To establish the first Human Heart Valve Tissue Bank (HVB) in Sri Lanka for the cryopreservation of heart valves and
systematically supply cryopreserved valve homografts for heart surgeries for children with Congenital Heart Defects
(CHD) in Sri Lanka thus reducing the cost and increasing the number of surgeries that can be undertaken to repair CHD.
Our aim is to give these children a chance to survive to adulthood and ultimately contribute to the Economy when they
are adults.
At present a serious limitation in providing medical treatment is the availability of an adequate supply of heart valves. The
HVB will be set up in accordance to international standards.

SUSTAINABILITY
What community needs will your project address and how were these needs identified? Provide any relevant
data or survey results
In Sri Lanka for every 1,000 newborn babies, 6-8 are born with a heart defect. It is estimated that approximately 2,500 to
3,000 children are born with heart diseases every year in Sri Lanka and around 1,600-2,100 of them will need
intervention for their heart lesions. If these children are not treated in a timely manner, some of them will not live to their
first birthday and a majority will succumb before they reach adulthood. A majority of these cases are curable if surgery or
medical interventions are made available at the proper time.
Currently, more than 95 per cent of children with congenital and acquired heart diseases in Sri Lanka are treated at the
Lady Ridgeway Hospital (LRH) for Children.
In a study conducted by the Professorial unit at LRH showed that the case fatality rate among children with heart
diseases was 18 per cent within a period of one year. It further states that CHD was the second commonest cause of
deaths in the unit and unless surgical care for patients with congenital heart disease in the public sector is improved,
many such children will continue to die early due to lack of facilities.
Congenital Heart Disease (CHD) is a major killer of children and a main contributing factor to the Infant Mortality Rate
(IMR). Sri Lanka, with its excellent free public healthcare system and low infant mortality (8/100 live births), is ready to
lower IMR further by reducing deaths due to CHD.
Sri Lanka does not have a human HBV and the current practice in Sri Lanka is to use imported bovine grafts, which
costs approximately US$ 2,300 per graft (for a country with a GDP per capita of US$ 3,280 in 2013 this is a major cost).
Furthermore, all sizes of these bovine grafts are not freely available when needed. In addition they have to be
frequently replaced due to early calcification.
The best option is to use human tissue grafts harvested from cadavers as human tissue grafts last longer and can be
provided at a lower cost thus releasing funds for more free surgeries to be carried out and to save more lives.
In Sri Lanka, it is easy to obtain donors as donating body organs is considered a noble deed. For example although the
United States is the world's biggest cornea provider, sending more than 16,000 corneas to other countries (2010) Sri
Lanka, which is 15 times smaller, actually donates about triple that number of corneas per capita each year.
Detail how your project will address these community needs.
About 1,000 children undergo cardiac catheterisations every year at LRH and more than 60 per cent of these are
interventional procedures, which provide a cure for the patient. Since 2007 the surgical procedures at LRH has increased
from 300 cases per year to 900 cases per year by 2013, which includes complex congenital heart surgeries.
Unlike adult heart disease paediatric heart disease is not covered by insurance, the parents cannot afford the expensive
procedures as they are mostly newly married and have little savings after expenses such as the wedding, pregnancy and
confinement, delivery and scans.
Currently, bovine grafts are used in surgeries and they are expensive, all sizes are not freely available when needed and
have to be frequently replaced. Therefore, to supply the growing demand for human heart valves in the country, it is
essential to establish our own heart valve bank in the country.
How were members of the local community involved in planning the project? Does your project align with any
current or ongoing local initiatives?
Cardiac surgery in children was initiated by the Ministry of Health (MOH) at Lady Ridgeway Hospital (LRH) in 2007 with
two operating theatres and a 6 bedded ICU. Then the ICU was expanded to 18 beds in 2010 to accommodate more
surgeries. There are plans to increase the theatres from 2 to 4 and ICU beds from 18 to 36 to accommodate more within
next few years. This project which runs parallel to above development will provide tissue needed for increase in demand

that will surely result with this expansion. It is on a national scale and the Rotary Club of Colombo Fort has been working
with the Management of LRH and the MOH in planning the project to ensure that it is very much in line with the MOHs
policy and plans.

