    WAIVER AND RENUNCIATION OF


    HEREDITARY RIGHTS AND PROPERTIES

    KNOW ALL MEN BY THESE PRESENT:

    I, xxx, of legal age, Filipino, married, and a resident of xxx, after having been sworn in
    accordance with law, do hereby deposes and state that;

    1. I am the legal surviving spouse of xxx who died on September 30, 2019 due to xxx;

    2. By reason of such death and pursuant to Art. 777 and to Art. 887, Book III of the Civil
    Code, successional rights have been transmitted to me by operation of law and, thus
    entitled to whatever property, whether due or otherwise, that my deceased spouse may
    have including proceeds from insurance, pensions, gratuities, benefits and other
    endowment.

    3. Pursuant to Art. 6 of the Civil Code which provides for waiver of any rights provided
    that the same not be contrary to law, public order, public policy, morals, or good
    customs, or prejudicial to a third person with a right recognized by law, I hereby
    expressly waive, transfer and convey whatever successional rights as a compulsory heir
    that I may have.

    4. This waiver and renunciation of successional rights is being made in favor of our
    legitimate children as an act of love, generosity and liberality. As a result of this waiver
    and renunciation, whatever successional rights I may have or whatever property or
    amount due to me as a compulsory heir be transmitted and conveyed to our legitimate
    children.

    5. This waiver, transfer and conveyance is with prejudice to any other successional rights,
    including hereditary properties, which may be due to me as a compulsory heir.

    6. This waiver, transfer and conveyance is accepted and received by our legitimate
    children with appreciation and gratefulness for the love, kindness and generosity.

    7. I have no more claims whatsoever arising from successional rights that I may have
    arising from the death of my spouse.

    8. I am executing this waiver and renunciation consciously without fear, violence or


    intimidation or undue influence, with full knowledge of the resulting imports and to
    attest to the truth of the foregoing facts and statements.

    IN WITNESS WHEREOF, I have hereunto set my hands on this ___ day of ______ 2019, at Quezon City,
