Prima di tornare sul sogno di questa paziente, il cui significato credo che sia quasi trasparente ed intuibile ai più, facciamo un brevissimo quadro della situazione attuale. Epidemiologia Psicopatologica Un italiano su tre ha sviluppato sintomi di depressione legati all'epidemia causata dal Covid-19 (dati della Società Italiana di Neuropsicofarmacologia). In uno studio coordinato dal Dipartimento di Salute Mentale dell'Università della Campania "Luigi Vanvitelli" finalizzato a valutare le aree del funzionamento psicosociale, tra cui la presenza di sintomi dello spettro ansioso-depressivo, ossessivo-compulsivo e post-traumatico da stress, i risultati, ottenuti da un campione di 20.720 partecipanti, evidenziano che durante il lockdown sono aumentati i livelli di ansia, depressione e sintomi legati allo stress, soprattutto nei soggetti di sesso femminile. Inoltre, la durata dell'esposizione al lockdown ha rappresentato un fattore predittivo significativo del rischio di presentare peggiori sintomi ansioso-depressivi. Erano gli inizi degli anni ottanta ed a Couvet, un delizioso paesino della Svizzera francese situ... more Erano gli inizi degli anni ottanta ed a Couvet, un delizioso paesino della Svizzera francese situato nella Val de Travers, dove risiedeva Silvio Fanti, il fondatore della Micropsicoanalisi, più volte noi giovanissimi medici e psicologi che eravamo li a fare la nostra formazione psicoanalitica udimmo "il Dottore" parlare dell'avvento venturo della "sindrome paranoide universale". I nostri studi già ci permettevano di pesare l'enormità dell'affermazione. Fanti intendeva dire che l'umanità si sarebbe orientata verso una patologia di massa che avrebbe avuto come perno il meccanismo di difesa della Proiezione. Non nascondo che molti di noi, me compreso, la presero come una boutade. La proiezione Vediamo innanzitutto di cosa stiamo parlando in dettaglio. La proiezione è un meccanismo di difesa primario che agisce nelle menti di ognuno di noi che consiste nello spostare su un soggetto esterno, o una situazione esterna, qualità, sentimenti, desideri e persino degli "oggetti" che egli non riconosce o rifiuta in sé. Si "sputa" all'esterno tutto ciò che ci disturba e lo si recupera come persecuzione. Diciamo subito che chiunque di noi è un proiettivo e che spesso un esame di realtà ben fatto di una situazione di conflitto a volte ci fa recedere da un vissuto persecutorio: proverbiali sono i litigi nelle compagnie di amici, spesso scatenati da equivoci banali. Fanti è purtroppo scomparso nell'anno 1997, ben prima dell'esplosione dei social media, un fenomeno che ha avuto ripercussioni epocali nello psichismo di massa. Il ruolo esplosivo dei social media In un articolo che pubblicai nel 2009, agli albori dell'esplosione del fenomeno, "La psicologia di Facebook", avevo già messo in luce le dinamiche che poi sarebbero dilagate. Ma ancora non si era manifestati due fenomeni evidenti: la manipolazione dell'informazione che i social perseguono ai fini della tutela del loro immenso potere finanziario (consiglio la rilettura del bellissimo articolo di Paolo Sordi e Domenico Fiormonte "Come controllare Internet in sei mosse: geopolitica dell'oro digitale" pubblicato su questa rivista) e l'esplosione incontrollata della violenza verbale che a stento, e spesso strumentalmente, l'algoritmo di tutela dei social riesce a contenere. Quest'ultimo fenomeno è reso possibile dall'assenza della corporeità dell'interlocutore che fa venir meno le prudenze istintive legate all'animalità della risposta. Assistiamo da una parte ad una diffusione quasi pandemica (questa si!) di una nevrosi traumatica: i cittadini sono shoccati, come instupiditi, disorientati, nessuno ha provveduto a dire loro che sta morendo lo 0,05 della popolazione (avete letto bene). E nessuno ha provveduto a ricordare loro che le epidemie passano, svaniscono, cessano naturalmente di essere un pericolo. Si è preferito instillare un senso di indefinibile terrore senza confini spazio/temporali.
Non vi è nulla di più terrifico di un pericolo senza forma, non rappresentabile, non contenibile nel tempo e nello spazio: è una regressione massiva ai fantasmi della prima infanzia: il buio indefinito che si anima di mostri letali.
Nel 1994 scrissi un libro dal titolo “la vita involucro vuoto”: non credevo di vederlo realizzato in una sorta di sindrome paranoide universale.
Gli arresti domiciliari a cui è stata costretta la popolazione (solo in poche altre nazioni dittatoriali, Cina in primis, si è osato tanto), hanno determinato una abolizione del contatto sociale, l’aspetto che rende umano l’animale uomo. Quando le relazioni umane vengono abolite, si badi bene, quella virtuale non può neppur lontanamente eguagliare quella reale, poiché è enormemente falsificabile (vedi qui), l’essere umano è costretto ad un dialogo interno con le sue proiezioni, ritorna in uno stadio narcisistico pre-relazionale rieditando meccanismi di difesa di tipo paranoide estremamente pericolosi. L’aggressività endofamiliare aumenta e sappiamo dalla cronaca quanti episodi di agiti violenti ci siano stati.

