INTRODUCTION
Vital statistics
Vital statistics refers to the data or the methods of analysis of data relating to such vital events of
human life as birth, death, sickness, marriage, etc. the basic data are usually available from two
principles sources:
Population census, conducted every ten years in almost all countries of the world, provides
information relating to the number of persons inhabiting a certain area, classified according to
age, sex, occupation, etc. at specified ‘points’ of time. Registration of births, deaths, etc by the
appropriate authorities provides data relating to births classified by sex of the new born child,
age of mother, etc. and the deaths classified according to causes, ages at death, etc during
different ‘periods’ of time. Hospital records show various data relating to sickness, number
surviving a certain disease, etc.
Here, we shall be concerned with the measurement of some rates of mortality, fertility and
overall growth of population. In general, we define,

Although, the above formula is always a fraction, rates of vital events are usually expressed “per
thousand”. Also, the rate must relate to a particular (i) type of event, example: birth rate, death
rate. (ii) Geographical area, example: India, Kerala, and (iii) period of time, example: year 1971.
For example, the birth rate in India during 1976 was 34.4 per thousand populations.

OBJECTIVES
Trends in Mortality
This aims to explain global and regional trends in life expectancy at birth. It assesses the
contribution of various major causes of death to differences in survival between populations. It
provides a detailed analysis of the sex- and age-patterns of mortality that produce regional trends
and differences in the levels of life expectancy at birth.
Many of the greatest advances in medicine have come as a result of public health research
interventions. The ability to intervene in the health of populations is dependent upon
development of appropriate tools for measuring health, illness, interventions, and outcomes.
Only by standardizing communication can clinicians hope to target high-risk populations with
effective interventions. Therefore, understanding the common language of public health is vital.
Each year, the Census Bureau's Population Estimates Program uses current data on births,
deaths, and migration to calculate population change since the most recent decennial census and
produces a time series of estimates of population, demographic components of change, and
housing units. The annual time series of estimates begins with the most recent decennial census
data and extends to the vintage year.
The fertility rate measures the number of births per 1,000 women of childbearing age (ages 15 to
44) occurring in a particular year; birth rates refer to this measure within particular age groups.
Disability
Having a disability places you in the world's largest minority group. Currently around 10% of the
world's population, or roughly 650 million people, live with a disability. In most of the OECD
countries, females have higher rates of disability than males.
As the population ages this figure is expected to increase. Eighty per cent of persons with
disabilities live in developing countries, according to the UN Development Program (UNDP).
The World Bank estimates that 20 per cent of the world's poorest people have some kind of
disability, and tend to be regarded in their own communities as the most disadvantaged. Statistics
show a steady increase in these numbers.
Deaths
As global population increases, life expectancy rises, and living standards improve, causes of
death across the world are changing. In this entry we present a global overview of the causes of
death. The data visualizations and explainers in the early part of this entry attempt to provide a
comparison between causes and risk factors of death across countries and age groups. The
sections which then follow explore the empirical data of specific causes in more detail.
Each of the charts which follow is shown at a global level. However, all of the data included in
this entry is available to explore at the national level using the "change country" function on the
interactive charts. As will become clear in the data which follows, causes of death across the
world remain heterogeneous and continue to change.
Population projections are estimates of the population for future dates. They are typically based
on an estimated population consistent with the most recent decennial census and are produced
using the cohort-component method.
TYPES
Measurement of Mortality
The most common form of measuring the mortality is the crude death rate. We can use the
following formula to compute the crude death rate:

Here, mcd is the crude death rate, D is aggregate deaths for a given area and time period, mostly a
calendar year, P is the size of population at the risk of death, usually taken as the estimated
population at the midpoint of the calendar year, and k is a constant that is usually considered as
1000. The crude rate is so named to differentiate it from various specific and adjusted rates and
represents the total or overall death rate without regard to the various component elements which
combine to produce the total figure. The crude death rate is usually expressed as “the number of
deaths per 1000 persons” for a specified place (country, city, state, etc.) for a given year.
Detailed analyses of vital statistics usually go beyond the overall risk of death in the population
as a whole. Many statisticians study the subsets of the population or specific classes of deaths.
Epidemiologists often focus on deaths from a disease or class of diseases. Actuaries and
demographers are concerned with differences in mortality by sex and in different age groups
within the population. Environmental and occupational health specialists are interested in the
differential risks of dying in selected occupations, and in different geographic subdivisions such
as urban and rural areas. To meet these kinds of needs, various specific death rates are
calculated. Specific death rates for different age group people are called age-specific death rates;
rates for males and females are called sex-specific death rates, rates for specific causes of death
are called cause-specific death rates. Rates may be specific for combinations of characteristics.
For example, age–sex– race-specific death rates are computed separately for each age group by
race and sex. Specific death rates are approximations of true probabilities. That is, the
denominator of the ratio is an estimate of the total number of events of a particular type that
could happen, while the numerator is a count of those that did happen. Specific death rates are
computed as follows:

