HOLD HARMLESS AGREEMENT
I ____________ hereby certify that my dog(s) _________________ is/are
in good health and has/have not been ill with any communicable condition in the last 30 days. I agree to not bring my dog if they show signs of illness (vomiting, diarrhea, coughing, limping, etc.). We will require veterinarian certification of health to be admitted or readmitted into the facility. This helps us protect your pet by preventing contagious disease or further injury. I further certify that my dog(s) has/have not harmed or shown aggressive or threatening behavior towards any person or any other dog and that my dog(s) is/are being admitted based on this representation.
Titans Doggy Daycare reserves the right to terminate any
future visits at any time if the dog is determined to be problematic in any manner.
I understand that I am solely responsible for any harm to staff
members/volunteers or other persons present, while at the facility, as well as other dogs, caused by my dog(s) while my dog(s) is/are attending Titans Doggy Daycare. I hereby release them of any liability of any kind whatsoever arising from my dog(s) while attending Titans Doggy Daycare.
I further understand and agree that Titans Doggy Daycare and their staff/volunteers, will not be liable for any problems that develop, provided reasonable care and precautions are followed, and I hereby release them of any/all liability of any kind whatsoever arising from my dogs/dogs attendance and participation at the daycare center.
I hereby acknowledge and am aware that the staff members of Titans
Doggy Daycare are not veterinarians and do not have backgrounds in animal medicine and are not expected to diagnose illnesses. Staff members will do their best to identify/detect any illnesses that may present themselves. I am fully aware that vaccines do not protect against ALL communicable illnesses that may affect a guest. I understand that even though all pets are closely monitored, it does not guarantee your dog may not come home without a few play marks. There are always risks involved, including scrapes, scratches and cuts, which are commonplace due to the nature of dog play. We do not allow rough play, but we do allow play. We will inform you of any injury that we are aware of; however, we may not see all scraps and scratches that occur while here at Titans Doggy Daycare. I hereby give permission for employees of Titans Doggy Daycare or a veterinarian to administer treatment to my dog if necessary. I understand that employees of Titans Doggy Daycare will do his or her best to contact me first before treatment.
I certify that my dog has never harmed or threatened anyone or any other animal and that my dog(s) is/are being admitted based on this representation.
There is inherent rough-housing in play. All dogs are screened for
aggression, but fights can and will still occur! Every dog has the potential to bite. All dogs have a bite threshold. It is also possible for a puncture wound to happen during play sessions. In the event of a fight or injury, if needed, we will contact your vet and transport the pet there or to the closest vet of our choice. You will be responsible for any and all bills occurred. Staff members of Titans Doggy Daycare always attempt to contact the owner or emergency contact before providing any medical attention.
I, the undersigned, am fully aware of Titans Doggy Daycare vaccination policy. I
agree to provide Titans Doggy Daycare with written proof of current vaccination records from a certified veterinarian. I am also aware that at NO time will my dog be allowed to enter daycare with no or expired vaccination records. Although we adhere to very strict daily cleaning and sanitation procedures, we cannot be held responsible should your dog contract kennel cough or any other illness.
I certify that I have read this Hold Harmless agreement in its entirety and fully understand the policies and procedures of Titans Doggy Daycare. Thank you for choosing Titans Doggy Daycare and Boarding!