Un mio paziente esce regolarmente da oltre trent'anni al mattino per fare una passeggiata veloce.... more Un mio paziente esce regolarmente da oltre trent'anni al mattino per fare una passeggiata veloce. La totale sedentarietà della professione che pratica lo predispone ad un pericoloso aumento di peso con complicanze ipertensive. La sua camminata è propriamente un farmaco salva-vita. Ha accettato con pena ed apprensione le limitazioni alla mobilità che il governo italiano ha deciso per fronteggiare l'epidemia di covid-19. Come il lettore saprà tantissime nazioni nel mondo, anche in Europa, pur chiudendo tutte le attività che implicassero una inevitabile concentrazione di persone, non hanno letteralmente sequestrato in casa la popolazione come è stato invece fatto in Italia, ma si sono limitate a vigilare attentamente che negli spazi pubblici non si producessero assembramenti e che la distanza tra persone fosse rispettata. Quando si corre o si cammina a fini sportivi all'aperto comunque, nell'attuale fase-2, nemmeno in Italia è richiesto l'uso della mascherina (pena un'ecatombe di accidenti cardiovascolari!) purché, giustamente!, si rispetti la distanza utile (1 metro per chi cammina, 2 metri per chi corre) ad evitare il contagio. Quindi nel più stretto rispetto della legge in tutte le diverse fasi il paziente si è concesso le sue sortite salva-vita. Andando incontro ad una serie interminabile di contumelie e minacce, pur tenendosi rigorosamente lontano, ben oltre le disposizioni di legge, dagli altri. Realizza ben presto di essere diventato, come migliaia di altri mal-definiti runner, un capro espiatorio. Avrete letto tutti, sui social, proposte più o meno serie di cecchinaggio dei runner dalle finestre delle case dei reclusi.

Never before has an issue regarding health had such vast media coverage. For obvious reasons, neither the Black Death which devastated Europe between 1347-1352 killing over a quarter of the population nor the Spanish flu epidemic which between 1918-1920 infected 200 million people causing over 10 million victims, had such coverage. The web, social media, whatsapp, Telegram, basically every modern Channel of communication bombard us with approximate and sensationalist information, often tailor-made and implausibly overdramatised for the sake of gaining exposure in the immense planetary magma of information. The virus is much more present psychically than in substance. To get an idea it is sufficient to look at the spike in the web search query "coronavirus" typed on google, shown in the image below. It is obviously the most searched key word in Italy.

Mai prima d'ora un fatto che riguarda la salute ha avuto una simile copertura mediatica. Per ovvie ragioni non lo ebbe la peste nera che devastò l'Europa dal 1347 al 1352, uccidendo oltre un quarto della popolazione, e l'epidemia di influenza spagnola che dal 1918 al 1920 contagiò 200 milioni di persone causando più di 10 milioni di vittime. Il web, i social, whatsapp, Telegram, insomma, ogni canale moderno di trasmissione di informazioni ci bombarda di notizie approssimate e sensazionalistiche spesso montate ad arte, drammatizzate oltre l'inverosimile, pur di ottenere visibilità nell'immenso magma dell'informazione planetaria. Il virus è ben più presente psichicamente che in sostanza. Per farsene un'idea basterà dare un'occhiata all'impennata della query "coronavirus" digitata su google proposta dall'immagine. E' ovviamente la parola chiave più ricercata in Italia. Il virus nella filmografia e nella letteratura Il virus ancor più del batterio ha da sempre affascinato la mente dell'uomo. Basterà pensare alla marea di film e romanzi dedicati all'argomento. Farne una lista esauriente sarebbe impossibile. Ne citeremo alcuni anche grazie alle indicazioni dell'esperto (Alessandro Gallo-comunicazione personale). "Resident Evil" di Paul W.S. Anderson (2002) è sicuramente il più conosciuto tra i giovani (è tratto da un videogioco): il virus T, dopo aver sterminato gli scienziati che stavano lavorando alla sua creazione, tramuta gli umani in zombie mutanti. Dello stesso anno "Ventotto giorni dopo" di Danny Boyle: qui si scardina la consolidata immagine dell'imperfezione degli zombie: anziché trascinarsi goffamente, corrono! In "The Road" di John Hillcoat (2010), tratto dall'omonimo romanzo di Cormac McCarthy, ci fa seguire le imprese di un padre (Viggo Mortensen) e un figlio in viaggio per gli Stati Uniti alla ricerca di un posto più pacifico. Dunque la massiccia attività proiettiva che si sviluppa nelle fasi dell’aurora dell’amore esplicitata nella frase frequentissima “E’ come se ti conoscessi da sempre”, sono necessarie per sedare il rigetto. Un proverbio di comune saggezza dice: “Chi si somiglia, si piglia”. E questo è il ruolo della proiezione: renderci somiglianti, se non a noi, ad un prototipo precedentemente assimilato.