Here, msd is the specific rate for any defined class, di is the number of deaths occurring in that
class for a given area and time, pi is the number of persons in the that class of the population for
the same area and time, and k is a constant, usually 100 000.
For cause-specific death rates, the denominator, pi, in the above formula is replaced by P, the
total population exposed to the risk of death. Therefore, a cause-specific death rate measures the
risk in the total population that is dying due to a specific cause of death.
The standardized death rate is abbreviated as SDR, is the death rate of a population adjusted to a
standard age distribution. It is calculated as a weighted average of the age-specific death rates of
a given population; the weights are the age distribution of that population.
As most causes of death vary significantly with people’s age and sex, the use of standardized
death rates improves comparability over time and between countries. The reason is that death
rates can be measured independently of the age structure of populations in different times and
countries (sex ratios usually are more stable).
Standardized death rates are calculated on the basis of the European standard population revised
by Euro stat in 2012 (and published in 2013).
As method for standardization, the direct method is applied. Standardized death rates are
calculated for the age group 0-64 ('premature death'), 65 years and more, and for the total of all
ages.
Here, SDRA is the standardized death rate for population in region A, Amx is the age specific
death rate at age x last birthday in population in region A and sPx the population exposed to the
risk of death at age x last birthday in the standard population.
Measurement of Fertility
Crude Birth Rate
The crude birth rate is the most frequently used overall measure of the reproduction of a
population. Like its counterpart, the crude death rate, it is influenced by many factors and
represents a proxy for more specific fertility measurements. It is calculated as follows:
This measure addresses the crude nature of the CBR by focusing on that section of the
population at risk of having births – women aged 15-49.
It is constructed as follows:

It can easily be constructed from vital registration, census or survey data and does not rely on
exact ages (except at beginning and end of age range).
This measure is a step above the previous two measures and a potentially useful and reasonably
accurate measure of fertility because the numerator and denominator are focused and well
specified and the denominator is truly those at risk of producing the numerator.
The General Fertility Rate (GFR) has specific uses in certain circumstances when one wants to
know a total number of births for all women in the fertile ages. An example would be in
calculations of maternal mortality – see PAPP104_S01. But in general, it is not widely used
because it still suffers a disadvantage concerning the structure of the population – similar to the
problem with the CBR although on a smaller scale.
This problem is that the fertile ages – nominally 15-49 years – is 35 years wide and so within
that range there can be substantial differences in age structure between populations. Because
fertility is concentrated at certain age’s populations can appear to have different levels of fertility
simply because they have different age structures between ages 15-49 years. For international or
secular comparisons, one wants to have a measure that is independent of age structure and the
GFR is not quite there. We need to move on to the Total Fertility Rate and a precursor to that is
the construction of Age-specific Fertility Rates.
The gross reproduction rate (GRR) is the average number of daughters a woman would have if
she survived all her childbearing years, which is roughly to the age of 45, subject to the age-
specific fertility rate and sex ratio at birth throughout that period. This rate is a measure of
replacement fertility if mortality is not in the equation.[1] It is often regarded as the extent to
which the generation of daughters replaces the preceding generation of women and so on and so
forth. If the value is equal to one that indicates that women will replace themselves. If the value
is more than one that indicates that the next generation of women will outnumber the current
one. If the value is less than one that indicates that, the next generation of women will be less
numerous than the current one.
The gross reproduction rate is similar to the Net Reproduction Rate (NRR), the average number
of daughters a woman would have if she survived her lifetime subject to the age-specific fertility
rate and mortality rate throughout that period.
The net reproduction rate (NRR) is the average number of daughters that would be born to a
female (or a group of females) if she passed through her lifetime conforming to the age-specific
fertility and mortality rates of a given year. This rate is similar to the gross reproduction rate
takes into account that some females will die before completing their childbearing years. An
NRR of one means that each generation of mothers is having exactly enough daughters to
replace themselves in the population. If the NRR is less than one, the reproductive performance
of the population is below replacement level.
The NRR is particularly relevant where sex ratios at birth are significantly affected using
reproductive technologies, or where life expectancy is low.
