A infecção parasitária pelo Ascaris lumbricoides representa um acometimento comum no Brasil e em outras regionalidades subdesenvolvidas. A localização do verme em vesícula biliar é rara e quando há presença de quadro clínico importante, essa hipótese deve ser levantada como diagnóstico diferencial frente a outras doenças da árvore biliar. Relatamos o caso de paciente de 65 anos admitido em emergência devido à pancreatite por etiologia de áscaris em vesícula biliar, o qual, posteriormente, evoluiu para quadro alarmante de disfunção de múltiplos órgãos.

Ascariasis is the most common helminthic infection. It is most common in children of tropical and developing countries where the transmission is by contamination of soil by human feces or use of untreated feces as fertilizer. Transmission in most endemic areas is via person to person contact. We hereby present a case report of acute pancreatitis due to ascariasis. Twenty-five-year-old male patient presented to us with chief complaint of acute epigastric pain radiating to back and associated with vomiting. Initial lab investigations revealed increased serum amylase and lipase. Probable diagnosis of acute pancreatitis was made. CT scan was done and report revealed bulky pancreas, and significant peri-pancreatic fat stranding. He was managed symptomatically with intravenous fluids, analgesics, anti-emetics and enteral nutrition. Ascaris lumbricoides-induced acute pancreatitis: diagnosis during EUS for a suspected small pancreatic tumor2009 •
Ascaris lumbricoides is the second most common intestinal parasite world-wide and, although the infection can be asymptomatic, in some cases it can present with complications, such as acute pancreatitis. We describe the case of a 37-year-old man, with a history of travelling in Eastern countries who presented with Ascaris lumbricoides-induced acute pancreatitis mimicking a small pancreatic cancer, diagnosed during an upper EUS. The endoscopy revealeda roundworm floating in the duodenum; its endoultrasonographic appearance showed a diffuse inhomogeneous pattern, with hypoechoic echotexture, such as in acute pancreatitis. Microbiological examination of the worm revealed a 20 cm long Ascaris lumbricoides. In non endemic countries, acute pancreatitis induced by Ascaris lumbricoides is an unusual diagnosis, and should be suspected especially in patients with history of traveling in endemic areas.
Ascariasis is caused by the nematode, Ascaris lumbricoides, commonly found in the tropics. These parasites are transmitted via the faecal oral route. Their eggs hatch in the small intestines and the larvae migrate through the gut wall into the bloodstream and to the alveoli. They subsequently move up the respiratory tract to the trachea and are swallowed. The larvae mature in the small intestines, deriving nutrients from ingested food. Most infections are asymptomatic. Common manifestations include malnutrition or Loeffler’s pneumonia (fever, cough and eosinophilia due to a heavy larvae burden). There are, however, reports of ascariasis in the biliary and pancreatic ducts that cause complications.(1-6).

Ascariasis is one of the most common helminthic diseases in humans. Migration of "Ascaris lumbricoides" into the biliary tree may cause acute cholangitis, acute cholecystitis, biliary colic, acute pancreatitis or hepatic abscess. We report a case of acute pancreatitis caused by a live worm that obstructed the common bile duct. A 60-year-old housewife presented at our hospital, half an year after traveling to Mainland China and Vietnam. She complained of severe epigastric pain with vomiting for one day. Abnormally high serum amylase and lipase were found. Abdominal ultrasonography showed two small, round, echogenic lesions inside the dilated common bile duct in transverse section. Common bile duct stone with cholangitis and acute pancreatitis were suspected initially. “Unpacking the courts”: prevenzione e reazione agli attacchi all’indipendenza dei giudici di Simone Pitto2024 •
Il presente scritto muove dalle riflessioni su indipendenza e imparzialità delle corti svolte dai relatori del Convegno “Giudice e stato di diritto”, organizzato dalla Scuola Superiore della Magistratura il 20.10.2023 presso l’Accademia Nazionale dei Lincei, alla presenza del Presidente della Repubblica, i cui atti sono stati recentemente raccolti nel volume “Il giudice e lo stato di diritto. Indipendenza della magistratura e interpretazione della legge nel dialogo tra le Corti”, Milano Giuffré, 2024[1]. I lavori del convegno si sono focalizzati su un tema di particolare attualità: il significato odierno dell'indipendenza delle corti a livello nazionale ed europeo e il suo valore per lo stato di diritto[2]. In una società libera e democratica, l’indipendenza della magistratura rappresenta per i cittadini la prima garanzia di una decisione giudiziaria fondata esclusivamente sulla legge[3] e dell’uguaglianza effettiva di tutti d’innanzi alla stessa[4]. Autonomia e l’indipendenza della magistratura, inoltre, costituiscono un baluardo a protezione del principio della separazione dei poteri. Sono infatti naturalmente orientate ad evitare che le decisioni del giudice risultino condizionate da interessi politici, personali o logiche di carattere maggioritario estranee a quelle del solo diritto[5]. Tali garanzie rappresentano l’eredità del costituzionalismo liberaldemocratico inteso come processo di progressiva limitazione del potere[6] e di affermazione di un nucleo di diritti inviolabili dell’individuo[7], i quali trovano nella giurisdizione la sede privilegiata per la loro protezione di fronte ad eventuali limitazioni e lesioni, anche da parte dei pubblici poteri[8].
