BMI/HFA Software using SF8 form

 
How to use it?
1.  Guide sheet will serve as your guide on how to use this program.
    2.  Summary sheet is an automatic computation of Summary by One Grade Level only.
    3.  Always start from the sheet number “1”.
      - Automatic computations of BMI and HFA by using SF8 form.
      - Each sheet represents one section; you can rename them if you want from sheet "1" to the
        name of the section.
      - Each sheet can accommodate a maximum of 32 Male & 32 Female for a Total of 64 pupils  or
        students but unused rows can be hidden. If more than 32 Male or Female, you can add them in 
        the next sheet according to their sex. Please read step 4 if additional sheet is needed).
- IMPORTANT: Please type/edit the Date of Weighing by using Short Date format (mm/dd/yyyy),
        the age computation is based here!
      - Edit the upper boxes for school detail, encode the Name of Students, Birthday (mm/dd/yyyy),
Weight and Height.
- If below 5 yrs. old, no age, no BMI, no HFA, and not be counted in the computations,
        and if above 19 yrs. old, no HFA (it appears "Adult").
 
    4. Add sheets if needed! 
      - Pls. follow the instructions on step 5.
Note: - Do not add sheet by not following the "step 5" the automatic computations may not work!
           - Always copy from sheet 2 and insert added sheets between sheet "1" and "Last". 
           - Never add sheets before sheet "1" or after the "Last" sheet or else, the data on that sheet 
             may not be included in the Summary. 
           - Do not use cut/paste in any part of the program.
           - The most important Do not Delete any part of the program!
 
    5. How to add another sheet?                    How to add multiple sheets?
       1. Right click a blank sheet                1. Click sheet “2”, then “SHIFT” and the last sheet!
       2. Click "Move or Copy"                2. Follow step 2 to 6 of “How to add another sheet?”
       3. Click "Last"
       4. Check the "create a copy box"
       5. Click "OK" or ENTER
       6. Rename the new sheet
