Determining which is the most effective solution (heparin flush compared to 0.9% saline flush) fo... more Determining which is the most effective solution (heparin flush compared to 0.9% saline flush) for reducing the risk of occlusions in central venous catheters (CVC) in adults. The systematic review followed the principles proposed by the Cochrane Handbook; critical analysis, extraction and synthesis of data were performed by two independent researchers; statistical analysis was performed using the RevMan program 5.2.8. Eight randomized controlled trials and one cohort study were included and the results of the meta-analysis showed no difference (RR=0.68, 95% CI=0.41-1.10; p=0.12). Analysis by subgroups showed that there was no difference in fully deployed CVC (RR=1.09, CI 95%=0.53-2.22; p=0.82); Multi-Lumen CVC showed beneficial effects in the heparin group (RR=0.53, CI 95%=0.29-0.95; p=0.03); in Double-Lumen CVC for hemodialysis (RR=1.18, CI 95%=0.08-17.82; p=0.90) and Peripherally inserted CVC (RR=0.14, CI 95%=0.01-2.60; p=0.19) also showed no difference. Saline solution is sufficient for maintaining patency of the central venous catheter, preventing the risks associated with heparin administration. Determinar qual é a solução (flush heparina comparado com o flushde soro fisiológico 0.9%) mais eficaz na redução do risco de oclusões de cateteres venosos centrais (CVC) em adultos. A revisão sistemática seguiu os princípios propostos pelo Cochrane Handbook; a análise crítica, a extração e a síntese dos dados foram realizadas por dois investigadores, isoladamente; e a análise estatística efetuada com recurso ao programa RevMan 5.2.8. Foram incluídos oito estudos randomizados controlados e um estudo de coorte e os resultados da meta-análise mostram não existir diferenças (RR=0.68, IC 95%=0.41-1.10; p=0.12). A análise por subgrupos mostra que nos CVC totalmente implantados não se verificaram diferenças (RR=1.09, IC 95%=0.53-2.22; p=0.82); nos CVC com vários lúmens existiu um efeito benéfico no grupo da heparina (RR=0.53, IC 95%=0.29-0.95; p=0.03); nos CVC de duplo lúmen para hemodiálise (RR=1.18, IC 95%=0.08-17.82; p=0.90) e nos CVC de inserção periférica (RR=0.14, IC 95%=0.01-2.60; p=0.19) também não se verificaram diferenças. O soro fisiológico é suficiente para manter a permeabilidade dos cateteres venosos centrais, prevenindo os riscos associados à administração da heparina.
Co-stimulatory molecules are essential in the orchestration of immune response and polymorphisms ... more Co-stimulatory molecules are essential in the orchestration of immune response and polymorphisms in their genes are associated with various diseases. However, in the case of variable allele frequencies among continental populations, this variation can lead to biases in genetic studies conducted in admixed populations such as those from Brazil. The aim of this study was to evaluate the influence of genomic ancestry on distributions of co-stimulatory genes polymorphisms in an admixed Brazilian population. A total of 273 individuals from the north of Brazil participated in this study. Nine single nucleotide polymorphisms in 7 genes (CD28, CTLA4, ICOS, CD86, CD40, CD40L and BLYS) were determined by polymerase chain reaction-restriction fragment length polymorphism. We also investigated 48 insertion/deletion ancestry markers to characterize individual African, European and Amerindian ancestry proportions in the samples. The analysis showed that the main contribution was European (43.9%) but also a significant contribution of African (31.6%) and Amerindian (24.5%) ancestry. ICOS, CD40L and CD86 polymorphisms were associated with genomic ancestry. However there were no significant differences in the proportions of ancestry for the other SNPs and haplotypes studied. Our findings reinforce the need to apply AIMs in genetic association studies involving these polymorphisms in the Brazilian population.

ABSTRACT Background: There is some debate in clinical circles about the potential advantages and ... more ABSTRACT Background: There is some debate in clinical circles about the potential advantages and disadvantages of cleansing solutions in wounds. Objective: To identify and synthesize the best available evidence on the effectiveness of cleansing solutions for wound treatment in clinical practice. Methods: Inclusion criteria "Participants": patients aged 18 years or more in any setting, excluding malnourished patients and with chronic and acute wounds, excluding obstetric wounds. Intervention: any cleansing or antiseptic solution or chemicals. Outcome: infection and healing rate. Results: Three original articles were included and the interventions were: Tap water versus Sterile saline and Povidone-iodine-soaked gauze versus Saline-soaked gauze. A positive effect in the prevention of infection rates was observed in the tap water group (OR = 0.79; 95% Cl: 0.36, 1.72). For acute wounds, the odds ratio of developing an infection when cleansing with tap water compared with saline was 0.98 (95% CI: 0.43, 2.25). Discussion: There were no statistically significant differences between the healing and infection rates in the wounds cleansed with tap water or normal saline. Therefore, tap water can be considered a safe and cost-effective alternative. There may be a trend towards a lower wound infection rate when povidone-iodine is used in surgical wounds, but this is not significant for varicose vein surgery. Conclusion: There is some evidence that tap water reduces infection when compared to saline. There may be a trend towards a lower wound infection rate when povidone-iodine is used in surgical wounds. This review included few cleansing solutions, thus the evidence is not strong enough to produce a best practice. However, these findings have extreme relevance for clinical practice, and they should be put into practice and considered by health professionals.
