Assent Form for Children and Adolescents


Please get your parent/guardian to read this with you. For students ages 8-14 What is this for? This is for a study being done at the University of British Columbia. We want to learn more information about what makes you happy. First, we will tell you about the study, then ask if you want to be in the study or not. Why are we doing this study? We want to learn about how material goods are related to your well-being, happiness, and spirituality. What happens if you agree to be in the study? Two things will happen if you agree: 1) You will complete a number of questionnaires online that ask about your well-being, satisfaction with life, hope, gratitude, spirituality, and material values. 2) Your parent/guardian will be asked to be in the study as well, and he/she will complete questionnaires about his/her well-being, life satisfaction, hope, gratitude, spirituality, material values, and perceptions of your well-being. Is there any danger to you by being in the study? There is no danger to you participating in this study. You will only be asked to fill out some questionnaires.

Is there a benefit to being in the study? If you and your parent/guardian participate, you will both be entered into a draw to win two free movie tickets. One out of every 150 participants will win. Your and your parents names will only be entered in the draw if BOTH of you participate. What if you have any questions? If you have questions about a word or meaning you can ask your parent/guardian, or you can have your parent/guardian email us (economicsofhappiness14@gmail.com). You are encouraged to ask questions. What if you dont want to be in this study? Nobody is forcing you to be in this study. The choice is yours. You will not be treated any differently than those who are in the study. Participating in this study is completely voluntary. If you choose to be in the study, click I accept at the bottom of the page.
