Background:
GHAZAL 1 MONTH 12 days old female infant , LATE PT 35 WKS , NICU ADMISSION FOR 1WK, CAME TO
our ER in MCH –Buraidah on 3/11/2019 early morning WITH 1 DAY HX OF URTI SYMPTOMS , SOB,
COUGH AND SECRETIONS , History of contact to other sick people having URTI at home .AT ER SHE
DEVELOPPED 1 ATTACK OF APNEA AND CYANOSIS FOR SECONDS IMPROVED WITH STIMULATION AND
SUCTIONING where for this issue patient admitted to PICU for close observation but at 8:00 AM patient
developed another attack of apnea with desaturation and bradycardia so patient connected to HFNC
WHICH UPGRADE TOP noninvasive ( BIPAP after developed another attack of apnea while on HFNC at
around 4 pm .) . but after connected to noninvasive , again the patient developed another attack of
apnea with desaturation and bradycardia , so was intubated and connected to mechanical ventilation .
patient was intubated on 3/11/2019 ( 4:30 pm ) , connected to mechanical ventilation VC mode then
changed to simv/vc with initial settings TV 21 peep 6 VR 45 fio2 40%and blood , throat and nasal
cultures were taken. Also, H1N1 and MERscov were sent . Patient started on cefuroxime and tamiflu.
Patient on mechanical ventilation now D6, hemodynamic stable on full NGT feeding.
-CNS: GCS 11/15, PUIPLS 2MM EQUAL AND REACTIVE, AF AT LEVEL, no abnormal movements. Patient on
morphine PRN
-RESP: BILATERAL EQUAL AIR ENTRY, TRANSMITTED SOUNDS, CREPITATION, SECRATION,, last BLOOD GAS
PH 7.27 PCO2 57 HCO3 23 . PATIENT NOW SHIFTED TO SMIV VC + PS PEEP 7, VR 35, TV 20, PS 12.
CXR: patient having wondering upper lobe lung collapse. PATINET HAVING PLENTY, COPIUES SECRETION
