytopenies auto-immunes dans 30 % des cas, et une atteinte des sereuses ou une atteinte renale dans 30 % des cas chacun. Deux patients remplissaient les criteres de syndrome des anticorps anti-phospholipides. Tous les patients avaient des anticorps antinucleaires positifs, 13/15 des anticorps anti-ADN natif, 5/15 des anticorps anti-SS-A, 2/16 des anticorps anti-RNP, 1/15 des anticorps anti-SS-B, et 1/15 des anticorps anti-Sm. Le nombre moyen de poussees de NMOSD par patient etait de 2,7 (1–6). Les manifestations incluaient une atteinte medullaire dans 80 % des cas, une NO dans 60 % des cas, et une atteinte encephalique dans 33 % des cas (dont un syndrome de l’area postrema). Au diagnostic de NMOSD, les patients avec un LS et/ou SS prealable etaient traites par hydroxychloroquine dans 60 % des cas, prednisone dans 20 % des cas (tous a la dose de 5 milligrammes par jour), et azathioprine dans 10 % des cas. Les poussees de NMOSD n’etaient pas concomitantes de poussees de LS chez nos patients. Seul un patient a presente, lors de sa cinquieme poussee de NMOSD, une pericardite avec elevation du titre d’anticorps anti-ADN natifs. Les poussees de NMOSD etaient traitees par bolus de methylprednisolone dans 60 % des cas, par rituximab dans 49 % cas, par cyclophosphamide dans 12 % des cas, et par echanges plasmatiques dans 7 % des cas. Les molecules utilisees comme traitement d’entretien etaient le rituximab dans 60 % des cas, l’azathioprine dans 47 % des cas, et le mycophenolate mofetil dans 33 % des cas. Parmi nos 15 patients, 14 etaient en remission lors de la derniere consultation, apres une duree mediane de 33 mois (6–240 mois). Chez un patient, nous avons pu effectuer la recherche d’anticorps anti-AQP4 dans un serum preleve deux ans avant les manifestations cliniques de la NMOSD, recherche qui s’est averee positive. Conclusion Les NMOSD associees aux anti-AQP4 peuvent etre concomitantes d’un LS ou d’un SS, parfois peu severe, et peuvent survenir plusieurs annees apres le diagnostic de LS ou SS, et en dehors des poussees de ces dernieres. Par ailleurs, les anticorps anti-AQP4 pourraient apparaitre quelques annees avant le diagnostic de la NMOSD. L’impact clinique et therapeutique de cette constatation reste a preciser.
