This article presents reflections about some questions emerged in the papers that several scholar... more This article presents reflections about some questions emerged in the papers that several scholars have produced for the Special Issue of Rivista di Psicologia Clinica 1/2014, dedicated to mental illness. The author here remarks the fact that many scholars agree on criticising the approaches mainly based on the individual as they are considered reductive and ineffective. He takes in consideration the different theoretical and operational methods exposed in the papers that go beyond the focus on the individual in the intervention for mental illness. The author's analysis pinpoints the most controversial aspects present in the various proposals and proceeds with the thesis that sometimes in the intervention the shift of focus towards the relationship between individual and social context occurs in the absence of a clear theoretical model. Paniccia, Di Ninni and Cavalieri (2006), highlighted two historical cultural trends informing CSM 1 operative patterns: 1) Militant ideology, prevailing in the first period, characterized by a de-institutionalizing culture and by sacrificial, oblational attitudes which don’t require any verification; 2) Technicality, where the therapeutic relationship is characterized by a feeble patient relying on a sound, skilled specialist.

Da molti anni, sia per i corsi di laurea in psicologia che per le scuole di specializzazione, il ... more Da molti anni, sia per i corsi di laurea in psicologia che per le scuole di specializzazione, il tirocinio nei servizi socio sanitari e considerato dalla normativa vigente prassi obbligata del percorso formativo. Tale obbligatorieta ci sembra abbia fatto prevalere una modalita adempitiva di guardare al problema piuttosto che un’attenta riflessione sul senso e sull’utilizzabilita dei tirocinanti dentro le strutture socio-sanitarie. Proviamo a fare un’ipotesi sulle ragioni di tale disattenzione.

Vengono descritti i cambiamenti della domanda e le trasformazioni avvenute nel Centro di Salute Mentale, dalla sua istituzione in Italia. Sono presi in esame i problemi clinici e organizzativi intervenuti in seguito all’evoluzione della domanda. Questa può essere letta attraverso categorie diagnostiche, oppure quale segno di problemi emozionali della convivenza. Nel Centro di Salute Mentale prendono forma due settori d’intervento: - - l’area dei disturbi emotivi comuni; l’area delle malattie mentali e dei casi complessi. Abstract
Mental Health Center: The space of clinical psychological intervention between psychiatry and psychotherapeutic techniques.
This paper describes the changes in the demand and the transformations occurred in the Mental Health Center since its foundation in Italy. The paper analyzes the clinical and organizational problems intervened after the evolution of the demand, which can be read through diagnostic categories or as sign of emotional problems in the social coexistence milieau. Two areas of intervention take shape in the Mental Health Center:-illness the area of common emotional disorders;-the area of mental and complex cases. Two clinical cases are discussed, for each of the two corresponding areas, in order to analyze the emerging clinical and organizational problems and the different perspectives of intervention. The analysis of the clinical cases here proposed tries to point out that, unlike psychiatry and psychotherapy organized on dual settings, clinical psychology-as reference discipline for intervention in the dynamics between individual/coexistence environment/Service-can be more effective answer to the new challenges deriving from the ongoing transformations.

Sono presentate riflessioni su alcuni temi emersi dagli scritti che diversi studiosi hanno prodotto per la Special Issue della Rivista di Psicologia Clinica N. 1/2014 dedicato alla malattia mentale. Si rileva la concordanza dei vari Autori nella critica agli approcci fondati prevalentemente sull’individuo, ritenuti riduzionistici e poco efficaci. Sono presi in esame i diversi modelli teorici e operativi esposti nei contributi che prevedono, nell’intervento per le malattie mentali, il superamento della centratura sull’individuo. L’analisi si sofferma sugli aspetti più controversi presenti nelle varie proposte. Viene sostenuta la tesi che lo spostamento di focus dell’intervento verso la relazione tra individuo e contesto sociale avviene, talvolta, in assenza di un modello teorico chiaro. Nell’intervento finalizzato all’integrazione dei malati mentali nell’ambiente di convivenza questo limite comporta il rischio di una definizione opaca della competenza psicologica, confondendola con un’attività fondata sull’impegno sociale e umanitario. Si rimarca la necessità di sviluppare una stretta correlazione tra prassi che implicano interventi sui sistemi di convivenza e modelli teorici che ne chiariscano il senso. Attraverso l’analisi di un episodio clinico sono evidenziati differenti percorsi dell’intervento terapeutico in un servizio di salute mentale con l’obiettivo di segnalare problemi, limiti e potenzialità di ognuno di essi. Le dinamiche relazionali presenti nella triade ‘individuo/ambiente di convivenza/Servizio’ sono proposte quale oggetto di competenza dell’intervento clinico sui problemi relativi alla malattia mentale. La rilevazione delle configurazioni assunte dalle simbolizzazioni affettive entro la suddetta cornice triadica permette l’individuazione di potenziali contesti critici ricorrenti. Viene esplorata la costruzione di un sistema di categorizzazione dei fenomeni clinici espressi nella domanda, alternativo rispetto ai principali sistemi di classificazione imperniati su sintomi e tratti di personalità. Si delineano, infine, potenziali linee di sviluppo e riorganizzazione del Centro di Salute Mentale.

