WAIVER OF RIGHTS
I, RHEA ALFORQUE AMOR, Filipino, of legal age, single, and with residence and post
office address at Purok 5, Cangmating, Sibulan, Negros Oriental Philippines, after having been
first duly sworn to in accordance with law, do hereby depose and say:
1) That I pay the down payment of P10,000.00 to Lady Bianca Funeral Services
with office address at National Highway, Tubtubon, Sibulan, Negros Oriental,
dated March 13, 2019 and with Official Receipt No. 4592;
2) That I hereby waived all my rights, claims the bereavement assistance from
Overseas Workers Welfare Administration (OWWA) in favor of the claimant
"Flordeliza Alforque Amor";
3) That I hereby declare that I have fully read and understood the contents of
this document and that this Waiver of rights, is made freely and voluntarily, with
full knowledge of my rights under the law.
4) As such, I execute this Affidavit to attest to the truth of the foregoing facts, and
for whatever legal purpose that this Affidavit may serve.

I, FLORDELIZA ALFORQUE AMOR, of legal age, Filipino, widow, and a resident of Purok 5,
Cangmating, Sibulan, Negros Oriental, Philippines, do hereby name, constitute and appoint Ms.
RHEA ALFORQUE AMOR, of legal age, Filipino, single and a resident of Purok 5, Cangmating,
Sibulan, Negros Oriental, Philippines, to be my true and lawful attorney in-fact, to act for and in
my stead, to do and perform the following acts deeds and things, to wit:
1. To appear, transact, and negotiate for and in my behalf with Overseas Workers
Welfare Administration (OWWA) Region VII Office, Cebu City, with regards the
processing of my deceased husband's, the late CONRADO SALVADOR C. AMOR,
bereavement assistance;
2. To follow-up and process the release of my above benefits with Overseas Workers
Welfare Administration (OWWA) Region VII Office, Cebu City;
3. To claim, receive and in-cash check/s for the bereavement assistance of my late
husband, from the above mentioned agency;
4. To make, sign, execute, deliver and received documents and other writings of
whatever nature or kind, or do such any other act which may be necessary or
incidental in carrying into effect the foregoing authority.
HEREBY GIVING AND GRANTING unto my said attorney-in-fact full power and authority to do
and perform any and every act, and thing whatsoever requisite and necessary to be done in and
about the premises, as fully to all intents and purposes as I might or could lawfully do if
personally present, with full power of substitution and revocation, and hereby ratifying and
confirming all that my said attorney-in-fact shall lawfully do or cause to be done by virtue of
these presents.
