Nonspecific periventricular white matter lucencies on computed tomograms (leukoaraiosis) were fou... more Nonspecific periventricular white matter lucencies on computed tomograms (leukoaraiosis) were found in 49 (55%) of 89 stroke patients. We compared the vascular risk factors and the types of stroke with and without leukoaraiosis. Patients with leukoaraiosis were significantly older than those without it. The duration of hypertension was longer than those without leukoaraiosis. Mean diastolic blood pressure was associated with leukoaraiosis. No association was found with systolic blood pressure, diabetes mellitus, angina pectoris, myocardial infarction, heart failure, valvular disease and intermittent claudication. Patients with leukoaraiosis were significantly more likely to have lacunar infarcts on computed tomograms but were less likely to have cortical infarcts, subcortical infarcts or cerebral hemorrhage.

The site of the lesion was found to be parietothalamic in 1 patient, posterolateral thalamic in 5 patients, dorsal thalamic in 2 patients and medial thalamic in 1 patient as defined by computerized tomography or magnetic resonance imaging. In addition to the hand dystonia, hemiballism was noted in 1 case, hemichorea in 2 cases, action tremor in 3, anxiety in 3 and pain in 2 cases. The time lapse from the stroke to the manifestation of dystonia was 1 month to 2 years.
Background: Atherosclerosis is the most common cause of ischemia in the periphery, coronary and c... more Background: Atherosclerosis is the most common cause of ischemia in the periphery, coronary and central nervous system. Ankle/Brachial Index (ABI) is the most important diagnostic test, which indicated the degree of occlusion in peripheral vascular diseases. This study was planned because there were limited number of studies comparing basically intracranial, extracranial (carotid vessels), peripheral vessels and coronary arteries being influenced with risk factors, carotid stenosis and ABI data in stroke case. Methods: Sixty patients with acute ischemic stroke were investigated prospectively for presence of atherosclerotic risk factors and markers, like hypertension, diabetes mellitus, hyperlipidemia, smoking, ischemic heart disease and claudicatio intermittens, in their medical histories. Carotid atherosclerosis was diagnosed by doppler ultrasonography, whereas intracranial atherosclerosis (leukoaraiosis) was diagnosed by cerebral computed tomography; coronary atherosclerosis was diagnosed by electrocardiography (ECG), and peripheral vessel atherosclerosis was diagnosed by ABI. According to degrees of carotid artery stenosis, patients were classified into three groups: mild, moderate and severe. Results: Although there was no significant differences in three patient groups, leukoaraiosis was significantly high in the moderate and severe stenosis groups (p=0.008). Mean age, smoking, ischemic heart disease and peripheral vessel atherosclerosis were confirmed significantly high in one-way analysis performed on severely stenotic group (p=0.02, p=0.001, p=0.05, p=0.02, respectively). There was a high degree of negative correlation between carotid artery stenosis and ABI (r=0.49). Conclusion: We detected a strong concomitance between carotid artery stenosis and smoking, coronary artery disease, cerebral atherosclerosis in cases with stroke. Also we detected a direct correlation between carotid stenosis and presence of leukoaraiosis, whereas an inverse correlation between carotid stenosis and ABI. These results indicated that large and small vessels might manifest parallel alterations on the same risk factor basis.

Daha öncesinde sağlıklı olan 29 yaşındaki erkek hasta, suçiçeği döküntülerinden beş gün sonra baş dönmesi, bulantı ve dengesizlik yakınması ile acil servise getirilmiştir. Hastanın yapılan ilk beyin-omurilik sıvısı (BOS) ve kan incelemelerinde Varicella-Zoster antikorları negatif bulunmasına karşılık onuncu günde yapılan incelemelerde hem kan hem BOS'ta antikorlar pozitif olarak saptanmıştır. Kontrastlı kraniyal manyetik rezonans görüntüleme (MRG)' sinde patoloji saptanmayan hastanın Technetium99m-hegzametilpropilenaminoksim (Tc-99m-HMPAO) ile işaretli beyin tek foton emisyon bilgisayarlı tomografi (single photon emission computed tomography: SPECT) incelemesinde; sağ serebral ve iki yanlı serebellar hipoperfüzyon saptanmıştır.