The article analyses the issue of the different types of setting and of the clinical tools available to the
psychologist in his/her intervention. It is assumed that the non interpretative interventions, although widely
employed in the clinical practice, have not found a suitable theoretical and methodological systematization.
The article reports and comments the way different types of setting and of operative tools have been set in
four brief clinical episodes occurred in a mental health centre. Further on are presented enquiries and
reflections about the criteria presiding over the modulation of the different clinical tools throughout the
intervention. The tools of the intervention, including the interpretation, are referred to as the psychologist’s
relational proposals aiming at the achievement of the objectives. In the absence of a unifying theory of the
technique, the considerable variety and the enhancement of the clinical tools available, expose to risk of
fragmentary and methodologically incoherent praxsis. It is argued that the creative exploration of new tools
in the clinical field, starting from the critical analysis of the individualist paradigm, enhances the potentiality
of the psychological intervention and contributes to the reflection on the theoretical-methodological
constructs including those inherent the classical dual setting.

Viene affrontato il tema delle diverse tipologie di setting e di strumenti clinici che lo psicologo ha a
disposizione nel proprio intervento. Si assume che gli interventi non interpretativi, pur ampiamente utilizzati
nella prassi clinica, non abbiano trovato una sufficiente sistematizzazione teorico metodologica.
Sono riferiti e commentati quattro brevi episodi clinici occorsi in un Centro di Salute Mentale nei quali sono
stati approntati diversi tipi di setting e di strumenti operativi. A seguire sono presentate domande e riflessioni
circa i criteri che presiedono la modulazione dei diversi strumenti clinici nel corso dell’intervento.
I dispositivi dell’intervento, interpretazione compresa, sono trattati in termini di proposte relazionali dello
psicologo finalizzate alla realizzazione di obiettivi. La notevole varietà e l’arricchimento degli strumenti
clinici a disposizione espone al rischio, in assenza di una teoria della tecnica unificante, di una operatività
frammentaria e sotto il profilo metodologico poco coerente. A partire dall’analisi critica del paradigma
individualista viene sostenuta l’ipotesi che l’esplorazione creativa sul campo clinico di nuovi dispositivi
accresca le potenzialità dell’intervento psicologico clinico e contribuisca al ripensamento dei costrutti
teorico-metodologici, ivi compresi quelli inerenti il classico setting duale.

This paper analisys the current state of the psychotherapeutic interventions in Mental Health Cen... more This paper analisys the current state of the psychotherapeutic interventions in Mental Health Centers, with regard to the significant increase of demand in the area of the so-called common emotional deseases and to the underlying managment question they pose to Mental Health Units in order to low avalaible resouces.
A clinical exprerience of Admission Process and Short Term Unit in a Mental Health Center is reported. The theoretical assumptions of psychodynamic short psychotherapy stressed by various authors are critically analyzed highlighting the poor applicability in the services. Within the same psychodynamic paradigm a different model of short term intervention is proposed in which the theorical and cilinical assumptions entail the contestual and organizzative aspect of the institution. The paper include various clinical situations in order to exemplify the proposed model.

