Factors Associated with Graft Rejection in the Cornea Preservation Time Study2018 •
To identify factors related to graft rejection following Descemet stripping automated endothelial keratoplasty (DSAEK) in the Cornea Preservation Time Study (CPTS). Cohort study within a multicenter randomized clinical trial. 1330 eyes of 1090 subjects undergoing DSAEK were randomized to receive a donor cornea with preservation time (PT) of 0-7 days (N=675) or 8-14 days (N=655) and followed for three years. Central endothelial cell density (ECD) was determined by a central image analysis reading center. Multivariable Cox models adjusted for PT, recipient diagnosis, and surgeon effect were used to identify factors associated with rejection. Cumulative probability of definite graft rejection was 3.6% (99% CI 2.5% to 5.3%). Younger recipient age was associated with graft rejection [p<0.001; HR: 0.53 (0.33, 0.83) per decade]. Abstract Backgrounds To evaluate the efficacy of 2% topical ciclosporin A in treating and preventing graft rejection episodes after penetrating keratoplasty (PKP) in patients with a history of graft rejection episodes. Methods In this prospective, randomised, double-blind clinical trial, a group of PKP patients were randomly given 2% topical ciclosporin A (group 1) or a placebo (group 2) in addition to a corticosteroid regimen upon an episode of subepithelial or endothelial graft rejection.
The high Andes of South America were among the last environments that Homo sapiens colonized during its Pleis-tocene dispersion out of Africa. The peopling of this high-elevation environment was constrained by atmospheric hypoxia, cold stress, and resource availability. Here we report archaeological and geoarchaeological analyses from Cueva Bautista, a dry rock shelter, located at 3933 m above sea level in southwestern Bolivia. We focus on a well-preserved occupation surface containing hearths and high-quality stone tools AMS dated to 12,700– 12,100 cal BP. Geoarchaeological resolution of the site supports its stratigraphic integrity and archaeological analyses indicate that the early human occupation was formed as a temporary camp by mobile foragers relying on a curated technological strategy. Regional paleoenvironmental reconstructions suggest that Cueva Bautista's occupation was synchronous with humid conditions and its abandonment with increased aridity. Our findings suggest that mobile hunter-gatherers explored – albeit not colonized – the high Andes during the late Pleistocene and provides further support that a combination of biological, behavioral, and environmental constraints affected human adaptation to this extreme environment.
