(at around 26 mins) When Alice tied up Claire after a fight, Claire's hair is untidy, her face and arms are dirty and and her clothes are ruined. Later, in the plane, Clair is perfectly coiffed, clean and made-up. It seems that she also has new clothes, yet the two were only on a small airfield with no infrastructure around.
(at around 1h 21 mins) When Alice confronts Wesker at the end of the movie, we notice that Chris follows her. However, when we see Chris and Claire later, they appear behind Wesker, even though they took the same path as Alice, who ended up in front of him.
The clones of Alice in the beginning go from a slight platform heel to flats and back and forth during the action scene.
(at around 54 mins) When the Axe zombie hits the prison gates, he receives a few shots in the head and at least one hole in the cloth covering his face. When later he appears in the prison bathroom, there are holes in the cloth only on the back of his head.
(at around 1h 5 mins) When Claire is chased by the huge Axe Zombie in the shower room he can be seen cutting several shower posts in half just behind Claire's back, but in the next scene while she is still running in a straight line, the posts in front of her are also destroyed and fountaining water, although neither have passed them yet.
The inside of the Arcadia does not match the exterior of the vessel in either shape or size.
(at around 45 mins) Alice is seen stacking quarters to load into her 12 gauge shotgun, and later uses her shotgun, apparently filled with quarters, to kill a number of zombies. When she does so, a quarter is seen embedded in the wall next to Luther West (at around 1h 4 mins). She again uses them on the "Arcadia" to kill one of the zombie dogs (at around 1h 23 mins). However, the diameter of a quarter is 0.955 inches; therefore, it would not fit in a 12 gauge shotgun, as the diameter of the barrel is 0.729 inches.
(at around 11 mins) When the Alice clones attack Umbrella Corporation HQ, they fire thousands of rounds from sub-machine guns without reloading. The Tmp machine pistol they were using can only be fired for about 4 seconds before emptying.
In the scene where Alice lands the plane on the roof of the prison the length of the roof is far too short to have landed in that distance at the speed they were traveling. The wings also impact several solid objects during the landing which would have damaged them, if not severed them completely. At the end of the landing the nose smashes through a solid concrete ledge at the end of the run. The propeller is too fragile and should have shattered in an impact that forceful but instead remains completely unscratched.
GPS readings on the video camera on the beach in Alaska show "61°13'N LON 149°54'W LAT".

(at around 8 mins) During the opening battle at the Tokyo Umbrella division, Alice throws a sword through the head of a guard and despite it penetrating the guard's head to just a few inches shy of the hilt, it does not penetrate through the back of the guard's head, which is incorrectly regarded as a goof because we see a bullet strike the sword, breaking the blade, as Alice spins through the air, making the sword almost useless, resulting in Alice's decision to throw it at the guard.
(at around 57 mins) Before going underwater in search of the armory, Alice gives Chris one of her revolvers and the other one to Crystal. Later after they are confronted by the zombies, Chris starts shooting with two revolvers, and we have not seen him take the one from Crystal. However, upon closer inspection it's clear that Crystal drops her gun when she gets pulled by the zombie into the water. Just before Alice strikes the next undead, Chris starts to lean to reach the gun so in the next scene he has both.
(at around 1h 4 mins) Alice plans to escape the prison by leaving through the storm drains in order to access the sewer through which she and the other survivors will reach the ocean. Storm drains do not drain into the sewer, and sewers do not drain into the ocean. Sewers are specifically designed to prevent contamination of storm water and are never designed to drain directly into the ocean, which would result in raw untreated sewage contaminating the ocean.

Throughout the series the undead must be shot in the head to be killed, whereas in this movie many undead are felled by body shots. This is not a goof: all the zombies that are shown to actually die are shot in the head. The rest are presumably only briefly incapacitated.
(at around 24 mins) After Alice lands in what she thinks is Arcadia, and after her video-log, while investigating the "mysterious figure" she readies both her revolvers and points them right at the camera. There are no bullets in the chambers. Also, even if the chamber behind the barrel of each gun WAS loaded, when she pulled the trigger, the cylinder would revolve and the hammer would strike the next (and obviously empty) chamber.
(at around 1h 6 mins) While Claire is fighting with the Axeman, he chases her only to have her do a wall run and vault back over his head. This would see her land 10-12 feet behind him. But when the action continues Claire starts running from the far end of the shower room, a good 30 feet away.

Even if the Axeman had superhuman strength, the total mass of the axe that's he's wielding has to exceed his own, meaning he would move in the direction of travel every time that he swung the axe. This doesn't occur.

At the end, when everyone is on the deck of Arcadia, Umbrella helicopters are heard by the heroes and they look at them. On the other hand, none of the civilians in white clothes bother to look at the helicopters as it's a usual thing. They continue their actions.
(at around 52 mins) When Chris explains to the group what the UPV stands for, he refers to it as a "sixteen-wheeler", whereas the vehicle seen in the garage is actually an eight-wheeler with double tires in the back.
When Alice is making her video she says "May 3rd 18:30 hours." However, a few seconds later when the perspective of the camera is shown the time is actually 19:31.