Chi si occupa di psicopatologia non avrà difficoltà alcuna a ravvisare in questa bizzarra moda una condotta da evitamento dell’oggetto fobico, in altri termini una inclinazione fobica, mascherata da una sistematizzazione ideologica.
Dobbiamo allora partire, per cercare di capire il fenomeno, dalla definizione di fobia.
Il termine fobia (dal greco φόβος, “panico, paura”) indica un’irrazionale e persistente paura e repulsione verso certe situazioni, oggetti, attività, animali o persone, e finalmente, aggiungiamo, aggiornandoci, alimenti, che può, nei casi più gravi limitare l’autonomia del soggetto.
La fobia non è una forma nosologica a sé stante, potendo manifestarsi in diverse forme cliniche, come la nevrosi ossessiva, o l’isteria d’angoscia.
A mio parere quando il meccanismo dello spostamento è preponderante e lo è anche la valenza simbolica dell’oggetto fobico ci si situa più sul versante dell’isteria d’angoscia mentre quando i rituali e le preoccupazioni rispetto al mangiar sano sono in primo piano è interessato l’asse ossessivo-compulsivo.

Je retiens que le noyau de quelque forme de psychopathologie soit l’impact et l’élaboration de l’angoisse de mort. On comprend donc aisément comment chez un sujet atteint d’une maladie néoplastique maligne se déclenche immédiatement une activation massive des mécanismes de défense psychiques, surtout ceux proches de la psychose, le déni et la négation. Le déni de la maladie est toujours et dans tous les cas un problème grave, il est aisément compréhensible que dans une forme pathologique comme le cancer un diagnostic précoce et le traitement approprié immédiat sont d’une importance fondamentale afin d’endiguer les progrès de la maladie!
Le psychothérapeute qui suit un malade cancéreux aurait tout intérêt à se libérer de vécus d’omnipotence et à se souvenir qu’il peut faire énormément pour le malade (redimensionner et lénifier son angoisse de mort) mais il doit se tenir hors du champ de la maladie organique. Son intervention devra être orientée avant toute chose vers l’affrontement et la recherche de solutions destitnées à amener le malade à reconnaître son état et à accepter les traitements que la médecine est à même de lui offrir, même si ceux-ci ne sont pas encore tout à fait au point d’une part et certainement encore douloureux et invalidants.

I believe that the founding nucleus of any psychopathological form is the impact and the elaboration of death anxiety. Thus is comprehensible why in a subject affected by a neoplastic malignant disease, a massive activation of psychic mechanisms of defence occurs, above all the ones near to psychosis: the denial and the negation. Denial of desease is always a serious problem, most of all in a pathological form as cancer, in which early diagnosis and the beginning of appropriate therapies, are very important for the possibilities of stopping the process.
Therefore the psychotherapist who follows oncologic patients should be free from omnipotence desires and remind himself that he can do very much for the patients (to reduce and calm their death anxiety), but the organic illness does not concern his interest. First of all the intervention must be directed in facing and trying to resolve, as soon as possible, the opposition of the patients into recognition of their own state and the refusal of offered treatments, although not yet perfect and surely still painful and disabling.

Ritengo che il nucleo fondante di qualsiasi forma psicopatologica sia l’impatto e l’elaborazione dell’angoscia di morte. Si comprenderà dunque come in un soggetto colpito da una malattia neoplastica maligna vi sia immediatamente un’attivazione massiva dei meccanismi di difesa psichici, soprattutto quelli vicini alla psicosi, il diniego e la negazione. Il diniego della malattia è sempre e comunque un problema grave, figurarsi in una forma patologica come il cancro in cui precocità della diagnosi ed inizio delle appropriate terapie, sono di fondamentale importanza per le possibilità di arresto della malattia.

