REQUEST FOR
EXTENSION TO HANDING-IN DEADLINE

This form should be submitted prior to the submission date for the assessment. The form
should be accompanied by a doctor’s certificate or a note from your Module/Personal Tutor
supporting the claim.

The decision on whether to give an extension to a deadline is the responsibility of the Course
Leader. Do not assume that a request for an extension will automatically be approved. The
Course Leader will return a copy of the form to you indicating whether or not the extension
has been agreed. If it has been agreed, you must attach this form to your assignment.

Section B: Nature of Claim/Details of Circumstances


Students should simply indicate the broad heading under which the claim is made, e.g. illness, accident,
jury service etc. Appropriate evidence should be attached. If the circumstances are confidential, full
details need not be written but the Module/Group Tutor’s agreement needs to be shown.
