PCI to very tough CTO LAD (JCTO-4) Proximal CAP at large Side branch, heavily calcific vessel, failed previous trial, length > 20. Over corsair MC, CP 12 puncture of proximal CAP, then deescalsted to Gaia III within the CTO body, Distal CAP at diffusely diseased segment. Sion Black went to small septal, distal CAP modification using 1.2 balloon 🎈, then successfull wiring of distal LAD.. 2 hours of hard work ended with successful PCI using 2 DESs. #CTO
