Dans les modifications du phenotype cellulaire survenant au cours de l'hypertension arteriell... more Dans les modifications du phenotype cellulaire survenant au cours de l'hypertension arterielle, celles interessant les cml des arterioles resistives sont au premier plan, en particulier au sein de la microcirculation renale, ce qui justifie que des travaux leur soient consacres. Tout d'abord, nous avons montre que l'expression de l'endopeptidase neutre, une ectoenzyme responsable du catabolisme des peptides natriuretiques et vasodilatateurs, est influencee par les facteurs de croissances egf et tgf-. En retour, la proliferation cellulaire induite par l'egf necessite l'activite de l'endopeptidase neutre, ce qui confere une nouvelle fonction pour cette enzyme vasculaire dans le rein. Nous avons etudie le role du monoxyde d'azote (no) sur la synthese de renine in vivo et nous avons alors observe que le no endogene exerce un effet permissif pour l'activation physiologique de ce phenomene. En l'absence de no endogene en effet, la synthese et la secretion de renine par les arterioles afferentes sont bloquees par l'activation des canaux calciques de type l sous le controle des recepteurs de l'endotheline de ce fait actives. Ainsi, l'equilibre des actions du no et de l'endotheline, outre la regulation du tonus arteriolaire, affecte la fonction endocrine des cmlv specialisees. En physiopathologie, l'inhibition chronique des no synthases chez le rat a demasque l'action fibrogene renale de l'endotheline. La synthese par les arterioles preglomerulaire de collagene de type i est activee de pair avec la synthese d'endotheline-1. La synthese vasculaire de collagene i comme la nephroangiosclerose ont pu etre prevenues par le blocage pharmacologique des recepteurs de l'endotheline. Ce resultat a ete obtenu sans modifier l'hypertension arterielle systolique ce qui suggere que ce critere d'efficacite clinique habituelle est insuffisant pour juger des effets d'un agent pharmacologique sur le phenotype vasculaire resistif renal. A contrario, cette etude ouvre des perspectives d'action therapeutiques antifibrosantes specifiques des maladies renales chroniques. La part du role de l'hemodynamique renale dans ce phenomene n'est cependant pas exclue comme le suggere la grande autonomie de cette circulation autoregulee vis a vis de la pression arterielle systemique. Nous avons enfin compare les voies cellulaires d'activation transcriptionnelle du gene de la chaine alpha 2 du collagene de type i (procol2(i)) respectivement par l'angiotensine ii (ang ii) et par le tgf-. Les resultats indiquent que l'activite biologique du tgf- est necessaire a l'action fibrogene aigue de l'angiotensine ii. Parmi les voies etudiees, celle des mapk/erk est egalement indispensable a la stimulation de procol2(i) par l'ang ii mais non a celle induite par le tgf- dans les aortes et le cortex renal de souris.
Le syndrome des apnees obstructives du sommeil (SAOS) est un desordre frequent associe a un risqu... more Le syndrome des apnees obstructives du sommeil (SAOS) est un desordre frequent associe a un risque accru de maladies cardio-vasculaires et d'accidents vasculaires cerebraux. Bien que le SAOS soit fortement associe au syndrome metabolique de l'obesite incluant l'insulino-resistance, l'hypertension arterielle et les dyslipidemies, il apparait comme un facteur de risque independant d'hypertension arterielle. La confrontation de la litterature de biologie fondamentale et des recentes etudes epidemiologiques et cliniques suggere, de plus, que le SAOS pourrait etre un facteur pathologique specifique aggravant l'obesite souvent associee en particulier par le biais d'une aggravation de la dysfonction endotheliale. En effet, le traitement des apnees par la ventilation nasale en pression continue positive (PCP) normalise au moins en partie les fonctions vasculaires. Ici sont passes en revue les mecanismes par lesquels le SAOS est susceptible d'alterer la fonction endotheliale et de determiner des consequences hemodynamiques et structurales vasculaires pathologiques. En effet, les SAOS se caracterisent par une activite elevee du systeme sympathique, une stimulation du systeme renine-angiotensine et une augmentation de la synthese d'endotheline. L'ensemble de ces mediateurs contribue a l'elevation des resistances arteriolaires, a l'ischemie tissulaire peripherique et au remodelage anormal du lit vasculaire. De facon concomitante, les effets ne sont pas contrebalances par le monoxyde d'azote, vasodilatateur d'importance majeure, dont la production endotheliale et les actions biologiques sont diminuees dans le contexte du SAOS. Enfin, le role de la leptine, hormone adipocytaire impliquee d'abord dans la regulation de l'appetit et du catabolisme energetique par une action hypothalamique, a recemment pris de l'importance en biologie vasculaire. Ce peptide agit sur des recepteurs propres a la surface des cellules endotheliales et musculaires lisses vasculaires. Les effets benefiques de cette hormone sur la fonction endotheliale et la relaxation du tonus vasculaire en situation normale semblent etre inverses dans le contexte du SAOS. De surcroit, meme si l'obesite par le biais de l'augmentation de la masse adipocytaire, est la premiere cause de l'augmentation de la concentration circulante de la leptine, une part de cette augmentation serait causee specifiquement par les apnees dans la mesure ou celle-ci regresse par la ventilation en PCP. Tout comme le SAOS est associe au syndrome de resistance a l'insuline, il semble egalement associe a un syndrome de resistance central mais aussi vasculaire a la leptine. En conclusion, l'evaluation de l'association au traitement de reference par PCP, de strategies agissant sur les mediateurs vasculaires et leurs recepteurs constitue une perspective a court terme attrayante qui pourrait permettre d'ameliorer le pronostic du SAOS.

