Introduction
• Neurological deficit resulting from
Cerebrovascular disease
a sudden interruption in the blood
or stroke flow to the brain.
• rapidly developing clinical symptom,
with symptoms lasting > 24 hours
Cardioembolic stroke
5
Introduction
Transient ischemic attack (TIA)
• An acute loss of focal brain function with symptoms lasting
< 24 hours.
• TIA = Mini stroke
• Major determinant of future stroke especially within the first week.
6
Introduction
Treatment of ischemic stroke
1. Thrombolytic agent
2. Antiplatelet
In several trials, aspirin has been shown to
Aspirin reduce the risk of recurrence stroke by
approximately 20%.
• Intervention
• Arm 1 : Clopidogrel 300 mg LD follow by 75 mg/d 90 days +
Aspirin 75 mg/d in first 21 days.
• Arm 2 : Placebo + Aspirin 75 mg/d 90 days.
8
Objective
To evaluate the efficacy
Aspirin alone
in reducing the risk of recurrent stroke after
a minor ischemic stroke or a TIA.
10
Trial design and oversight
Study design
• Randomized, double blind, placebo-controlled trial
12
Trial population
Inclusion criteria
• High-risk transient ischemic attack.
• Minor ischemic stroke.
• Ability to randomize within 12 h.
• CT or MRI of the head ruling out hemorrhage.
• Ability to tolerate aspirin at a dose of 50–325 mg/day.
Exclusion criteria
• Receipt of any thrombolysis within 1 week prior to index event
• Clear indication for anticoagulation (e.g., AF, DVT, PE)
• Contraindication to clopidogrel or aspirin
• Anticipated requirement for long-term (>7 days) antiplatelet drugs
(e.g., dipyridamole, ticlopidine) or NSAIDs.

4 CASP checklist
30
A Are the results of trial valid?
31
1. Did the trial address a clearly focused issue?
YES
Objective
To evaluate the efficacy of Clopidogrel plus Aspirin compared with
Aspirin alone in reduce the risk of recurrent stroke after a minor is
chemic stroke or a TIA.
Population
Patients who were at least 18 years old and could undergo
randomization within 12 hours after
• an acute minor ischemic stroke (NIHSS ≤ 3)
• a high risk TIA (ABCD score ≥ 4)
32
1. Did the trial address a clearly focused issue?
Comparators
• Arm 1 : Clopidogrel 600 mg LD, 75 mg/d from day 2-90 + ASA 50-325 mg/d
• Arm 2 : Placebo + ASA 50-325 mg/d
Outcome
• Efficacy outcome : Ischemic vascular events
• Safety outcome : Hemorrhage
33
2. Was the assignment of patients to treatments
randomised?
YES
34
3. Were all of the patients who entered the trial
properly accounted for at its conclusion?
YES
Intervention

Sensitivity analysis
37
5. Were the groups similar at the start of the
trial ?
YES
Baseline
38
39
40
6. Aside from the experimental intervention,
were the groups treated equally ?
YES
41
B What are the results ?
42
7. How large was the treatment effect ?
Efficacy outcome
• Primary efficacy outcome : composite of ischemic stroke, MI
or death from ischemic vascular causes
• HR = 0.75(0.59-0.95) , P=0.02
Safety outcome
• Primary safety outcome : major hemorrhage
• HR = 2.32(1.10-4.87) , P=0.02
As-treated 43
8. How precise was the estimate of treatment
effect ?

45
C Will the results help locally ?
46
9. Can the results be applied in your context ?
(or to the local population)
Baseline characteristics
Local population
48
10. Were all clinically important outcomes
considered?
YES
Efficacy outcome
• Composite of ischemic stroke, myocardial infarction, or death
from ischemic vascular causes
• Each component of primary outcome
Safety outcome
• Major hemorrhage
• Minor hemorrhage
• Death from any cause 49
