There is a large spectrum of tumors presenting as nodular lesions that may affect the subungual space. We report the case of a 62-year-old woman presenting with a rapidly growing nodular lesion under the nail of the first left toe. Non-invasive examinations using dermoscopy, ultrasonography and elastography were performed for the preoperative assessment of the lesion. The biopsy of the lesion revealed superficial acral fibromyxoma, a benign tumor with predisposition for acral sites. The patient underwent radical surgery with wide resection margins. This is the first case report of a superficial acral fibromyxoma affecting the subungual region characterized by dermoscopic, ultrasonographic and elastographic features. We also performed a short review of the literature.

Superficial Acral Fibromyxoma involving the nail's apparatus. Case report and literature reviewSuperficial Acral Fibromyxoma is a rare tumor of soft tissues. It is a relatively new entity described in 2001 by Fetsch et al. It probably represents a fibrohistiocytic tumor with less than 170 described cases. We bring a new case of SAF on the 5th toe of the right foot, in a 43-year-old woman. After surgical excision with safety margins which included the nail apparatus, it has not recurred (22 months of follow up). We carried out a review of the location of all SAF published up to the present day.

Superficial Acral Fibromyxoma (SAF): A Case ReportSuperficial Acral Fibromyxoma (SAF) is a benign soft tissue tumor characterized by the presence of stellate and spindle-shaped cells arranged in a storiform and fascicular pattern. This tumor is relatively rare, benign and slow-growing. If often involves peri-and subungual regions of fingers and toes in middle-aged adults, with a male predominance. This acral fibrous tumor is poorly known and its histology may suggest myxoid dermatofibrosarcoma, which carries a completely different prognosis and treatment. Thus, immunohistochemistry is extremely important to differentiate SAFM from other acral fibrous tumors. We present a case of SAFM that involved the nail matrix with canalicular dystrophy of the nail plate. This finding has not been described in the literature as of the date of publication of this case report. Hence, we believe that it is important to note this presentation so that this disease can be considered in the differential diagnosis of diseases that produce this type of nail dystrophy.

Superficial acral fibromyxoma: a case of missed diagnosisSuperficial acral fibromyxoma (SAFM) is a rare, benign, slow-growing fibroblastic tumour of the soft tissue that is part of the group of myxoid soft-tissue neoplasms. It is a rare entity and usually occurs in the acral regions. We report the case of a 64-year-old man who presented to the emergency room for a lesion expected to have occurred as a result of an ingrown toenail. Because this patient had a history of repeated recurrences despite multiple surgical wedge excisions, we performed a complete surgical excision, and the pathological analysis confirmed the suspected diagnosis of SAFM. There was no recurrence at the 6-month follow-up. This case highlights the fact that this tumour is still misunderstood and underrecognized by surgeons and this often leads to delayed diagnosis. Although it is a rare entity, clinicians should be aware of this tumour in cases of recurring ingrown toenails.

Superficial acral fibromyxoma (SAFM) is a rare soft tissue tumor most often located in the ungual region of the fingers and toes. This tumor was first described in 2001, and since then very few cases have been reported. We present the case of a 35-year-old male with a SAFM located in the toe, with involvement of the nail and erosion of the distal phalanx. The lesion was surgically removed, and the histopathological study confirmed the diagnosis of SAFM. The differential diagnosis must be established with other myxoid tumors and with those lesions showing a predilection for the distal portions of the limbs. After 2 years, the patient remains disease free, with no disability of any kind.

Galt noch vor 20 Jahren der Grundsatz, dass Bodendenkmäler im Wald besonders gut geschützt sind, so ist das Kulturerbe-gleichermaßen wie der Wald-heute insbesondere durch die Folgen des Klimawandels bedroht.
