ABSTRACT
Despite recent developments in the Bangladesh healthcare sector, there is still great
concern about the quality of healthcare services in the country. This study compared the
quality of healthcare services by different types of institutions, i.e. Public and private
hospitals, form the perspective of Bangladeshi patients to identify the relevant areas for
development. A survey was conducted among Bangladesh citizens who were in patients in
public or private hospitals in Comilla. About 100 exit interviews were conducted using a
structured questionnaire that addressed the probable factors of quality of service in 5 point
interval scales.
The quality of service in private hospital scored higher than that in public hospitals for
nursing care .Tangible hospital matters i.e. cleanliness supply of utilities and availability of
drugs .The overall quality of service is better in the foreign hospitals compared to that the
private hospital in Bangladesh in all factors , even the perceived cost factors. This paper
provides insights into the specific factors of the quality of hospital services that need to be
addressed to meet the needs of Bangladeshi patients.

INTRODUCTION
Our country has a good healthcare network covering both rural and urban areas. There are
about 3,976 healthcares in the public sector and 975 privately-run hospitals/clinics. In
Bangladesh, 13% of treatment-seekers use government services, 27% use private/NGO
services, and 60% get non qualified services.
Large segments of the population in developing countries are deprived of a fundamental
right: access to basic health care. Without an appropriate and adequate health support and
delivery system in place, its adverse effects will be felt in all other sectors of the economy.
In simple terms, an ailing nation equates to an ailing economy as manifested in lower
income-earning capacity of households and significant productivity losses in those sectors
that sustain the economy.
To identify the preference of people towards public and private healthcare services.
To examine whether income and occupation have any impact for choosing private or
public healthcare services.
To identify the major factors that affect for choosing private or public healthcare
services.
To find out why people give special emphasis to private and public hospital.
SOURCES OF DATA
To prepare our term paper we collect and use primary and secondary data. We visit public
and private hospital and also browse web address and talk to with some people and collect
information. Here we point out some of this sources:

METHODOLOGY
Our study was a critical and constructive one. We survey public and private healthcare
service and also take interviews from 100 people in our perspective area. For the selected
people we used specialized methods as we divided our whole area into four parts on the
basis of location or standard of living. First area is the center of the town consist of
Kandirpar, Monohorpur, Jhawtala, Police line, second area is the area close to the town
like cantonment, Paduarbazar and Dowlatpur, Debidwar, Chandina and Barura are the
area third and the area fourth consist of several rural villages. To get hospital authority
opinion, we select one famous public hospital name Comilla medical college hospital
and one renowned private hospital name Comilla medical center (pvt.) Ltd.
First we draw a questionnaire and same technique for 100 respondents . We took interview
to our respondents personally so that we can attain our necessary information. We gave
maximum priority to reduce several disturbances or misunderstanding in our field work.
However we find out the choice of the public or private healthcare service. Income and
occupation are the prime factors that verify the choice. When we went to private and public
hospital then we asked same question to the authority and collect different information.
Problems of bureaucracy
WHAT IS HEALTH?
Heath is a study of complete physical, mental and social wellbeing and not merely the
absence of disease or infirmity. It agrees that health is a fundamental human right at that
the attainment of the highest possible level of health is a most important world -wide social
goal whose realization requires the action of many other social and economic sectors in
addition to the health sector.
PREVAILING CONDITION
Despite recent development in Bangladesh healthcare sector, there is still great concern
about the quality of healthcare service in the country. In Bangladesh medical facilities are
not so good. Most of the people in our country are not get public or private healthcare
service. Hospital is not available and they are not aware about their health thats why they
take suggestion and service from village doctor or pharmacy.
Although a lot of public healthcare centre in Bangladesh but these are not fulfill our needs.
Moreover many healthcare centers in rural area have no qualified doctor or have no doctor.
Among the public healthcare service Medical college hospital provide better service. On the
other hand from rural hospital people get limited service, most of the time rural hospital
suggests going to district hospital for treatment and various test.
The private healthcare centre provide better service in comparison to public healthcare
centre but the cost of the private healthcare service is so high for this reason most of the
people are not able to take this service. Only the city people and rich people get this
service properly.
Sometime patients are harassed by brokers and middleman. They also harassed by the
diagnostic centre and others medical related syndicate in various test and operation.
Although public and private hospitals provide a lot of medical facilities to the people but
all the medical facilities and treatment are not available in Bangladesh. For some incurable
diseases people prefer to go foreign countries such as India, Singapore and other developed
countries.
WORLDS PERSPECTIVE
In prevailing condition we see that the condition of healthcare service in Bangladesh. We
see that, the healthcare service is not well in Bangladesh. On the other hand the world
healthcare services are better than Bangladesh. In first world countries all the medical
services are available and the quality of the services is highly affordable. Many people of
the world come here to get better treatment and other facilities. Although the treatment is
available and affordable but all the people cannot get this facilities for higher cost and
other formalities. People face many problems to go developed countries. In third world
countries all the medical facilities are not available and reliable.
medical facilities because their hospitals are not able to provide all the medical treatment
for lacking of qualified doctors, nurses and sufficient instrument. For their poverty a few
number of people go to foreign countries to get better treatment. In the world few poor
countries such as Congo, Zimbabwe and some African countries people die for lacking of
treatment. Most of the people in this country do not get basic treatment for this reason they
suffering from many diseases. Moreover developed countries medical facilities are good
and developing countries medical facilities are not good.
Table- 01, Shows that among 100 people (in percentage) 60 people prefer public hospital
and 40 people prefer private hospital to get medical service on the other hand 40 people is
disappointed to public hospital and 60 people is not going to private hospital.

