The aim of the study was to analyse the impact of different prognostic factors on the time until the development of contralateral fracture and to determine the incidence of contralateral hip fractures after femoral neck fractures. Patients aged 60 years and over with contralateral hip fracture between 01 Jan 2000 and 31 Dec 2008 were identified among those who suffered their femoral neck fracture in Hungary in 2000. Risk factors as age, sex, comorbidities, type of fracture and surgery, place of living and hospitals providing treatment for primary fracture were analysed by one way ANOVA focusing on the time until the development of contralateral hip fracture. 312 patients met the inclusion criteria. The incidence of contralateral hip fracture after femoral neck fracture ranged between 1.5% and 2.1%...

By the analysis of 67 retrospective studies and reviews the numerical effects of treatment initiation time on survival were assessed. The "time factor" has a leading role on cancer types with aggressive biological behaviour, like breast cancer in younger age, testicular cancers, or head and neck tumours. However, a few days and reasonable delay to the first oncology intervention has no negative impact in numerous cancer diseases. The course of the primary check-up could be modified by several medical and psychosocial factors, and many times the treatment of the most advanced cancers are privileged causing an inverse survival effect. The effectiveness of the cancer therapies is determined by the optimal treatment decision, however, further research is necessary for the determination of the exact role of the "time factor" in oncology. Data derived from the nationwide pharmaceutical database of the Hungarian National Health Insurance Fund Administration. We observed the turnover and price of the medicaments containing the active substance montelukast sodium from 2007 to 2015. Accordingly, our indicators were: consumer price, DCT (daily cost of therapy), co-payment, quasi co-payment, DOT (days of treatment). Due to the increasing DOT, the total amount of the public price paid by the patients increased until 2011, reaching the amount of 1 million USD; then, due to the generic competition and the blind bid methods, it decreased to 490 000 USD. The total amount of the public price of the brand-name Singulair moved to the generics during 3 years (2011-2014). The DCT of the originator Singulair 10 mg tablets decreased from 1.1 USD to 0.34 USD; the DCT of the generic product Montelukast TEVA decreased from 0.67 USD to 0.16 ...

Eighty psoriatic patients who underwent a 3-week-long inward balneotherapy-based rehabilitation were enrolled. Psoriasis Area and Severity Index (PASI) score and high sensitivity C-reactive protein (CRP) were determined on admission and before discharge. The mean PASI score and CRP level -determined on admission and before discharge-decreased significantly after the 3-week-long rehabilitation 7.15±7.3 vs. 2.62±3.05 (p<0.001) and 4.1±3.8 vs. 3.5±3.1 (p=0.026). A negative correlation was found between PASI delta and the number of spa therapies received (r=-0.228). After completing the 3-week-long spa therapy based rehabilitation, both PASI score and CRP levels showed improvement of psoriasis. The complex spa therapy used during the rehabilitation is an effective tool to reduce the symptoms of psoriasis and improve the patient's well-being.

The data derives from the financial database of the National Health Insurance Fund (NHIF) and covering the period 1998-2001. In this study the authors calculated the market share of the medical schools within the financing of the NHIF in the field of out- and in-patient care, renal dialysis, CT/MRI examinations, task financed services under special rules and fee for progressive (tertier level) care. The authors performed a detailed analysis concerning the Diagnosis Related Groups (DRG) of active inpatient care, where the market share of medical schools was calculated within the TOP-15 DRGs with highest and lowest point value, and within the TOP-15 most common and most infrequent DRGs. The market share of medical schools increased from 13.7% (1998) to 15.0% (2001). The increase was significant in the active and chronic in-patient care and in renal dialysis. The market share of medical schools from progressive fee decreased from 32.3% (1998) to 26.5% (2001). Within the active in-patient care the medical schools provide health care for 37.7% of the most difficult cases with highest DRG value and for 30.1% of the patients with rare diseases. The market share of the medical schools is much lower in cases with lowest DRG value (10.2%) and in most common cases (9.8%). The medical schools fulfilled their expected role in the progressive (tertier) care, answering the public and professional expectations.

Aim: To explore the health state of the Hungarian population by socio-demographic characteristics, and to compare it with the results from 2000. Method: As part of an international research project, a cross-sectional, online survey was carried out among the general population in 2010 using the EQ-5D-3L questionnaire. Results: In total, 2281 respondents (female: 62.3%) completed the questionnaire with an average age of 40.8 years. The EQ-5D score ranged from 0.902 (in age-group 18–24) to 0.795 (65+). The authors found significant association between the EQ-5D score and all the socio-demographic variables (gender, age, education, income) included in the regression model (F(4,1967) = 35.12, p = 0.000). The results did not differ significantly from the health survey in 2000, except for the youngest population group (age 18–24), wher...

Patients were identified from the financial database of the National Health Insurance Fund Administration and a questionnaire was used for further analysis. The costs of patients were analysed in three groups I) all patients, (II.) patients with further treatment because of complications, and (III.) patients cured by one primary treatment. Altogether 518 patients were included into the study: 413 (79.7%) with primary screw fixation and 48 (9.3%) with hip replacement. The average cost for all patients (I) for the 2 years follow up was 582.181 Hungarian Forint (HUF) with screw fixation and 545.300 HUF with hip replacement. The average cost per patients in the group with one primary surgical treatment (II) was (N = 337, 81.6%) 441.466 HUF with screw fixation and (N = 44, 91.6%) 561.027 HUF with hip replacement. The average cost per patients in the subgroup with further hospitalization because of complications (II) was (N = 76, 18.4%) 1.005.578 HUF with screw fixation and (N = 4, 8.4%) 775.640 HUF with hip replacement. The costs according to Garden classification (I-IV.) were as follows: patients without complications from 436.681 HUF to 659.160 HUF and patients with complications from 628.323 HUF to 1.192.564 HUF. The results suggest that patients (N = 76, 18.4%) having displaced fractures with screw fixation receiving further treatment should be treated - knowing the vitality of femoral head - with stable osteosynthesis or hip replacement in order to reduce further reoperations and finally, health insurance expenditures and patients&amp;amp;amp;amp;amp;amp;amp;amp;amp;#39; burden.
