    SUBJECT : SIX MONTHS TRAINING ON FORKLIFT OPERATOR

    In the exigency of service and in the interest of personnel development, your request to
    attend a six (6) months training for forklift operator at the Zamboanga City Integrated
    Port Service Inc. covering the period November 16, 2015 to May 16, 2016 is hereby
    approved.

    This privilege does not cover the grant of full study leave thus you are to continue
    hospital duties for the morning shift until the end of your training as specified. No
    additional remuneration and allowances shall be given for this training.
