Facing the challenge of polypharmacy when prescribing for older people with cardiovascular disease. A review by the European Society of Cardiology Working Group on Cardiovascular PharmacotherapyPopulation ageing has resulted in an increasing number of older people living with chronic diseases (multimorbidity) requiring five or more medications daily (polypharmacy). Ageing produces important changes in the cardiovascular system and represents the most potent single cardiovascular risk factor. Cardiovascular diseases (CVD) constitute the greatest burden for older people, their caregivers, and healthcare systems. Cardiovascular pharmacotherapy in older people is complex because age-related changes in body composition, organ function, homeostatic mechanisms and comorbidities modify the pharmacokinetic and pharmacodynamic properties of many commonly used cardiovascular and non-cardiovascular drugs. Additionally, polypharmacy increases the risk of adverse drug reactions and drug-interactions, which in turn can lead to increased morbi-mortality and healthcare costs. Correlation of rise in high sensitive troponins following positive exercise testing with underling coronary artery disease2015 •
Background: Cardiac troponins are sensitive markers of acute myocyte injury.Recent studies with high sensitive cardiac troponins (hs-TnT) assays have shown prognostic value even in chronic stable angina. It seems plausible that subjects of hsTnT that is greater than the release seen in subjects with positive stress test but without ischemia. Purpose: The objectives of the study were to correlate the rise of circulating hsTnT post exercise treadmill testing with underlying coronary artery disease (CAD) presence & severity. Methods: 99 subjects of suspected stable angina with positive exercise TMT test were enrolled. Blood samples was obtained before the TMT and again 20 hr after peak exercise and samples were analyzed for troponins using a high sensitive assay.Coronary anatomy was determined by invasive coronary angiography. Results: There was significant increase in mean hsTnT levels following a positive exercise TMT (0.009±0.012 ng/ml, p=0.0001).The rise in hsTnT levels were significant only in patients obstructive CAD versus those with normal coronaries (p=0.0001). The elevation in hsTnT in subjects with single vessel, double vessel, triple vessel & left main-triple vessel CAD were 0.008±0.007 ng/ ml, 0.010±0.012 ng/ml, 0.020±0.015ng/ml, 0.22±0.015 ng/ml respectively.Males had more hsTnT rise (p=0.003). Smoking, diabetes and hypertension did not affect the troponin rise. Conclusion: In patients of stable angina, the rise in hsTnT from baseline after a positive stress test predicts the presence underlying of coronary artery disease and that the degree of rise correlates with the severity of disease conversely. Absence of elevation in hsTnT may indicate a normal anatomy despite a positive stress test.

Update on Management of Cardiovascular Diseases in Women2022 •
Cardiovascular diseases (CVD) have a lower prevalence in women than men; although, a higher mortality rate and a poorer prognosis are more common in women. However, there is a misperception of CVD female risk since women have commonly been considered more protected so that the real threat is vastly underestimated. Consequently, female patients are more likely to be treated less aggressively, and a lower rate of diagnostic and interventional procedures is performed in women than in men. In addition, there are substantial sex differences in CVD, so different strategies are needed. This review aims to evaluate the main gender-specific approaches in CVD.

Risk factors associated with bleeding after multi antithrombotic therapy during implantation of cardiac implantable electronic devices2016 •
Previous studies showed that continuous anticoagulation or single antiplatelet therapy during implantations of cardiac implantable electronic devices (CIED) was relatively safe. However, the safety of continuous multi antithrombotic therapy (AT) in patients undergoing CIED interventions has not been clearly defined. We sought to evaluate the safety of this therapy during CIED implantations. A total of 300 consecutive patients (mean 69 years old, 171 males) with CIED implantations were enrolled in this study. The patients were divided into 6 groups [No-AT, oral anticoagulant therapy (OAT), single antiplatelet therapy (SAPT), OAT and SAPT, dual antiplatelet therapy (DAPT), triple AT (TAT)], and the perioperative complications were evaluated.
