Checklist for Applicants for the Certifying Examination in Anatomic & Clinical Pathology

REQUIREMENTS
1. Accomplished Application for Certification form (BOP Form 2).
2. Two (2) pieces 2x2 photos (taken within the last 6 months).
3. Photocopy of birth certificate or naturalization form.
4. Accomplished Application Form for Board Eligibility (BOP Form 3).
5. Photocopy of the Certificate of Residency Training awarded by the training
institution and signed by both the medical director and the department chair.
Certification must state that the applicant has fulfilled all requirements and
will graduate on December 31.
6. Letter of endorsement/certificate of good moral character signed by both the
department chair and training officer. If in a combined program, separate
certificates should be issued for anatomic and clinical pathology.
7. Photocopies of ten (10) autopsy reports:
- Must include at least five (5) complete autopsies.
- Only autopsies with the applicant as the main prosector will be accepted.
- All copies must be duly certified as true copies by the Record Section of
the training institution.
8. Photocopies of fifty (50) Surgical Pathology Reports with the applicant as a
co-signatory:
- All cases must be different.
- At least twenty (20) of the cases must be from radical procedures.
- All copies must be duly certified as true copies by the Record Section of
the training institution.
9. Research or scientific paper pertinent to the specialty being applied for
either published or presented in a research forum, with the applicant as the
principal author. A certificate verifying publication or presentation duly
signed by the Chair, Training Officer and Medical Education Department
must also be submitted.
10. Photocopy of Philippine Board of Medicine license. (Bring original)
11. Photocopy of Medical School Diploma. (Bring original)
12. Photocopy of the Certificate of Accreditation for Residency Training
issued to the training hospital by the PSP.
13. Duly accomplished form of Agreement to compliance with the rules and
regulations of the Board (BOP Form 4).
14. Examination fee Php 3,500.00 for each examination.