Therefore the psychotherapist who follows oncologic patients should be free from omnipotence desires and remind himself that he can do very much for the patients (to reduce and calm their death anxiety), but the organic illness does not concern his interest. First of all the intervention must be directed in facing and trying to resolve, as soon as possible, the opposition of the patients into recognition of their own state and the refusal of offered treatments, although not yet perfect and surely still painful and disabling.

where in the First, the sacrifice was exclusively on the shoulders of young people and death was sown on the front only; and where the Second, despite its immense destruction, had an enemy to fight and hate, and human comfort and solidarity were not only allowed but sought after and cared for. Today we do not even have the comfort of the binding on the persecutory object: “the virus is impalpable, unrepresentable” we are told that it is everywhere, and also that we are responsible for this havoc. Nobody cares to explain to us why Italy (the lower case is a must) is the worst country in the world of the so-called civilized West for the correlation between the deaths and the number of people infected (case-fatality ratio), and the second worst, after Belgium, for mortality (deaths in relation to the population).
Assistiamo da una parte ad una diffusione quasi pandemica (questa si!) di una nevrosi traumatica: i cittadini sono shoccati, come instupiditi, disorientati, nessuno ha provveduto a dire loro che sta morendo lo 0,05 della popolazione (avete letto bene). E nessuno ha provveduto a ricordare loro che le epidemie passano, svaniscono, cessano naturalmente di essere un pericolo. Si è preferito instillare un senso di indefinibile terrore senza confini spazio/temporali.
Non vi è nulla di più terrifico di un pericolo senza forma, non rappresentabile, non contenibile nel tempo e nello spazio: è una regressione massiva ai fantasmi della prima infanzia: il buio indefinito che si anima di mostri letali.
Nel 1994 scrissi un libro dal titolo “la vita involucro vuoto”: non credevo di vederlo realizzato in una sorta di sindrome paranoide universale.
Gli arresti domiciliari a cui è stata costretta la popolazione (solo in poche altre nazioni dittatoriali, Cina in primis, si è osato tanto), hanno determinato una abolizione del contatto sociale, l’aspetto che rende umano l’animale uomo. Quando le relazioni umane vengono abolite, si badi bene, quella virtuale non può neppur lontanamente eguagliare quella reale, poiché è enormemente falsificabile (vedi qui), l’essere umano è costretto ad un dialogo interno con le sue proiezioni, ritorna in uno stadio narcisistico pre-relazionale rieditando meccanismi di difesa di tipo paranoide estremamente pericolosi. L’aggressività endofamiliare aumenta e sappiamo dalla cronaca quanti episodi di agiti violenti ci siano stati.
Dunque la massiccia attività proiettiva che si sviluppa nelle fasi dell’aurora dell’amore esplicitata nella frase frequentissima “E’ come se ti conoscessi da sempre”, sono necessarie per sedare il rigetto. Un proverbio di comune saggezza dice: “Chi si somiglia, si piglia”. E questo è il ruolo della proiezione: renderci somiglianti, se non a noi, ad un prototipo precedentemente assimilato.
Il termine fobia (dal greco φόβος, “panico, paura”) indica un’irrazionale e persistente paura e repulsione verso certe situazioni, oggetti, attività, animali o persone, e finalmente, aggiungiamo, aggiornandoci, alimenti, che può, nei casi più gravi limitare l’autonomia del soggetto. La fobia non è una forma nosologica a sé stante, potendo manifestarsi in diverse forme cliniche, come la nevrosi ossessiva, o l’isteria d’angoscia. A mio parere quando il meccanismo dello spostamento è preponderante e lo è anche la valenza simbolica dell’oggetto fobico ci si situa più sul versante dell’isteria d’angoscia mentre quando i rituali e le preoccupazioni rispetto al mangiar sano sono in primo piano è interessato l’asse ossessivo-compulsivo.
Le psychothérapeute qui suit un malade cancéreux aurait tout intérêt à se libérer de vécus d’omnipotence et à se souvenir qu’il peut faire énormément pour le malade (redimensionner et lénifier son angoisse de mort) mais il doit se tenir hors du champ de la maladie organique. This explains, among other things, the frequency of the vicissitudes of possession or the delusion of influence in the paranoiac psychosis: the paranoiac is effectively a person possessed by the messages of the Image which are transmitted through an oneiric activity that is not sufficiently dreamed-off; he is a loudspeaker who tries desperately, without managing, to become a transmitting centre.
The obvious connection between a dream not dreamed-off and a delusion was already evident in Freud’s research.
