    I, NERISA V. CAON, Filipino, of legal age, widow and resident of


    Blk. 110, Lot 9, Brgy. Sta. Lucia, Dasmarias City, Cavite, after being duly
    sworn to in accordance with law, depose and state: That - -

    1. I am the mother of the following minor children to wit:

    KATHLENE MAE V. CAON JULY 5, 1999
    ALDWIN V. CAON AUG 14, 2000
    MARK ADRIANE V. CAON SEPT 12, 2006
    ALBERT V. CAON OCT 14, 2009

    2. My common-law-husband ALBERTO CAON died on 28


    October 2016 at Army General Hospital, Fort Bonifacio, Taguig
    City;

    3. Since the demise of my common-law-husband I am the one


    taking custody and guardianship over the above-mentioned
    minor children;

    4. Through this Affidavit, I hereby declare under oath that I am


    the mother, guardian and the one taking custody of the above-
    mentioned minor children since the demise of their father;

    5. I am executing this Affidavit to attest to the truth and veracity


    of the foregoing and for whatever legal purpose the same may
    serve;

    IN WITNESS WHEREOF, I have hereunto affixed my signature
