    I, _________________________, of legal age, single, Filipino and resident of


    _____________________________, under oath, do hereby depose and say that:

    1. I am an actual tiller of a particular agricultural land


    and is currently applying for an Agricultural
    Competitiveness Enhancement Fund (ACEF) at the
    Land Bank of the Philippines, San Gabriel Branch,
    Tuguegarao City;

    2. That I have no outstanding loans from any lending


    institutions for the same project;

    3. I am executing this affidavit to attest the truth of the


    foregoing statements in support of my aforesaid
    application and for whatever legal purpose it may
    serve;

    AFFIANT FURTHER SAYS NAUGHT.

    IN WITNESS WHEREOF, we have hereunto set our hands, this 1st day of July