How will recipients be selected?
Once set up an awareness campaign will be carried out in collaboration with MOH and/or LRH. There will be special
emphasis to target the poorer sections of recipients.

How will you meet these goals?
CHD is more common than acquired disorders, affecting an estimated 3,000 newborns every year in Sri Lanka. It is one
of the most common birth defects in children, and a major contributor to morbidity and mortality in children under five
years of age. Reduce the number of deaths due to CHD by performing more surgeries using the lower cost human heart
valves, which will be harvested, preserved and made available by the HVB when required, thus releasing more funds for
more surgeries to be carried out.

LRH is one of the largest paediatric hospitals in the world that provides its services free of charge. It is the tertiary care
referral centre for children with heart diseases in Sri Lanka. LRH will provide the following:

ROTARIAN PARTICIPATION

Describe the role of the host Rotarians in this activity and list their specific responsibilities.
The host Rotarians carried out initial project assessment, needs analysis, built a project budget, the initiated the project,
found funding and partners, conducted the tender process for the required equipment for setting up the HVB,
coordinated with LRH and MOH.
The Host Rotarians will also be responsible for disbursing the funds, maintaining project accounts and preparing the final
report. At all stages they will interact with the International project partners.
Describe the role of the international Rotarians in this activity and list their specific responsibilities.
The international Rotarians created awareness in their respective countries, will provide funding for the project and assist
in obtaining more partners/funding for the project. RC-Madras Coromandel shared their display booth at the Sydney
Convention 2014 with RC-Colombo Fort to create awareness of the project. They will be in constant contact with the
Host Rotarians and if possible visit the project site as well.
SUSTAINABILITY
Describe the role that members of the local community will play in implementing your project. What incentives
(e.g., compensation, awards, certification, promotion) will you provide to encourage local participation?
Local community (LRH-MOH) will provide,
The location and building for the establishment of the HVB, this will include all civil work of the HVB;
Technical assistance from Doctors/Surgeons/Nurses and technical staff required to operate the HVB;
Operate and maintain the HVB including meeting operating expenses.

Dr. Duminda Samarasinghe, has worked with the RC-Colombo Fort from the initial stages of the project from scoping
and planning to date. Both of them have provided technical expertise and at their own cost visited other HVB in the
region (at All India Institute of Medicine to ascertain the requirement. They are also in constant communication with the
Heart Valve Bank in Oxford University Hospital in the UK and all planning is carried out as advised by the technical team
at Oxford Heart Valve Bank.

Where possible we have obtained quotations from local medical equipment agents and where required from regional
offices of medical equipment manufacturers. A competitive bidding process was carried out and we have obtained
quotes from the leading Medical Equipment Agencies in Sri Lanka and overseas.
How will the beneficiaries maintain these items? If applicable, confirm that spare or replacement parts are
readily available and that the beneficiaries possess the skills to operate equipment.
When purchasing the equipment a 5 year Service Level Agreement will be included. The MOH via LRH will maintain the
equipment.
Who will own the items purchased with grant funds at the end of the project, including equipment, assets, and
materials? Note that items cannot be owned by a Rotary club or Rotarian.

FINANCING

The Rotary Foundation funds global grants from the World Fund, and awards range from US$15,000 to
US$200,000. The Foundation matches cash contributions at 50 percent and District Designated Fund (DDF)
contributions at 100 percent. The Foundation will also match non-Rotarian contributions toward a grant,
provided they do not come from a cooperating organization or a beneficiary.
To determine the World Fund match for your global grant, list all sources of funding, specifying contributions
from cash, DDF, and other sources. Non Rotarian contributions with no match from TRF can be included in the
grant financing if they are being used to purchase grant budget items. These contributions should not be sent
