AIM OF THE STUDY This study intends to research experiences of employees of ambulatory and statio... more AIM OF THE STUDY This study intends to research experiences of employees of ambulatory and stationary residential facilities for people with mental and multiple disabilities and family members concerning the health care of the target group. METHODS A qualitative research design was applied. As a data collection tool expert interviews were used. Proxy-interviews of employees of ambulatory and stationary residential facilities for people with mental and multiple disabilities and of family members were carried out. RESULTS In total, 21 interviews could be evaluated with the software program MAXQDA. In particular, the results show that interviewees experienced or perceived a lack of individual time, prescribed measurements, conducted interventions and measurements, etc in the health care field of independent physicians. CONCLUSION Systematic, structural, qualification-related and financial limitations impede a high-quality care for the target group. Further studies are necessary to receive differentiated knowledge about the health situation of the target group and to optimize the health care provision.

However, in the heightened stress of emergency situations, interaction between nurses and relatives can raise ethical challenges.Research objectivesThe present analysis aimed at elaborating a typology of nurses’ experience of ethical support and challenges in their interaction with relatives in emergency situations.Research designThirty-three semi-structured interviews and six focus groups were conducted with nurses from different nursing homes in Germany. Data were analysed according to Mayring’s method of qualitative content analysis.Participants and research contextParticipants were licensed nurses working in nursing homes.Ethical considerationsEthical approval was granted by Ostfalia University of Applied Sciences (02.07.2020) and the Ethics Committee of Hannover Medical School (Nr. 8866_BO_K_2020; 27.01.2020). Interviewees were anonymised and focus group were pseudonymised during transcription. All participants provided written consent.Findings/resultsIn emergency situations, relatives can represent important sources of support for nurses. However, they may also give rise to different challenges, relating to four ethical conflicts: (1) the challenge of meeting the information needs of relatives while providing appropriate care to all residents; (2) the challenge of managing relatives’ demands for hospitalisation when hospitalisation is not deemed necessary by nurses; (3) the challenge of managing relatives’ demands for lifesaving treatment when such treatment contradicts the will of the resident; and (4) the challenge of attempting to initiate hospitalisation when relatives oppose this course of action. Several external factors make these conflicts especially challenging for nurses: fear of legal consequences, a low staffing ratio, and a lack of qualified nursing staff.ConclusionsConflict between nurses and relatives typically revolves around hospitalisation and the initiation of lifesaving treatment. Whether nurses perceive interaction with relatives as supportive or conflictual essentially depends on the quality of the relationship, which may be negatively influenced by a number of external factors.
Das Thema der Gesundheitsforderung und Pravention in der stationaren Langzeitpflege erfahrt durch... more Das Thema der Gesundheitsforderung und Pravention in der stationaren Langzeitpflege erfahrt durch das Praventionsgesetz, respektive durch § 5 SGB XI, neue Aktualitat in den diversen Debatten. Trotz dieser neuen gesetzlichen Moglichkeiten und Forderungen, sind Begrenzungen und Herausforderungen zu berucksichtigen. In den stationaren Pflegeeinrichtungen leben zunehmend Menschen mit hoheren Pflegebedarfen und Pflegegraden in einer immer kurzeren Wohnzeit. Des Weiteren sind weder differenzierte Ziele noch theoretisch wie wissenschaftlich fundierte praventive sowie gesundheitsforderliche Konzepte fur die stationare Langzeitpflege bekannt. Praventive Potenziale pflegerischer Berufsgruppen sowie die Erforderlichkeit eines betrieblichen Gesundheitsmanagements und einer betrieblichen Gesundheitsforderung bleiben in den Diskussionen weitgehend unbeachtet. In diesem Beitrag werden sowohl gerontologische Theorien im Kontext von Gesundheitsforderung und Pravention stationarer Langzeitpflege sowie Chancen und Herausforderungen diskutiert.

Für Pflegeheimbewohner soll die Basis dieser Abwägung auf Grundlage der im Deutschen Reanimationsregister verfügbaren Endpunkte verbreitert werden. Methodik Retrospektive Auswertung prähospitaler RV der Jahre 2012 bis 2019. Ergebnisse Eingeschlossen wurden 2510 Datensätze. Das Durchschnittsalter betrug 83,7 (±7,5) Jahre, 1497 (59,6%) waren weiblich. Es erfolgten 98 (3,9%) Lebendentlassungen; davon 61 (2,4%) mit neurologisch gutem Outcome. Im Krankenhaus verstarben 791 (31,5%); 561 (22,4%) innerhalb 24 Stunden. Am Einsatzort verstarben 1621 (64,6%). Diese und weitere Endpunkte wurden stratifiziert als Spektrum dargestellt und Subgruppen nach Alter, Pre Emergency Status und Einsatzsituationen gebildet.
