Kersey's daughter is released from the hospital and within a week her trachea has seemingly healed up. She isn't suffering from any muscle atrophy and/or psychological problems as a result of this heinous crime. She's very well adjusted.

At one point during the first gunfight, Kersey hides behind a car with his face toward the window where Natasha is standing and recording him. He appears to be looking in the direction of her window. She should have recorded a good image of his face in the video.
The detectives recover Kersey's college ring. They visit him in the office and show him the ring. First, this ring would have been inventoried on scene. Second, even if they did bring him the ring, why didn't Kersey ask for it back? In fact, he doesn't even ask about the process of having the ring returned to him.
Paul moves the three cans of brake fluid from the floor next to Joe's head and places them on his body. They briefly reappear next to his head again before disappearing.
The same two detectives seemingly investigate every single shooting and/or homicide in the entire city. In reality, Chicago Police Detectives are assigned geographically, on specific shifts, and respond to specific crimes.
Kersey appears to be using Propofol as a local anesthetic, and refers to it "wearing off". Besides the obvious question of why issue pain relief to someone you are going to torture, propofol renders the patient unconscious for a medical procedure and "wearing off" means the patient is regaining consciousness. They got the color, but not the pharmacological effects, correct.
Murder case tags on bulletin note type of weapon. Some list knife, those with firearms list caliber." .45 caliber" is listed in several cases as "45mm." Caliber is diameter, and .45 is .45 of an inch, not 45 mm. "45mm" would be a bullet almost two inches wide.

When Bruce Willis is in the nightclub and looks at his cell phone, the messages are reversed. The owner of the phone's messages should be on the right and the sender's on the left, not the other way around.
So Kersey's daughter suffers blunt force trauma on scene. Kersey's wife is shot twice and seemingly unresponsive. How does Kersey know that there were three offenders and that one of them has a burn on his face? The large wrench that Dr. Kersey hits the mechanic with is a pipe wrench. Not only is it the wrong tool, it's far too large for any kind of auto work. There is no reason why it would be on the car that was being worked on or even in the mechanic's shop to begin with as there are no pipes in a building like that which would warrant a wrench that large. Unless, of course, you are trying to remove a tie-rod end on any vehicle with a tie-rod.
Dr. Kersey works at a hospital in Chicago and the Kersey home is in Evanston, there are hospitals much closer to the Kersey home in Evanston that an ambulance would have taken them too.

When Frank Kersey uses a key to enter his brother's house, there is a keypad for an alarm system that is never used. The alarm may not have been armed on the night of the initial attack against Paul's family, but it seems highly unusual for Paul not to arm the system when he has so much more to hide later in the film.
In the liquor store, when the owner texts another criminal to help him,he texts about "The Doctor from that thing". Paul hasn't met or interacted with any of the burglars.