In table- 03, Show that, 40 people out of 100 (In percentage) dislike public medical service
on the other hand 60 people out of 100, also dislike private medical service for distinct
attributes. They are: Inefficiency, Higher cost, Ineffective treatment, lacking of doctors and
nurses and other attributes.

Table- 04, Shows that, Income level and occupation have great impact on choosing public
and private healthcare service. Here we divide income in three categories under 10000,
10000-20000 and 20000- above, and we divide occupation in 4 categories. They are
students, service holder, businessman and others, farmer in a different income level and
different occupation, preferring public and private medical service is not same.

FINDINGS
We were asked our respondents to maintain their most favorite hospital with a view to
examine the condition of different hospital. The respondents showed their opinion about
their favorite hospital in accordance with responsiveness, discipline, best quality, effective
treatment, availability, low cost. We can decide from table- 01, that about 60% of our total
respondents prefer public healthcare service. Respondents also showed their cause for
voting public healthcare service. They prefer it because it ensures quality, low cost,
availability, good facility and other attributes of public expectation. Rural area, urban area,
town area and in big city public healthcare service are available.
We also decide from table-01, that about 40% of our total respondents prefer private
healthcare service. They prefer it because it ensures high quality, availability, good facility,
standard service, high status and other attributes of public expectation. In Bangladesh,
private healthcare service are available all town, city and big cities.

RECOMMENDATION
To recover this problem Govt. should take some effective steps. People also prevent this
problem by increasing awareness among them.
Govt. can impose specific rules and regulation in healthcare service which is strictly
maintain
by healthcare service.
Seat problem can be solved by providing extra seat and improving other residence
facilities.
Every healthcare service should have rules and consciousness about Brokers.
Govt. can solve instrument problem by providing latest and necessary instrument.

At the end of the day we see that although our healthcare condition is not good but if govt.
and hospital authority try to recover this problem then there is huge possibility to recover
this situation. Although private hospital provide better service but its cost is so higher than
public hospital. It should minimize this cost. On the other hand public hospital quality is not
good. It should improve its quality because a large number of people in our country depend
on public hospital.
