CONTRACT OF CONSULTANCY

KNOW ALL MEN BY THESE PRESENTS:


This Contract entered into this __________ day of _________________, 20_______ in
Quezon City by and between:
THE HOUSE OF REPRESENTATIVES, Republic of the Philippines, with offices at the
Constitutional Hills, Quezon City, represented by the Honorable CESAR S. PAREJA,
Secretary General, hereinafter referred to as the Party of the First Part
- and with ________________________________________________________ Filipino, of legal age,
and with postal address at__________________________________________________
hereinafter called the Party of the Second Part.
WITNESSETH
That the Party of the First Part hereby engages and retains the services of the Party
Second Part for the position CONSULTANT, ______________________________________ who
undertakes to perform the work and duties pertaining to said position, subject to the following
terms and conditions:
1. This Contract shall take effect ___________________________________________
up to ___________________________________;
2. For such services rendered he will be paid the amount of _________________
_______________________________ (P_____________) pesos monthly, according
to the payroll periods of the Party of the First Part;
3. That the Party of the Second Part shall draw whatever allowances and other benefits
as may be authorized by the Party of First Part. It is understood, however, that
these benefits shall always be the option of the Party of the First Part and shall be
issued only when essential to the efficient performance of official functions.

4. That this Contract shall cease to be of any force and effect in the event of
unsatisfactory performance of work by the consultant or for such other
causes authorized by law, or in the event that contracting parties agree to
have the contract terminated, provided, however, that written notice shall be
made at least fifteen [15] days before; and
5. Upon the lapse of the contracted period, the services of the Consultant shall be
deemed already terminated.

IN WITNESS HEREOF, the Parties have hereto set their respective signatures at the
above-mentioned place and date.
