Nous avons etudie la cinetique en plongee d'un nouveau marqueur hormonal cardiaque, le NT-pro-BNP. 10 sujets ont effectue une plongee de 1 heure en eau de mer a 10 metres de profondeur avec 5 prelevements sanguins a T0, T30 minutes, T60, T240 et T24 heures pour la mesure du peptide natriuretique. Les resultats obtenus revelent une augmentation significative des taux plasmatiques a T60min (p<0,046) et T240min (p<0,016). Cette etude montre que la plongee agit sur le myocarde comme une contrainte mecanique. L'augmentation des concentrations du NT-pro-BNP au-dela de la plongee suggere l'existence d'une activite myocardique endocrine persistante malgre l'arret des variations pressionnelles exercees sur le coeur.

The main stimulus for N-BNP synthesis and secretion is cardiac wall stress, which is recognized as a common denominator of many cardiac diseases. Diving is associated with environmental factors leading to variations in thoracic blood volume and hemodynamic changes. The purpose of the present study was to examine the changes in the concentration of N-BNP in healthy men during and after scuba diving. There were 10 healthy military divers (mean age 33 yr) who performed a dive in the sea for 1 h at 10 m depth. Venous blood samples were taken at timed intervals to allow evaluation of plasma levels of N-BNP at different steps, namely at To (before immersion), at T30 min (during the dive, after a short surfacing), at T60 min (right after surfacing), at T300 min (post-dive), and finally at T24 h. Au cours du mois d'octobre 1996, une partie du personnel d'un navire ayant fait escale en... more Au cours du mois d'octobre 1996, une partie du personnel d'un navire ayant fait escale en Afrique de l'Ouest a ete placee en observation medicale apres son arrivee a Brest pour malaise general, troubles digestifs non specifiques, manifestations cutanees et signes neurologiques. Ces signes cliniques, peu specifiques, se manifesterent quelques heures apres la consommation d'espadon fume achete sur la cote africaine. La relation entre ces deux evenements et l'intervalle de temps reduit qui les separe permirent aux medecins de suspecter une toxi-infection alimentaire collective probablement d'origine histaminique [1].

Non‐steroidal anti‐inflammatory drugs (NSAIDs) are well known for their most frequent side effect... more Non‐steroidal anti‐inflammatory drugs (NSAIDs) are well known for their most frequent side effects (digestive, renal and metabolic disorders) but are lesser known for other effects, such as coagulation disturbances. In this issue, we report the case of a 58‐year‐old woman who ingested 26 g of naproxen in a suicidal attempt and developed cardiovascular shock, hypocoagulability and thrombopenia. Her outcome was positive (extubation 3 days after admission [D3], correction of haemostatic disruptions on D5 and of thrombopenia on D6). Naproxen plasma concentration was at a toxic concentration of 1320 mg/L at 6 hours after drug ingestion. Only few cases of hypocoagulopathy are reported with the NSAIDs, and this is the first case that can be attributed to naproxen. A possible explanation of this phenomenon following naproxen ingestion is an inhibition of thromboxane A2, usually attributed to NSAIDs, combined with an inhibition of activation of downstream the cascade.

The isolation of Vibrio cholerae serogroup O1 (Ogawa serotype) explains this typical clinical presentation of cholera, seen in 10% of cholera cases only. The patient had co-infection with Vibrio cholerae and Campylobacter coli. Co-infections appear to be frequent in endemic areas. The purpose of this case report is to recall the relevance of Vibrio isolation when the clinical context is evocative (diarrhea on travel return, raw sea food consumption).
