When Mike and John are eating breakfast in the beginning of the movie, Mike's toast switches sides on his plate between shots.

John Q.'s shirt after the stabbing, as he's letting the pregnant woman and Latino mom go - the stab hole changes between shots, along with the blood stain.

The amount of, and positioning of, dirt on Mike's face in the moments following his seizure.

During the closing scene in which John is being sentenced in court he is found guilty for the "kidnapping and false imprisonment of [each hostage]." In reality this would not have been a single charge, rather a series of individual charges with a guilty or not guilty verdict in the case of each hostage taken. Moreover, "kidnapping" would not apply to most if any of the cases as such a charge necessitates asportation (the moving of a person or property illegally). Given that they never left the ER, false imprisonment would likely be the only charge.

Michael talks to John Q while intubated. It is impossible for a person to talk in this state.

When the cardiac surgeon agrees to do the surgery he says "what are they going to do... disbar me...?" But doctors are not members of the Bar, this is a legal profession term not a medical profession term.

There are numerous medical and procedural errors in the hospital.

When the gun shot wounded victim comes into the ER, John forces cardiac doctor to treat the patient, implying that without medical insurance, the doctor wouldn't treat the GSW and that John saved his life. However, the law says that all trauma ER doctors must treat their patients and save their lives, regardless of insurance. So what John does is moot.

In the opening scene, "beautiful woman" sticks her head out of her car window, and her hair does not move.

When the woman's car gets demolished by the oncoming semi truck at the beginning of the movie one can clearly see that there is no one in the front seat of the car.

When Dr. Turner passes through the doors into the glass corridor where John takes him hostage, you can see the feet of crew members behind the door below the white backdrop.

The medical staff should be wearing medical protective glasses when they're performing the autopsy on the BMW Driver, one medical staff member confirms this goof when he squints when blood sprays on his mask as he drills her chest.

Denise wouldn't have been allowed to lie next to Mike in his hospital bed, this is against hospital regulations, furthermore she could easily dislodge his IV needle and his respiratory tube.

Denise should've been charged as an accessory for not cooperating with Grimes to resolve the hostage situation.
