This is a horror movie made during the Pre-Code Era, meaning that while there are no blood and guts, there are a lot of frightening moments that sometimes rely on the unseen. Not to mention even though the movie is INCREDIBLY tame by today's standards, it was edited heavily in America for many years, and was banned in many countries.

There are several scenes where men are screaming and obviously in great pain. This would be frightening not for what it shows but the pain imagined.
The experiments' appearances are modeled after victims of World War I. They might be frightening to younger children.
The final scenes will be pretty terrifying to young children, even by modern standards.