Vengono descritti i cambiamenti della domanda e le trasformazioni avvenute n... more Abstract
Vengono descritti i cambiamenti della domanda e le trasformazioni avvenute nel Centro di Salute Mentale, dalla sua istituzione in Italia. Sono presi in esame i problemi clinici e organizzativi intervenuti in seguito all’evoluzione della domanda. Questa può essere letta attraverso categorie diagnostiche, oppure quale segno di problemi emozionali della convivenza. Nel Centro di Salute Mentale prendono forma due settori d’intervento: - - l’area dei disturbi emotivi comuni; l’area delle malattie mentali e dei casi complessi. Sono discussi due casi, ognuno per la corrispettiva area, con la finalità di analizzare sia la natura dei problemi clinici e organizzativi, sia le differenti prospettive d’intervento. L’analisi dei casi clinici qui proposta cerca di evidenziare che, a differenza della psichiatria e delle tecniche psicoterapeutiche organizzate in setting duali, la psicologia clinica – in qualità di disciplina competente a intervenire sulle dinamiche affettive tra individuo/ambiente di convivenza/ servizi socio sanitari – può rivelarsi più efficace nella risposta alle nuove sfide poste dalle trasformazioni in atto.

Abstract
Mental Health Center: The space of clinical psychological intervention between psychiatry and psychotherapeutic techniques.
This paper describes the changes in the demand and the transformations occurred in the Mental Health Center since its foundation in Italy. The paper analyzes the clinical and organizational problems intervened after the evolution of the demand, which can be read through diagnostic categories or as sign of emotional problems in the social coexistence milieau. Two areas of intervention take shape in the Mental Health Center:-illness the area of common emotional disorders;-the area of mental and complex cases. Two clinical cases are discussed, for each of the two corresponding areas, in order to analyze the emerging clinical and organizational problems and the different perspectives of intervention. The analysis of the clinical cases here proposed tries to point out that, unlike psychiatry and psychotherapy organized on dual settings, clinical psychology-as reference discipline for intervention in the dynamics between individual/coexistence environment/Service-can be more effective answer to the new challenges deriving from the ongoing transformations.

Sono presentate riflessioni su alcuni temi emersi dagli scritti che diversi studiosi hanno prodotto per la Special Issue della Rivista di Psicologia Clinica N. 1/2014 dedicato alla malattia mentale. Si rileva la concordanza dei vari Autori nella critica agli approcci fondati prevalentemente sull’individuo, ritenuti riduzionistici e poco efficaci. Sono presi in esame i diversi modelli teorici e operativi esposti nei contributi che prevedono, nell’intervento per le malattie mentali, il superamento della centratura sull’individuo. L’analisi si sofferma sugli aspetti più controversi presenti nelle varie proposte. Viene sostenuta la tesi che lo spostamento di focus dell’intervento verso la relazione tra individuo e contesto sociale avviene, talvolta, in assenza di un modello teorico chiaro. Nell’intervento finalizzato all’integrazione dei malati mentali nell’ambiente di convivenza questo limite comporta il rischio di una definizione opaca della competenza psicologica, confondendola con un’attività fondata sull’impegno sociale e umanitario. Si rimarca la necessità di sviluppare una stretta correlazione tra prassi che implicano interventi sui sistemi di convivenza e modelli teorici che ne chiariscano il senso. This article presents reflections about some questions emerged in the papers that several scholar... more This article presents reflections about some questions emerged in the papers that several scholars have produced for the Special Issue of Rivista di Psicologia Clinica 1/2014, dedicated to mental illness. The author here remarks the fact that many scholars agree on criticising the approaches mainly based on the individual as they are considered reductive and ineffective. He takes in consideration the different theoretical and operational methods exposed in the papers that go beyond the focus on the individual in the intervention for mental illness. The author's analysis pinpoints the most controversial aspects present in the various proposals and proceeds with the thesis that sometimes in the intervention the shift of focus towards the relationship between individual and social context occurs in the absence of a clear theoretical model. In the intervention which aims at the integration of the mentally ill patient in the coexistence milieu, this limit involves the risk of an opaque definition of psychological expertise, getting it mistaken for an activity based on social and humanitarian efforts – hence the necessity to develop a tight correlation between praxis involving interventions on coexistence milieus and theoretical models that clarify their sense.

Attraverso l’analisi di un episodio clinico sono evidenziati differenti percorsi dell’intervento terapeutico in un servizio di salute mentale con l’obiettivo di segnalare problemi, limiti e potenzialità di ognuno di essi. Le dinamiche relazionali presenti nella triade ‘individuo/ambiente di convivenza/Servizio’ sono proposte quale oggetto di competenza dell’intervento clinico sui problemi relativi alla malattia mentale. La rilevazione delle configurazioni assunte dalle simbolizzazioni affettive entro la suddetta cornice triadica permette l’individuazione di potenziali contesti critici ricorrenti. Viene esplorata la costruzione di un sistema di categorizzazione dei fenomeni clinici espressi nella domanda, alternativo rispetto ai principali sistemi di classificazione imperniati su sintomi e tratti di personalità. Si delineano, infine, potenziali linee di sviluppo e riorganizzazione del Centro di Salute Mentale.

