purposes, this error is hereby requested rectified in order to set the
records straight so as to reconcile his correct middle name and date of
birth in to his SSS ID, issued by the Social Security System,
Zamboanga District Office, Zamboanga City, which is Santos, born
on December 19, 1970, instead of Elavasantos, born on
December 19, 1971, as evidenced by his Tax Identification Number
(TIN) ID and Certificate of Marriage, issued by the Bureau of Internal
Revenue (BIR) and Philippine Statistics Authority (PSA) respectively;
