Giriş: Semptomatik torakal disk hernileri oldukça nadir görülmesi, farklı klinik durumlarla ortay... more Giriş: Semptomatik torakal disk hernileri oldukça nadir görülmesi, farklı klinik durumlarla ortaya çıkması, ortak kabul edilebilir bir cerrahi endikasyonu olmaması ve farklı cerrahi girişimlerin olması nedeniyle tedavisi zor bir patolojidir. Biz bu çalışmada kalsifiye torakal disk hernili olgu serimizi ve sonuçlarını sunduk.Hasta ve Yöntemler: Bu geriye dönük çalışmada 2008-2013 yılları arasında torakal disk hernisi nedeniyle ameliyat edilmiş olgular incelendi. Bilgiler, temel demografik veriler, klinik bulgular, sonuçlar, ameliyat notları ve komplikasyon bilgilerini içeren hasta dosyalarından elde edildi.Sonuçlar: 10 olgunun 6'sı bayan, 4'ü erkekti (yaşları ortalaması 35,6 yıl olup 22-58 arasındaydı). Ortalama takip süresi 28,1 aydır . Bütün olgular transpediküler yaklaşımla ameliyat edildi ve 7 olguya posterior transpediküler fiksasyon uygulandı. Introduction: This report presents an unusual case of symptomatic vasospasm following resection o... more Introduction: This report presents an unusual case of symptomatic vasospasm following resection of posterior fossa tumor in a 9-year-old female patient. To the best of our knowledge, only 6 pediatric cases of vasospasms as a complication of brain tumor surgery have been reported in the literature previously. Case Presentation: After an uneventful 12 days postoperatively, the patient suddenly presented with acute neurological deterioration on the 13th day and MR angiography showed bilateral narrowing of the supraclinoid segments of the internal carotid artery and the proximal parts of the A1 and M1 segments. Hypervolemia and vasospasm treatments were administered, and the patient exhibited no neurological deficit at the time of discharge, confirmed via normal MRI. Conclusion: Early diagnosis and intervention in case of suspected symptomatic cerebral vasospasm after pediatric posterior fossa tumor surgery are essential in order to achieve favorable outcomes.
OBJECTIVE: This study evaluates the efficiency of interbody polyetheretherketone (PEEK) cage impl... more OBJECTIVE: This study evaluates the efficiency of interbody polyetheretherketone (PEEK) cage implantation in 85 consecutive cases treated for discogenic cervical disorders with radiculopathy or myelopathy. METHODS: Between the years 2002-2005, 85 patients were treated with cervical interbody fusion using a PEEK cage. There were 60 male and 25 female patients and the mean age was 46 years (range, 21-82 years). PEEK cages were packed with demineralised bone grafts or synthetic bone grafts. Additional plating was not used in any case. The median duration of follow-up was 12 months (range, 6-36 months). Cervical x-rays were routinely used in the followup to assess the fusion, pseudoarthrosis, kyphosis, cage migration, subsidence or breakage. RESULTS: No implant insufficiency was observed in any case. CONCLUSION: Efficient interbody replacement is still an ongoing problem in cervical surgery. We reviewed the effect of the extent of surgical resection and reoperation on the length and quality of survival in 152 consecutive patients who underwent operation for supratentorial gliomas at GATA Neurosurgery clinic between 1985 to 1995. Seventy-two patients (50%) had glioblastoma multiforme (GBM), and 48 patients (33%) had anaplastic astrocytoma (AA). Gross total resection was achieved in 70 cases (49%), subtotal resection was performed in 60 cases (42%), and biopsy was carried out in 14 cases (9%). Thirty-two patients were reoperated for recurrency and the median interval between the first operation and reoperation was 9.5 months in glioblastoma multiforme, and 11.7 months in anaplastic astrocytoma. The resection groups were compared for age, sex, preoperative and postoperative Karnofsky rating, tumor location, postoperative radiation therapy, and chemotherapy, and survival according to multivariate analysis. Preoperative Karnofsky rating and surgical resection type were the most important factors related to survival after operation or reoperation. The gross total resection group lived longer than the subtotal resection group by life table analysis. Median survival of GBM was 76 weeks in gross total resection group, and 33 months in AA group with total resection (p < 0.001). Preoperative Karnofsky scores had a statistically significant effect on the quality of life and survival after operation and reoperation in all cases (p = 0.005). Radical surgery and reoperation also improve quality and length of life in selective malignant supratentorial gliomas.