The article analyses the issue of the different types of setting and of the clinical tools available to the
psychologist in his/her intervention. It is assumed that the non interpretative interventions, although widely
employed in the clinical practice, have not found a suitable theoretical and methodological systematization.
The article reports and comments the way different types of setting and of operative tools have been set in
four brief clinical episodes occurred in a mental health centre. Further on are presented enquiries and
reflections about the criteria presiding over the modulation of the different clinical tools throughout the
intervention. The tools of the intervention, including the interpretation, are referred to as the psychologist’s
relational proposals aiming at the achievement of the objectives. In the absence of a unifying theory of the
technique, the considerable variety and the enhancement of the clinical tools available, expose to risk of
fragmentary and methodologically incoherent praxsis. It is argued that the creative exploration of new tools
in the clinical field, starting from the critical analysis of the individualist paradigm, enhances the potentiality
of the psychological intervention and contributes to the reflection on the theoretical-methodological
constructs including those inherent the classical dual setting.

Viene affrontato il tema delle diverse tipologie di setting e di strumenti clinici che lo psicologo ha a
disposizione nel proprio intervento. Si assume che gli interventi non interpretativi, pur ampiamente utilizzati
nella prassi clinica, non abbiano trovato una sufficiente sistematizzazione teorico metodologica.
Sono riferiti e commentati quattro brevi episodi clinici occorsi in un Centro di Salute Mentale nei quali sono
stati approntati diversi tipi di setting e di strumenti operativi. A seguire sono presentate domande e riflessioni
circa i criteri che presiedono la modulazione dei diversi strumenti clinici nel corso dell’intervento.
I dispositivi dell’intervento, interpretazione compresa, sono trattati in termini di proposte relazionali dello
psicologo finalizzate alla realizzazione di obiettivi. La notevole varietà e l’arricchimento degli strumenti
clinici a disposizione espone al rischio, in assenza di una teoria della tecnica unificante, di una operatività
frammentaria e sotto il profilo metodologico poco coerente. A partire dall’analisi critica del paradigma
individualista viene sostenuta l’ipotesi che l’esplorazione creativa sul campo clinico di nuovi dispositivi
accresca le potenzialità dell’intervento psicologico clinico e contribuisca al ripensamento dei costrutti
teorico-metodologici, ivi compresi quelli inerenti il classico setting duale.

This paper analisys the current state of the psychotherapeutic interventions in Mental Health Cen... more This paper analisys the current state of the psychotherapeutic interventions in Mental Health Centers, with regard to the significant increase of demand in the area of the so-called common emotional deseases and to the underlying managment question they pose to Mental Health Units in order to low avalaible resouces.
A clinical exprerience of Admission Process and Short Term Unit in a Mental Health Center is reported. The theoretical assumptions of psychodynamic short psychotherapy stressed by various authors are critically analyzed highlighting the poor applicability in the services. Within the same psychodynamic paradigm a different model of short term intervention is proposed in which the theorical and cilinical assumptions entail the contestual and organizzative aspect of the institution. The paper include various clinical situations in order to exemplify the proposed model.

The models of clinical intervention are represented through three categorical axsis. Different approaches to therapeutic intervention in a Mental Health Service are highlighted, through the analysis of a clinical case, in order to point out problems, limits and potentialities of each approach. The relational dynamics inherent the triad “individual/ social coexistence milieu /Service” are proposed as object of the clinical intervention for problems concerning mental illness. The survey of the configurations assumed by the
emotional symbolizations within the above mentioned triadic frame allows to individuate potential recurrent critical contexts. The construction of a categorization system of the clinical phenomena expressed in the demand is here explored as an alternative to the main systems of classification pivoting on symptoms and personality traits. In the end, potential development lines of the Mental Health Center are analyzed along with its possible reorganization.

