Background Laparoscopic sleeve gastrectomy (LSG) is considered a standard bariatric procedure, which is easy to perform. However, there are pitfalls to be aware of, because complications from the operation, on the other hand, pose great challenges. Methods We describe our innovative three-port-technique of LSG in a step-by-step instruction. To investigate the safety and efficacy, a retrospective evaluation and analysis was performed on consecutive 116 patients. For this purpose, the operation time, complication rates and weight loss were recorded. Results Mean BMI was 35.47 ± 4.53 kg Mean procedure duration was 90.13 ± 5.68 mins. Mean weight loss achieved was 11.57 ± 5.09 kg at first month; 19.75 ± 6.57 kg at third month; 26.85 ± 7.60 kg at sixth month, and 28.86 ± 11.23 kg at one year. According to the Clavien-Dindo classification, no seriously complications were observed after 1, 3, 6 12 months follow-up. Conclusion LSG in this innovative three-port technique can be performed safel...

Technical Details of Laparoscopic Sleeve Gastrectomy Leading to Lowered Leak Rate: Discussion of 1070 Consecutive Cases2017 •
Introduction. Laparoscopic sleeve gastrectomy is a widely utilized and effective surgical procedure for dramatic weight loss in obese patients. Leak at the sleeve staple line is the most serious complication of this procedure, occurring in 1–3% of cases. Techniques to minimize the risk of sleeve gastrectomy leaks have been published although no universally agreed upon set of techniques exists. This report describes a single-surgeon experience with an approach to sleeve leak prevention resulting in a progressive decrease in leak rate over 5 years. Methods. 1070 consecutive sleeve gastrectomy cases between 2012 and 2016 were reviewed retrospectively. Patient characteristics, sleeve leaks, and percent body weight loss at 6 months were reported for each year. Conceptual and technical changes aimed towards leak reduction are presented. Results. With the implementation of the described techniques of the sleeve gastrectomy, the rate of sleeve leaks fell from 4% in 2012 to 0% in 2015 and 20...

Is laparoscopic sleeve gastrectomy a lower risk bariatric procedure compared with laparoscopic Roux-en-Y gastric bypass? A meta-analysis2014 •
Laparoscopic Roux-en-Y gastric bypass (LRYGB) is the current "gold standard" bariatric procedure in the United States. Laparoscopic sleeve gastrectomy (LSG) has recently become a commonly performed procedure for many reasons, including patients' perception that LSG has less complexity and invasiveness, and lower risk. Our objective was to review the literature and compare the leak rates, morbidity, and mortality for LRYGB versus LSG. Publications from 2002 to 2012 with n greater than or equal to 25 and postoperative leak rate reported were included. Statistical analysis included chi-square according to patient number. Twenty-eight (10,906 patients) LRYGB and 33 (4,816 patients) LSG articles were evaluated. Leak rates after LRYGB versus LSG were 1.9% (n = 206) versus 2.3% (n = 110), respectively (P = .077). Mortality rates were .4% (27/7,117) for LRYGB and .2% (7/3,594) for LSG (P = .110). Timing from surgery to leak ranged from 1 to 12 days for LRYGB versus 1 to 35 day...
