OBJETIVOS El tratamiento fonoaudiológico de las disfonías, en otras palabras, terapia de voz, es ... more OBJETIVOS El tratamiento fonoaudiológico de las disfonías, en otras palabras, terapia de voz, es un proceso que involucra procedimientos de diversas naturalezas para así desarrollar una mejor comunicación oral, reduciendo el esfuerzo fonatorio y adecuando las cualidades vocales a las necesidades personales, sociales y profesionales del individuo. La atención de un paciente disfónico requiere de un equipo multidisciplinario que en la mayor parte de los casos incluye esencialmente un fonoaudiólogo y un otorrinolaringólogo. Existen varias filosofías de trabajo vocal con pacientes portadores de disfonía, no necesariamente excluyentes y muchas veces complementarias entre sí; ellas son: sintomatológica, psicológica, etiológica, fisiológica y ecléctica. El fundamento de una rehabilitación vocal exitosa es la precisión diagnóstica, sin embargo, una serie de características pueden ser utilizadas cuando ésta es inexacta, imprecisa o inexistente. La rehabilitación fonoaudiológica consiste, de forma simplificada, en emplear procedimientos integrados de orientación, psicodinámica y entrenamiento vocal. Por el carácter abstracto de la voz y por la falta de conocimiento vocal en general, la orientación al paciente es la base de cualquier trabajo de rehabilitación. El trabajo de psicodinámica vocal depende del impacto de la alteración vocal, su profundidad y su dimensión de acuerdo con las demandas del paciente respecto a sus necesidades profesionales. El entrenamiento vocal esta en búsqueda de nuevos ajustes para una producción mejor pudiéndose utilizar abordajes simples o combinados. Los innumerables abordajes de entrenamiento vocal expresan no solamente la dimensión multifacética del fenómeno de la voz sino que también la diversidad de caminos fisiológicos para que se obtenga un ajuste motor más adecuado. La naturaleza y la extensión de la rehabilitación deben ser racionalmente determinadas de acuerdo con la etiología de la disfonía, el comportamiento vocal del individuo y los datos semiológicos disponibles. La terapia fonoaudiológica debe ser elaborada individualmente aunque sus fundamentos estén basados en reglas genéricas de una producción vocal saludable con ajustes anatomofisiologicos eficientes y que produzcan una voz socialmente aceptable. El objetivo de la rehabilitación es obtener la mejor voz posible, lo que no siempre significa una voz sin desvíos.