The article analyses the issue of the different types of setting and of the clinical tools available to the
psychologist in his/her intervention. It is assumed that the non interpretative interventions, although widely
employed in the clinical practice, have not found a suitable theoretical and methodological systematization.
The article reports and comments the way different types of setting and of operative tools have been set in
four brief clinical episodes occurred in a mental health centre. Further on are presented enquiries and
reflections about the criteria presiding over the modulation of the different clinical tools throughout the
intervention. The tools of the intervention, including the interpretation, are referred to as the psychologist’s
relational proposals aiming at the achievement of the objectives. In the absence of a unifying theory of the
technique, the considerable variety and the enhancement of the clinical tools available, expose to risk of
fragmentary and methodologically incoherent praxsis. It is argued that the creative exploration of new tools
in the clinical field, starting from the critical analysis of the individualist paradigm, enhances the potentiality
of the psychological intervention and contributes to the reflection on the theoretical-methodological
constructs including those inherent the classical dual setting.

Viene affrontato il tema delle diverse tipologie di setting e di strumenti clinici che lo psicologo ha a
disposizione nel proprio intervento. Si assume che gli interventi non interpretativi, pur ampiamente utilizzati
nella prassi clinica, non abbiano trovato una sufficiente sistematizzazione teorico metodologica.
Sono riferiti e commentati quattro brevi episodi clinici occorsi in un Centro di Salute Mentale nei quali sono
stati approntati diversi tipi di setting e di strumenti operativi. A seguire sono presentate domande e riflessioni
circa i criteri che presiedono la modulazione dei diversi strumenti clinici nel corso dell’intervento.
I dispositivi dell’intervento, interpretazione compresa, sono trattati in termini di proposte relazionali dello
psicologo finalizzate alla realizzazione di obiettivi. La notevole varietà e l’arricchimento degli strumenti
clinici a disposizione espone al rischio, in assenza di una teoria della tecnica unificante, di una operatività
frammentaria e sotto il profilo metodologico poco coerente. A partire dall’analisi critica del paradigma
individualista viene sostenuta l’ipotesi che l’esplorazione creativa sul campo clinico di nuovi dispositivi
accresca le potenzialità dell’intervento psicologico clinico e contribuisca al ripensamento dei costrutti
teorico-metodologici, ivi compresi quelli inerenti il classico setting duale.

A clinical exprerience of Admission Process and Short Term Unit in a Mental Health Center is reported. The theoretical assumptions of psychodynamic short psychotherapy stressed by various authors are critically analyzed highlighting the poor applicability in the services. Within the same psychodynamic paradigm a different model of short term intervention is proposed in which the theorical and cilinical assumptions entail the contestual and organizzative aspect of the institution. The paper include various clinical situations in order to exemplify the proposed model.

The models of clinical intervention are represented through three categorical axsis. Different approaches to therapeutic intervention in a Mental Health Service are highlighted, through the analysis of a clinical case, in order to point out problems, limits and potentialities of each approach. The relational dynamics inherent the triad “individual/ social coexistence milieu /Service” are proposed as object of the clinical intervention for problems concerning mental illness. The survey of the configurations assumed by the
emotional symbolizations within the above mentioned triadic frame allows to individuate potential recurrent critical contexts. The construction of a categorization system of the clinical phenomena expressed in the demand is here explored as an alternative to the main systems of classification pivoting on symptoms and personality traits. In the end, potential development lines of the Mental Health Center are analyzed along with its possible reorganization.

I modelli d’intervento clinico sono rappresentati tramite tre assi categoriali. Attraverso l’analisi di un episodio clinico sono evidenziati differenti percorsi dell’intervento terapeutico in un servizio di salute mentale con l’obiettivo di segnalare problemi, limiti e potenzialità di ognuno di essi. Le dinamiche relazionali presenti nella triade: ‘individuo/ambiente di convivenza/Servizio’ sono proposte quale oggetto di competenza dell’intervento clinico sui problemi relativi alla malattia mentale. La rilevazione delle configurazioni assunte dalle simbolizzazioni affettive entro la suddetta cornice triadica permette l’individuazione di potenziali contesti critici ricorrenti. Viene esplorata la costruzione di un sistema di categorizzazione dei fenomeni clinici espressi nella domanda, alternativo rispetto ai principali sistemi di classificazione imperniati su sintomi e tratti di personalità. Si delineano, infine, potenziali linee di sviluppo e riorganizzazione del Centro di Salute Mentale.