Cardiac and renal dysfunction frequently go hand in hand in hospitalized patients, and epidemiolo... more Cardiac and renal dysfunction frequently go hand in hand in hospitalized patients, and epidemiological studies have suggested an inverse correlation between renal function and cardiovascular morbidity and mortality. This relationship exists regardless of what organ is first affected (1). It reflects upon a complex interplay between heart and kidneys, with dysfunction of one organ often impairing the function of the other. The causal association between chronic kidney disease (CKD) and cardiovascular risk was initially discussed by Bright in 1836. The notion of passive renal congestion arising from cardiac dysfunction was coined “rein cardiaque” by French pathologist Frederic Justin Collet in 1903, while “cardiorenal syndrome” (CRS) was introduced in the 1940s to describe the bidirectional interactions between heart and kidneys (2). Kidney injury will stress both the heart and the circulatory system and cardiac dysfunction can, reciprocally, inflict injury on the kidney. Determinants of CRS include hemodynamic parameters such as central venous pressure, extracellular fluid volume, cardiac output, arterial pressure, pulmonary hypertension, and edema. Reduced cardiac performance ultimately limits blood perfusion of all organs including the kidneys and thereby contributes to renal injury. Altered tissue perfusion with disproportionate effects on the kidney leads to overactivation of both the sympathetic nervous and renin–angiotensin systems (RAS) reported in CRS. Inadequate renal extracellular fluid handling may have deleterious effects on the heart with an ensuing increase in volume preload and afterload and thus myocardial oxygen demand and vasoconstriction—including of coronary vessels—and an increase in inflammation, reactive oxygen species, and fibrosis (3). Clinical and epidemiological studies support a causal relationship between CKD, cardiovascular risk, and heart failure (4, 5). In patients with congestive heart failure, a moderate elevation in serum creatinine levels (e.g., by 26.5 µmol/L [0.3 mg/dL]) will significantly increase cardiovascular mortality (6). Red blood cell sickling associated with sickle cell disease is affected by a complex interplay of... more Red blood cell sickling associated with sickle cell disease is affected by a complex interplay of vascular components and conditions which are not yet fully understood. A primary trigger of sickling is low oxygen partial pressure. Therefore, we have developed a microfluidic device to rapidly vary the oxygen partial pressure within flowing microdroplets. By using a carrier oil at a known oxygen partial pressure, a microdroplet was subjected to a deoxygenation at a rate that depends on both flow conditions and droplet size. The oxygen concentration in a single droplet was characterized with a ruthenium compound whose fluorescence lifetime is proportional to the oxygen concentration. The device was shown to provoke sickling of individual red blood cells which was readily detected by birefringence. The versatility of the same device was also demonstrated by initiating a chemical reaction in a microdroplet using a gaseous reactant.
Hydroxychloroquine (HCQ) is considered as efficient and safe to treat systemic lupus. We report a... more Hydroxychloroquine (HCQ) is considered as efficient and safe to treat systemic lupus. We report a case of fatal toxic cardiomyopathy, an underrecognized adverse effect of HCQ. A 72-year-old woman with systemic lupus erythematosus treated for 24 years by HCQ received a kidney allograft. One year later she developed a cardiomyopathy with conductive disorders. An endomyocardial biopsy showed severe non-specific myocardial fibrosis and hypertrophic vacuolated myocytes. Transmission electron microscopy showed curvilinear bodies and pseudomyelin figures consistent with HCQ-induced cardiomyopathy. At immunohistochemistry LC3b, p62 and beclin-1 accumulated in vacuolated cardiac myocytes suggesting impaired cell autophagy. When unexplained cardiac disease develops in patients receiving long-term HCQ treatment, toxic cardiomyopathy should be considered leading to a diagnostic endomyocardial biopsy.
