Research
The first step is we need to thoroughly understand your business, your business goals,
your customers and their expectations and behavior, and how your website factors into
all of it. We’ll also look at the competitive landscape to establish context and
benchmarks for how your site should function and then improve on that. This stage will
give us the foundation for moving forward with just the right recommendations for the
design and development of your new creative website.

Design
Once we’re armed with this information, we’ll set to work sketching out interface layouts
for 3-5 different views, including the home page, sub pages, the services page, and any
additional templates needed further like e- commerce view. Wireframes look like simple
skeletons of your website without the visual polish of the finished design, allowing us to
experiment and iterate on solutions quickly so that we have a good sense of how
content should be laid out across different screen sizes.

Next we’ll design high-fidelity comps that show what the final designs will look like on
desktop, tablet, and smart-phone screen sizes, providing up to two rounds of revisions
on the design.

Development
Once you’ve approved the design, we’ll start building them using HTML, CSS, and
Javascript. A feature driven development of the project is used to complete this project.
Every valued piece of functionality is considered as a design feature and listed. The
schedule and budget are decided as per the number of the features implemented.

User Testing
Testing the designs with real users will greatly improve the quality of the finished design
and help catch usability issues before time is spent building out the designs. User
testing involves our usability professionals recruiting up to 5 users and asking them to
perform tasks. We record the results of the test and then deliver the video to you along
with a list of usability issues or comments that our design team can implement.

Support Retainer
We recommend retaining our services after the site is launched to assist with any further
support issues that arise. As part of this retainer we will also provide monthly analytics
reports and recommendations to further improve your site conversion.

REhAB ME is a home health services search engine. The admin intends to be the
Patient’s first authentic and trustworthy source of information for finding the right
House health service.

The patients are in an acute environment in the hospitals. After their Emergency
department care most of them are less likely to obtain follow-up. The reason being, lack
of awareness of a reliable after care home. Being in the comfort of one’s own home
provides intangible benefits that improve overall health.

The mission of REHAB me is to provide a standardized system to the home health

agencies for the aftercare patients, that provide a multitude of services.

T ECHNICAL O VERVIEW

Production Environment

The final tested version of the application will be deployed in the production
environment and will be available to the end users. This will also serve as the staging/QA
environment before the first release/Beta of the application.

Development Environment

The development environment will be used for developing and testing future releases of
the application. The version of the application deployed in the development
environment will not be available to the end users and will be accessible only to the
developers.

The development environment will also serve as the staging/QA environment for
subsequent releases of the application but will not be used for any performance related
testing since the server configurations in the development environment are not
intended to replicate the production environment and thus will not give an accurate
measure of how the application would perform in the production environment.

C HANGES AND R EVISIONS

We know from experience that fixed-price contracts are rarely beneficial to you, as they
often limit you to your earliest ideas. We don’t want to limit your ability to change your
mind or make decisions later when you might be better informed. The price at the
beginning of this contract is based on the length of time we estimate. We are needed to
accomplish everything you’ve told us you want to achieve, but we’re happy to be
flexible. If you want to change your mind or add anything new, that won’t be a problem
as we’ll provide a separate estimate for you to cover the additional work.

Charges for Services Performed

Functionality or feature requests above and beyond those listed in the budget and/or
the functionality specs may be considered out-of-scope and an amendment to the
budget will be recommended. Projects that go dormant for longer than 45 days will
incur fee to resume work at the discretion of Banyon Hub.

S IGNOFF AND A CCEPTANCE OF D ELIVERABLES
During the course of the project, Banyon Hub will make relevant deliverables available
to Client. Banyon Hub expects these deliverables to be reviewed, accepted and signed-

off by in the timeframe mentioned against each deliverable. If Client find defects and
represent the s defects in the deliverables, Banyon Hub will rework the deliverables for

acceptance.

G ENERAL T ERMS AND C ONDITIONS

The efforts estimation and planning is based on high level Business. Any
reduction/addition to this will be considered during requirement analysis and
knowledge transition phase and re-estimation will be carried out.

Any change in scope during the project execution of the project will be handled through
the Change Management process

Banyon Hub assumes that Client would provide one functional expert(s) during
Requirement analysis and Knowledge Acquisition phase.

Cost will not include cost of domain registration, hosting, payment gateway charges,
data migration from old/existing site to new site and server management charges etc.

Cost Estimation is valid for 30 days from the date of proposal has been sent. Then after
the cost is subject to be varied as per the change of the company policy & other terms.

Banyon Hub expects that Client will provide timely input/feedback for all reviews and
queries raised during the project execution. It is expected that Client will resolve queries
in reasonable time frame. Any delay can result in slippage of the schedule.

Banyon Hub assumes that all sign-offs from Client will be provided within agreed and
specified timeframe. Such time frame can be decided at the beginning of each key
milestone/activity

All the documentation will be provided in English.

It is requested to the clients provide confirmation/approval as & when required within 2

business days.

This proposal is valid for 30 days unless extended by Banyon Hub.

In case Client requires any extension of the proposed acceptance schedule, the
associated effort and cost of such extension can be mutually reviewed.

Banyon Hub may display the project on its portfolio after completion of successful
project and may get a credit of the work as developed and designed for the site as a
part of this contract unless it has been provided in written.

For any circumstances if project needs to be put on Hold / Stop it requires minimum
request notice period of 1- 2 weeks along with duration for which request will be
addressed by management and final decision on the request will be based on that.

Source code will only be delivered or uploaded on the live server once the due payment
is made.

A CCEPTANCE C RITERIA AND P ROCEDURE
The Client will perform acceptance testing of the deliverables on their systems. Banyon
Hub design team will assist the acceptance-testing phase. Any issues reported during
the Acceptance Testing will be analyzed and categorized as "Defects" or "Change
Requests (CRs)". Problems arising due to the defects in the realization from the starting
points will be categorized as Problem Reports and rectified by Banyon Hub. Banyon Hub
will attempt to solve the problems reported at the earliest. In case the estimated time
for solving the problems is going to be significant, it will be communicated to Client.

Formal Acceptance Testing will start after Banyon Hub sends the final delivery. Banyon
Hub will make out a formal Delivery Note to the Client. Client will acknowledge, review
the work and approve the delivery as complete and acceptable or incomplete and
unacceptable, within 7 working days of delivery being made as mentioned below. If
rejected, Banyon Hub will rework the software to requirements.

The scheduled acceptance period, for the completed delivery, would depend upon the
release.

Banyon Hub anticipates the acceptance period to be a total of 1 week for the UAT.

Banyon Hub uses following defect classification:

Type of Error Description

P0 Fatal Error. Runtime error occurs which
causes the application to crash.
P1 No Fatal Error, but functionality differs
from the specification.
P2 Error wherein the software works but a
degree of in- convenience is caused.
Correction is not deferrable and an
easy work around exists.

Contract Reviews to ensure clear understanding and identification of customer
